A case of gestational diabetes complicating the pregnancy of a hypoparathyroid gravid patient is presented. Labor was induced by prostaglandin E2 vaginal tablets, but delivery was by cesarean section. Separate treatment of the two endocrinopathies was successful. After the induction of labor, hypocalcemia and hypoglycemia were seen, and they may have been caused by the prostaglandin E2.

The authors analyzed results of 295 labors induced with endocervical application of dinoprostin (152 term- and 143 postterm labors) in relation to 242 (125 term- and 117 postterm-) spontaneous labors. Delayed labor increases the occurrence of meconial amniotic fluid (21.96%:9.03% p < 0.01), and pathological and prepathological CTG records (23.85%: 11.97% p < 0.005). OBJECTIVE: To compare induction of labor methods in patients attempting a trial of labor after cesarean (TOLAC) with an unfavorable cervix.
METHODS: This is a retrospective cohort study from patients attempting TOLAC from 2009 to 2013. Patients with a simplified Bishop score of three or less where labor was initiated with either a Cook balloon or oxytocin were included. Our primary outcome was mode of delivery. Our secondary outcomes included duration of labor and multiple maternal and neonatal morbidities.

BACKGROUND: Hysteroscopic metroplasty has become the method of choice for the treatment of uterine septa. Uterine perforation has been reported in about 1% of surgical hysteroscopic procedures. Ultrasound allows the detection of uterine lesions.
CASE: A woman who conceived after complicated hysteroscopic metroplasty underwent emergency cesarean section because of uterine rupture during labor induced with prostaglandins (PGE2). An ultrasound scan performed two years later revealed a uterine lesion that corresponded to the myometrial tear reported at cesarean section.

In a prospective randomized study, pregnancies with unfavorable cervix and well established gestational age of at least 42 weeks were selected for management by either antepartum fetal testing or prostaglandin gel induction of labor. A total of 615 women with hypertensive disorders during pregnancy were the study subjects. Of these, 145 (23.58%) women had HELLP or partial HELLP (only one or two of the three components H,EL, LP).
