In late 2014, 56 hospitals in the United States were designated by state and federal public health authorities as specially designed high-level isolation units (HLIUs) equipped with advanced infrastructure, laboratory capabilities, and trained staff to care for patients with highly hazardous communicable diseases (HHCDs), such as Ebola virus disease. This survey describes the clinical laboratory support capabilities of U.S. HLIUs, including the specific test menus that HLIUs have identified to safely manage HHCD patients and the locations where such testing would be performed. In spring 2016,

BACKGROUND: In the wake of the coronavirus disease 2019 (COVID-19) pandemic, there have been dramatic changes in the testing protocols for respiratory illness. Laboratory identification of respiratory pathogens causing clinical symptoms has become more important in the post-COVID-19 era, both for epidemiological and clinical reasons. This is particularly the case for respiratory syncytial virus (RSV) where a number of preventatives are at the point of clinical translation. The laboratory diagnosis is one of the critical elements in understanding the disease burden of respiratory pathogens and

CONTEXT: Human immunodeficiency virus (HIV) RNA testing (viral load testing) is increasingly important in the care of patients infected with HIV-1 to determine when to initiate, monitor, and change antiretroviral therapy. Patient viral load testing information is communicated to the clinician through the laboratory test report.
OBJECTIVES: To examine the format and information used in reporting viral load testing results and determine the clarity of the information provided in these reports.
DESIGN: Patient test reports with all personal identifiers removed were requested of viral load testi

Hospitals must find ways to decrease costs in the face of decreasing reimbursements. The cost of blood products and special testing services provided by a regional blood center may be a source of excessive expense. Our facility has had its own blood donor program since June of 1950 and has achieved significant cost savings by collecting and processing blood for the facility rather than buying it from an outside agency. This article describes the benefits and disadvantages of having a hospital-based blood donor program. Benefits include cost savings, safety, better customer service for patients

BACKGROUND: Serological assays designed to detect SARS-CoV-2 antibodies are being used in serological surveys and other specialized applications. As a result, and to ensure that the outcomes of serological testing meet high quality standards, evaluations are required to assess the performance of these assays and the proficiency of laboratories performing them.
METHODS: A panel of 60 plasma/serum samples from blood donors who had reverse transcriptase-polymerase chain reaction (RT-PCR) confirmed SARS-CoV-2 infections and 21 SARS-CoV-2 negative samples were secured and distributed to interested

Only few customers consult with their using commercial laboratories about how to interpret the laboratory examination results, or how to proceed the laboratory diagnosis. But there are many questions about the test item, methodology, time course, and how to collect a specimen, and also there are many complaints about the test results, methods, reference values and reporting styles. Therefore, most of the commercial laboratories set up the information desk instead of the consultation desk. In our laboratory, Japan clinical laboratories, Inc., information services are done by not only the inform

We used population-based data to evaluate how often groups of randomly selected clinical laboratories accurately estimated the prevalence of resistant pneumococci and captured trends in resistance over time. Surveillance for invasive pneumococcal disease was conducted in eight states from 1996 to 1998. Within each surveillance area, we evaluated the proportion of all groups of three, four, and five laboratories that estimated the prevalence of penicillin-nonsusceptible pneumococci (%PNSP) and the change in %PNSP over time. We assessed whether sentinel groups detected emerging fluoroquinolone r

BACKGROUND: The cumulative costs of diagnostic and interventional procedures in the catheterization laboratory in public hospitals are still largely unknown, notwithstanding the present stress upon the cost-effectiveness issues in medicine.
METHODS: From January through April 2000 we have evaluated procedures in the catheterization laboratory of the Ravenna USL. Costs taken into consideration were the following: the mean cost of materials for each type of examination and of medications used for each patient, personnel costs, machinery mortgages' costs, ambulance transportation's costs, and ge

At least one full-time doctor in laboratory management and consultation on data analysis is required to receive laboratory examination management fee III or IV according to the rules for Japanese public health insurance medical fees. A qualified pathologist, board-certified as a clinical laboratory physician cannot receive this fee together with the pathological diagnosis management fee even if he or she manages laboratory examinations. As a result of this regulation, surgical pathologists working in laboratory examinations are gradually decreasing in Japan; however, it is possible for surgica

Although transplantation of solid organs has become a more standardized method of treatment, liver transplantation represents an exceptional multidisciplinary clinical procedure requiring understanding of specific pathophysiological changes that occur in the end stage of liver disease. Liver transplantation has been performed at Merkur University Hospital since 1998, with 360 transplantations performed to date. The most common indications are alcohol liver disease, cirrhosis caused by hepatitis B and C virus, hepatocellular carcinoma and cryptogenetic liver cirrhosis. Laboratory tests required

Molecular typing is of considerable importance for the surveillance and epidemiology of human transmissible spongiform encephalopathies (TSEs). It relies on the detection of distinct protease-resistant prion protein (PrP(Sc)) core fragments that differ in molecular mass and/or glycoform ratio. In this collaborative study, we tested the inter-laboratory agreement in TSE molecular typing. Sixteen characterized brain specimens from sporadic TSEs and variant Creutzfeldt-Jakob disease (vCJD) cases were distributed blindly to seven laboratories for molecular characterization by a defined protocol an

Urine of diabetics without concomitant diseases with a certain concentration of glucose was collected for preparing reference material. Water saturated with phenol (872 mmole/liter) was added to urine pool during intensive stirring on a magnetic stirrer at 20 degrees C, so that the final concentration of phenol were 50 mmole/liter. Glucose content in the resultant reference material remained stable for 130 days at 18-20 degrees C. For glucose measurements, 2 ml reference material and 0.5 ml 2.5 M sodium oxide hydrate are put in centrifuge tubes, stirred, put on boiling water bath for 2 min, co

Laboratories provide information beyond test results, including information related to patient preparation, specimen collection and handling, test methodology, test availability, and interpretation of results. Most laboratories publish reference manuals to distribute this information to clients, while relying on the laboratory information system to provide this information to laboratory staff. Maintaining duplicate sources of information is expensive and error-prone, and printed materials become rapidly outdated. We developed a process to automate the production of a web-based reference manual

We reviewed laboratory-acquired infections occurring in Utah from 1978 through 1982. Written and telephone interviews of supervisors of 1,191 laboratorians revealed an estimated annual incidence of 3 laboratory-acquired infections per 1,000 employees. Infections, in order of frequency, included hepatitis B (clinical cases), shigellosis, pharyngitis, cellulitis, tuberculosis (skin test conversion), conjunctivitis, and non-A, non-B hepatitis. One-half of large laboratories (over 25 employees), but only 12% of smaller laboratories, reported infections. The annual incidence, however, at smaller la
