OBJECTIVE: To review the history of the treatment of leprosy and leprosy reactions after World War II.
METHODS: Treatments based on experience and clinical evidence are compared with those advised by the WHO in their quest to eliminate leprosy by the year 2000, later extended to 2005.
RESULTS: Leprosy is not eliminated. Analyses of data on reaction treatment suggest that the treatment regimens for leprosy reactions as advised by the WHO may lead to more impairment among leprosy patients than the 'old' established regimes.
CONCLUSION: WHO policies to eliminate leprosy may have jeopardized th

A 29 year-old Bengali male case is presented in this paper which was a borderline tuberculoid leprosy (BT) at detection. His father contracted a lepromatous leprosy of G = 2 deformity. He took anti-leprosy drugs including MDT/MB regimen irregularly and had maculae widely-spread with anesthesia 16 months after being released from treatment (RFT). The histopathology of the maculae unexpectedly showed that of an indeterminate group of leprosy. The recurrent skin lesions were susceptive to a four-week regimen of Rifampicin and Ofloxacin. This case can not be defined as a relapsed case, because sli

Previous studies have shown that when multibacillary leprosy patients were treated with recombinant human interferon gamma (rhuIFN-gamma) for 6-10 months there was an accelerated reduction in the number of acid-fast bacilli in the skin at the site of injection as well as an accelerated bacillary reduction at distal sites. However, this favorable out-come of IFN-gamma treatment was associated with the development of erythema nodosum leprosum (ENL). The present study was undertaken to investigate whether rhuIFN-gamma-induced bacillary clearance could be disassociated from the induction of ENL. r

BACKGROUND: Mycobacterium abscessus is the second most common nontuberculous mycobacterium respiratory pathogen and shows in vitro resistance to nearly all oral antimicrobials. M abscessus treatment success is low in the presence of macrolide resistance.
RESEARCH QUESTION: Does treatment with amikacin liposome inhalation suspension (ALIS) improve culture conversion in patients with M abscessus pulmonary disease who are treatment naive or who have treatment-refractory disease?
STUDY DESIGN AND METHODS: In an open-label protocol, patients were given ALIS (590 mg) added to background multidrug

We report on granulomatous infections of the skin, as well as the current state of therapeutics. Cutaneous tuberculosis and presently desired therapeutic regiments are treated in detail. Diseases regarded as eradicated in Germany, such as brucellosis and listeriosis, do still occur. Their recognition and cure are discussed from a modern point of view.

This study aims to formulate Clofazimine (CLOF)-loaded nanostructured lipid carriers (NLCs) for transdermal application, thereby improving the overall efficacy of the drug. NLCs loaded with clofazimine were developed using biocompatible lipids, characterised w.r.t. particle size, PDI and % entrapment efficiency and optimised using 'Box-Behnken design'. The optimum formulation was assessed for in vitro drug release, dermatokinetics & in vivo biocompatibility study. The characterisation of NLCs formulation revealed their globular shape with a particle size of around 192 nm, zeta potential of app

Leprosy primarily affects the skin and peripheral nerves, with neuropathy usually accompanying skin lesions. Rarely, neural involvement may precede cutaneous manifestations, leading to diagnostic delay. We report the case of a 36-year-old male presenting with bilateral lower limb sensory motor neuropathy initially attributed to Vitamin B12 deficiency. Despite normalization of B12 levels, symptoms persisted, and subsequent skin changes led to biopsy confirmation of borderline tuberculoid leprosy. This case emphasizes considering Hansen's disease in patients with unexplained neuropathy, particul

A 43-year-old woman of Mayan origin from Quintana Roo, Mexico, was diagnosed with diffuse lepromatous leprosy. The etiologic bacillus was determined to be Mycobacterium lepromatosis instead of Mycobacterium leprae. This case likely represents the first report of this leprosy form and its agent in the southeastern tip of Mexico.

In developed countries, Hansen disease, or leprosy, is a rare and little-known disease. Over the last few years, its prevalence in New Caledonia has remained stable (0.35 per 10,000 inhabitants). We report the case of an 11-year-old child who presented lepromatous leprosy complicated by a type 2 reaction. Despite appropriate treatment, the course was unusual with fever lasting a few weeks associated with asthenia, weight loss, and biological perturbations such as inflammatory syndrome, anemia, and hyperferritinemia. After a brief review of Hansen disease and its complications, we discuss the d

Edema, which is commonly described as a symptom of reactional states, may occur during the course of leprosy. Both diagnosis and adequate treatment measures are often difficult to achieve and failure to do so may result in permanent damage to the lower limbs. In a one-year follow-up study of leprosy patients--ten multibacillary and one paucibacillary--who had been submitted to a clinical protocol for diagnosis and pathological classification, a clinical pattern of localized and/or systemic edema was observed. Among these patients, five simultaneously presented other symptoms related to reactio

Thirty nine cases of borderline tuberculoid leprosy having nerve abscesses (15 with sinuses) were treated with daily dose of rifampicin and isoniazid for six months along with standard multidrug therapy. The patients were followed up for three to five years. No recurrence of abscess or sinus was observed. Observations indicate that medical approach is required at times to supplement surgical intervention for management of these cases.

A double blind controlled clinical trial to assess the role of anti-histamines as a supplement in the treatment of leprosy was conducted in multi-bacillary cases of leprosy. In all, 120 patients with lepromatous or borderline leprosy were randomly allocated to a regimen of clofazimine and dapsone for 12 months with or without a supplement of pheniramine maleate for the first 3 months. During the 12-month period, 92% of the patients who received the supplement and 86% of the patients who had not received it had moderate or marked clinical improvement. The BI values decreased from 4.1 to 3.4 and

Nigeria continues to report over 2000 new leprosy cases annually. This retrospective study reviewed leprosy case records from 2006 to 2023 at a teaching hospital (TH) and a leprosy treatment referral centre (LTRC) in Lagos, Nigeria. Demographic, clinical and treatment data were analysed and challenges highlighted. At the TH, 96 patients with leprosy were seen. The median age was 34.6 years (range 11-79); 61 (64%) were male; and 75% had multibacillary disease. At the LTRC, 354 patients were seen. The median age was 39.8 years (range 7-85); 230 (65%) were male; and 349 (98.6%) had multibacillary

INTRODUCTION: Dapsone hypersensitivity (DHS) is a potentially fatal and severe cutaneous adverse reaction that occurs in patients taking dapsone. As leprosy, for which dapsone is used as part of multidrug therapy, usually occurs in countries with resource limitations, the morbidity and mortality caused by DHS are more dreadful. Herpesviruses (especially HHV-5 and 6) are frequently associated with the etiology of drug-induced hypersensitivity reactions, and the reactivation of these viruses coincides with the reflare of clinical symptoms even after the cessation of culprit drugs.

BACKGROUND: Early detection of nerve impairment and its progression is critical in all leprosy cases, regardless of spectrum and treatment. This study aims to identify subclinical neuropathy at presentation in newly diagnosed leprosy patients and to monitor its progression after initiating multidrug therapy (MDT) using a combination of clinical nerve palpation, small fibre neuropathy tests (e.g. vibration, temperature, touch, pain, sweat), monofilament testing (MFT), voluntary muscle testing (VMT) and nerve conduction studies (NCSs).
METHODS: We studied 38 nerves among 19 patients and followe

BACKGROUND: Leprosy is one of the neglected tropical diseases associated with significant morbidity in endemic regions. It causes disability affecting the daily activities and social participation of affected individuals. Understanding the prevalence and trend of leprosy-related disability throughout the world and the accuracy of disability data counted by WHO is crucial in guiding efforts to be made towards the targets set by WHO to be achieved by 2030. This study aims to show the significance of reporting leprosy-related disability at the end of MDT and critique how disability is counted in
