Passing return to sports tests after ACL reconstruction is associated with greater likelihood for return to sport but fail to identify second injury risk.

BACKGROUND: A limited number of patients return to sport (RTS) after an anterior cruciate ligament reconstruction (ACLR) and patients who RTS have a relatively high risk for second ACL injury. The purpose of the current study was to compare the results of a test battery between patients who returned to the pre-injury level of sport (RTS group) and patients who did not (NO-RTS group). It was hypothesized that the RTS group showed better test results. METHODS: Sixty-four patients (age 27.8 ± 8.8 years) were included. The results of a multicomponent test battery (jump-landing task assessed with

Vascularized fibula graft for the traumatically induced long-bone defect.

Long-bone defects resulting from trauma can be managed with a variety of techniques, including conventional bone grafting, distraction osteogenesis, bone graft substitutes, prosthetic devices, and vascularized bone grafting. Although there is an array of available methods for long-bone reconstruction of bony defects, vascularized bone transfer is particularly useful in large defects (> 6 cm) and in cases in which osteomyelitis and unstable soft-tissue or beds make conventional techniques difficult.

Serious lower limb injuries following car gearbox explosion: A rare case report.

Car Gearbox explosion-related injuries are very rare but could be serious. We reported a 43-year-old female patient who sustained severe bilateral lower limb injuries following an explosion of the gearbox in her private car while waiting at a traffic signal. The patient suffered from a left open tibia shaft fracture and severely macerated soft tissues, along with an open wound on the right leg. This case report highlights the unpredictable and serious nature of an incident due to a rare car gearbox explosion and the challenges associated with its management and outcomes.

Radiculopathy as Delayed Presentations of Retained Spinal Bullet.

Bullet injuries to the spine may cause injury to the anatomical structures with or without neurologic deterioration. Most bullet injuries are acute, resulting from direct injury. However, in rare cases, delayed injury may occur, resulting in claudication. We report a case of intradural bullet at the L3-4 level with radiculopathy in a 30-year-old male. After surgical removal, radicular and claudicating pain were improved significantly, and motor power of the right leg also improved. We report the case of intradural bullet, which resulted in delayed radiculopathy.

Nonoperative management of athletic ankle injuries.

Few injuries in sports are more ubiquitous than those involving the ankle. Athletes in some endeavors, notably football and basketball, routinely have their ankles prophylactically taped at a cost of hundreds of thousands of dollars and, probably, millions of man hours. Other sports, such as skiing, involve encasement of the foot, ankle and lower leg in plastic and foam to the extent of almost complete exclusion of motion. In spite of these rather heroic measures, ankle injuries continue to constitute a significant threat to athletes in these and most other activities.

Correlation Between Synovial Fluid Biomarkers and Leg-Fat Area Ratios in Patients Undergoing ACL Reconstruction With or Without an Associated Meniscectomy.

PURPOSE: This study attempts to establish whether local adiposity of the knee at the level of the joint line is associated with alterations in synovial fluid biomarker concentrations in patients undergoing ACL reconstruction with or without an associated meniscectomy. METHODS: Patients undergoing ACL reconstruction were prospectively enrolled at the time of surgery from July 2011 to January 2015. Synovial fluid samples were collected just prior to incision and the concentrations of 10 biomarkers of interest were determined using a multiplex magnetic bead immunoassay. Knee adiposity was assess

Using a Y-shaped vein graft with drain-out branches to provide additional arterial sources for free flap reconstruction in injured lower extremities.

BACKGROUND: Reconstruction of the distal lower extremity with poor arterial circulation is a challenge for reconstructive surgeons, especially when a microsurgical free tissue transfer is necessary. One may face the problem of having no suitable recipient artery to be used except the main one. To overcome this difficult situation, utilization of a Y-shaped vein graft (YVG) was developed. METHODS: We report on 3 successful reconstructions of the lower extremities with free flaps using YVG to supply the arterial inflow and in which the distal circulation of the lower extremity was preserved at

Lower extremity electromyographic characteristics of patients with noncontact anterior cruciate ligament rupture in one-legged jump landings: a case-control study.

There have been many studies on neuromuscular adaptation after anterior cruciate ligament (ACL) reconstruction, while the understanding of muscle activation patterns in unreconstructed patients with ACL rupture is still limited. The aim of this study was to investigate the lower limb electromyographic characteristics of unreconstructed patients with complete ACL rupture in a single-legged hopping landing task in order to deepen the understanding of motor control strategies in the ACL-deficient state and to provide a reference for rehabilitation assessment and intervention. Forty-two subjects w

Real-time feedback on knee abduction moment does not improve frontal-plane knee mechanics during jump landings.

Excessive knee abduction loading is a contributing factor to anterior cruciate ligament (ACL) injury risk. The purpose of this study was to determine whether a double-leg landing training program with real-time visual feedback improves frontal-plane mechanics during double- and single-leg landings. Knee abduction angles and moments and vertical ground reaction forces (GRF) of 21 recreationally active women were quantified for double- and single-leg landings before and after the training program. This program consisted of two sessions of double-leg jump landings with real-time visual feedback o

Femur fractures and hemorrhagic shock: Implications for point of injury treatment.

BACKGROUND: Femur shaft fractures (FSF) are perceived as potentially life-threatening injuries due to significant blood loss. However, these injuries are rarely the sole cause of hemorrhagic shock. Clinical practice guidelines for the prehospital management of FSF are inconsistent, especially concerning the use and timing of traction splinting which is postulated to reduce bleeding. We sought to understand the association between FSF and shock, and identify risk factors for shock among casualties with FSF. METHODS: This is a retrospective analysis of trauma casualties treated by Israeli Defen

[Surgical treatment of lower extremity peripheral nerve injuries].

Peripheral nerve injuries of the lower extremities are not frequent. The most common are traction injury of the peroneal nerve at the knee level or iatrogenic trauma of the pelvic nerves during abdominal surgery. Civil sharp injuries are rare.Indications for surgical revision follow the general rules of nerve surgery. Sharp injury should be treated as soon as possible, ideally within 72 hours. Closed lesions are indicated for surgery if a complete denervation remains unchanged three months after the injury. Best results can be achieved within six months from the injury. Irritations caused by b
