Linezolid-resistant Staphylococcus aureus strain 1128105, the first known clinical isolate possessing the cfr multidrug resistance gene.

The Cfr methyltransferase confers resistance to six classes of drugs which target the peptidyl transferase center of the 50S ribosomal subunit, including some oxazolidinones, such as linezolid (LZD). The mobile cfr gene was identified in European veterinary isolates from the late 1990s, although the earliest report of a clinical cfr-positive strain was the 2005 Colombian methicillin-resistant Staphylococcus aureus (MRSA) isolate CM05. Here, through retrospective analysis of LZD(r) clinical strains from a U.S. surveillance program, we identified a cfr-positive MRSA isolate, 1128105, from Januar…

Linezolid activity against disseminated Listeria monocytogenes meningitis and central nervous system abscesses: focus on early drug myelotoxicity.

A very infrequent report of Listeria monocytogenes meningitis and multiple subtentorial abscesses (cerebellar, bulbar, and pontine in location), occurred in an immunocompetent man, and is described and discussed on the ground of the most recent literature evidences regarding the frequency, presentation, and outcome of this disease. The possibility to resort to combination linezolid therapy in patients who appear to be not responsive to first-line treatment of cerebral abscess of definite origin, and the appearance of anemia as an early untoward event of this novel oxazolidinone antibiotic, are…

Successful treatment of disseminated cerebritis complicating methicillin-resistant Staphylococcus aureus Endocarditis unresponsive to vancomycin therapy with linezolid.

A unique case of community acquired methicillin resistant Staphylococcus aureus (MRSA) sepsis, with endocardial and cerebral metastatic seeding, caused by a strain representative of the Italian clone, is described. The patient was a 47-y-old man without apparent risk factors for endocarditis and for MRSA infection who developed coma with multiple cerebritis lesions under vancomycin plus amikacin therapy. He was eventually cured with the addition of linezolid to the initial antimicrobial regimen. This observation seems to confirm previous reports of the efficacy of linezolid for the treatment o…

Prediction model of thrombocytopenia on patients using linezolid: a ranked-based covariate selection with multi-center data.

BACKGROUND: Thrombocytopenia is a common adverse reaction of linezolid, often leading to severe complications. It is challenging to externally validate directly apply developed in other countries. We aim to develop and validate a risk prediction model of linezolid-associated thrombocytopenia (LAT) tailored to Vietnamese setting and to construct a simplified risk score calculation to support clinical decision-making. MATERIALS AND METHODS: Data was collected retrospectively from three large hospitals in Northern Vietnam. We selected inpatients treated with linezolid from November 2019 to March…

Single Saliva Sample Linezolid Dosing for Multidrug-Resistant Tuberculosis: A Population Pharmacokinetic Modelling of Plasma and Saliva.

BACKGROUND AND OBJECTIVE: Therapeutic drug monitoring (TDM) of linezolid for treating multidrug-resistant tuberculosis (MDR-TB) is recommended but is hindered by the invasive and logistical challenges of plasma sampling. Saliva is a promising alternative, but a saliva pharmacokinetic model to inform dosing is lacking. This study aimed to develop a saliva-based population pharmacokinetic (popPK) model and evaluate limited sampling strategies for linezolid in MDR-TB. METHODS: Plasma and saliva samples were collected at pre-dose, 2-, and 5-h post-dose from adults treated with linezolid for ≥ 7 d…

Ongoing independent evolution of linezolid and vancomycin-resistance pELF-type linear plasmids across the One Health spectrum.

Vancomycin and linezolid are crucial for treating enterococcal infections. The global emergence of linezolid- and vancomycin-resistant enterococci is a growing concern. Enterococcal pELF-type linear plasmids, which confer both types of resistance, have been identified worldwide from clinical and environmental sources. Phylogenetic and structural analyses of pELF-type linear plasmids revealed that they diverged into two distinct lineages and evolved through the integration of circular plasmids. Our findings on this ongoing evolution advance the fundamental knowledge required for the future deve…

Exploring the impact of baseline platelet count on linezolid-induced thrombocytopenia: a retrospective single-center observation study.

BACKGROUND: Patients treated with linezolid (LZD) frequently develop thrombocytopenia, and previous studies have identified the risk factors for this condition. However, the relationship between the development of LZD-induced thrombocytopenia and baseline platelet count has varied according to different reports. AIM: To explore the relationship between platelet count and the development of LZD-induced thrombocytopenia. METHOD: Patients who underwent LZD at Hokkaido University Hospital in Japan from September 2008 to March 2023 were included. We collected data on patient characteristics and p…

Evidence of Linezolid Resistance and Virulence Factors in Enterococcus spp. Isolates from Wild and Domestic Ruminants, Italy.

The aim of this study was to evaluate the resistance patterns against selected critically and highly important antibiotics (quinupristin/dalfopristin, vancomycin, and linezolid) in 48 Enterococcus isolates obtained from wild (red deer and Apennine chamois) and domestic (cattle, sheep, and goats) ruminants living with varying degrees of sympatry in the protected area of Maiella National Park (central Italy). According to CLSI breakpoints, 9 out of 48 isolates (18.8%) showed resistance to at least one antibiotic. One Apennine chamois isolate was resistant to all tested antibiotics. The PCR scree…

Risk of serotonin syndrome in acutely ill patients receiving linezolid and opioids concomitantly: a retrospective cohort study.

INTRODUCTION: Linezolid is an oxazolidinone antibiotic with a reversible, non-selective, monoamine oxidase inhibitory effect. Combining linezolid with serotonergic agents may increase serotonin syndrome (SS) risk.Linezolid is recommended in patients with suspected or confirmed resistant Gram-positive bacterial infections, especially if vancomycin cannot be used. However, it is unclear whether co-administration of linezolid with opioids increases the risk of serotonin syndrome. RESEARCH OBJECTIVE: To establish whether combining linezolid with opioids will increase the incidence of SS in acutel…

Factorial design and post-antibiotic sub-MIC effects of linezolid combined with fosfomycin against vancomycin-resistant enterococci.

BACKGROUND: Antimicrobial drug resistance, including vancomycin-resistant enterococci (VRE), has long been an inescapable clinical problem. If vancomycin loose its therapeutic relevance, a regimen of linezolid combined with fosfomycin may provide an alternative option. METHODS: In this study, the in vitro antimicrobial effect of linezolid combined with fosfomycin on several different types of VRE was investigated using a checkerboard method and time-kill assays. Based on the results of the 24 h time-kill assays, a 22 factorial design was then adopted. Finally, the post-antibiotic effect (PAE)…

Simple strategy to assess linezolid exposure in patients with multi-drug-resistant and extensively-drug-resistant tuberculosis.

Linezolid is used increasingly for the treatment of multi-drug-resistant (MDR) and extensively-drug-resistant (XDR) tuberculosis (TB). However, linezolid can cause severe adverse events, such as peripheral and optical neuropathy or thrombocytopenia related to higher drug exposure. This study aimed to develop a population pharmacokinetic model to predict the area under the concentration curve (AUC) for linezolid using a limited number of blood samples. Data from patients with MDR-/XDR-TB who received linezolid and therapeutic drug monitoring as part of their TB treatment were used. Mw\Pharm 3.8…

Linezolid decreases susceptibility to secondary bacterial pneumonia postinfluenza infection in mice through its effects on IFN-γ.

Influenza infection predisposes patients to secondary bacterial pneumonia that contributes significantly to morbidity and mortality. Although this association is well documented, the mechanisms that govern this synergism are poorly understood. A window of hyporesponsiveness following influenza infection has been associated with a substantial increase in local and systemic IFN-γ concentrations. Recent data suggest that the oxazolidinone antibiotic linezolid decreases IFN-γ and TNF-α production in vitro from stimulated PBMCs. We therefore sought to determine whether linezolid would reverse immun…

Inhibition of biofilm maturation by linezolid in meticillin-resistant Staphylococcus epidermidis clinical isolates: comparison with other drugs.

Biofilm resistance mechanisms are multifactorial and vary from one organism to another. The purpose of this study was to investigate the efficacy of linezolid against indwelling device-related meticillin-resistant Staphylococcus epidermidis (MRSE) biofilm, and compare this with other antimicrobials. MICs, minimum biofilm inhibitory concentrations (MBICs) and minimum biofilm eradication concentrations (MBECs) were determined by the microtitre plate method. Fourteen and thirteen isolates from patients with indwelling device-related bacteraemia (IDB) and indwelling device colonization not associa…

Linezolid versus vancomycin for the treatment of gram-positive bacterial infections: meta-analysis of randomised controlled trials.

This review aimed to compare data regarding the effectiveness and safety of linezolid and vancomycin in the treatment of gram-positive bacterial infections. PubMed and other databases were searched to identify relevant randomised controlled trials (RCTs). Nine RCTs studying 2489 clinically assessed patients were included in the meta-analysis. Overall, there was no difference between linezolid and vancomycin regarding treatment success in clinically assessed patients [odds ratio (OR)=1.22, 95% confidence interval (CI) 0.99-1.50]. Linezolid was more effective than vancomycin in patients with ski…
