Secondary calyceal neck atresia induced by postoperative hemorrhage and hyperinflammation after FURL in a solitary functional kidney with contralateral renal atrophy: a case report.

Flexible ureteroscopic holmium laser lithotripsy (FURL) is a mainstream minimally invasive treatment for nephrolithiasis with rare late complications. We report a case of secondary calyceal neck atresia after FURL in a 71-year-old male with left renal atrophy, hypertension, and preoperative Escherichia coli urinary tract infection. Standardized anti-infective preparation was given preoperatively, with no intraoperative calyceal neck injury observed. However, severe hemorrhage and hyperinflammation occurred postoperatively, which was controlled with treatment. Three-month follow-up confirmed co

A Rare Case of a Giant Prostatic Urethral Stone and Its Management.

Prostate calculi, or prostatic stones, are typically small, greyish-brown, ultrasound-dense formations composed mainly of carbonated calcium phosphate apatite. We present a 65-year-old male with severe lower urinary tract symptoms (LUTS), including weak urinary stream, difficulty urinating, and acute retention, who was found to have a 4 cm prostate stone. Imaging confirmed obstruction of the prostatic urethra. Surgical intervention with transurethral resection of the prostate (TURP) and Holmium laser lithotripsy successfully removed the stone and relieved symptoms. This case emphasizes the nee

Invasive fungal bezoar requiring partial cystectomy.

A 67-year-old man developed dysuria and position-dependent obstructive voiding symptoms after undergoing holmium laser ablation of the prostate (HOLAP) for benign prostatic hypertrophy. A large fungal (candidal) ball adherent to the bladder wall was removed by loop excision, but the bezoar recurred in 2 weeks despite systemic fluconazole and intravesical amphotericin B. A second attempt at endoscopic removal with ultrasonic lithotripsy, endoscopic graspers, and fulguration was also unsuccessful. The patient underwent open partial cystectomy to remove his invasive fungal bezoar. Convalescence w

Nonsurgical treatment of cholecystolithiasis with percutaneous transhepatic cholecystoscopy.

Early surgical intervention in acute cholecystitis is sometimes fatal to patients in the high-risk group. Since the technical development of ultrasonically guided puncture of the gallbladder, percutaneous transhepatic cholecystostomy has become a safer method for the treatment of acute cholecystitis. We have been developing percutaneous transhepatic cholecystoscopy procedures since 1981, and have used this method in 11 patients with cholecystolithiasis. In all cases, we were able to destroy the stones with the Nd-YAG laser, and remove the fragments with a basket catheter through the fistula. T

'Boxing in the corner': A modified retrograde approach for the management of proximal ureteric stones of 1-2 cm.

Objectives: To study a modification to the conventional retrograde ureteroscopic approach for treating proximal ureteric stones of 1-2 cm; we intentionally push the stone from the proximal ureter into a favourable calyx then the flexible ureteroscope is used to fragment the trapped stone using laser lithotripsy ('boxing in the corner'). Patients and methods: The study was conducted in a randomised prospective manner and included 100 patients who presented with a single proximal ureteric stone of 1-2 cm. We randomised the patients into two equal groups: Group A (50 patients) underwent the conve

Bouveret's Syndrome: A Case Series and Literature Review on a Gallstone Disease Causing Gastric Outlet Obstruction.

Introduction Bouveret's syndrome refers to a gastric outlet obstruction due to the impaction of a large gallstone following retrograde migration via a bilio-duodenal fistula. Although no clear management guideline has been formulated, different treatment modalities have been described, including endoscopic stone removal using classical endoscopic devices, like snares and forceps, or fragmentation of stones with new devices, such as lasers and extracorporeal shockwave lithotripsy (ESWL). Results This case series reports six patients who have been diagnosed with Bouveret's syndrome and have pres

Step-by-step strategy in the management of residual hepatolithiasis using post-operative cholangioscopy.

Hepatolithiasis is the presence of calculi within the intrahepatic bile duct specifically located proximal to the confluence of the left and right hepatic ducts. The ultimate goal of hepatolithiasis treatment is the complete removal of the stone, the correction of the associated strictures and the prevention of recurrent cholangitis. Although hepatectomy could effectively achieve the above goals, it can be restricted by the risk of insufficient residual liver volume, and has a 15.6% rate of residual hepatolithiasis. With improvements in minimally invasive surgery, post-operative cholangioscopy

Improved effectiveness and safety of flexible ureteroscopy for renal calculi (<2 cm): A retrospective study.

INTRODUCTION: We discuss the efficacy and safety of flexible ureteroscopy for renal calculi with a burden of <2 cm, as well as the prevention and treatment of complications. METHODS: A total of 108 renal calculi with flexible ureteroscopy and holmium laser treatment were retrospectively analyzed. The stone-free rate was evaluated. The effectiveness, safety, surgical technique, incidence of complications, and relevant treatments were analyzed. RESULTS: All patients underwent only one lithotripsy procedure. The success rate of flexible ureteroscopy was 97.2% (105/108). Among the 105 cases, the

Endoscopic Management of Benign Pancreaticobiliary Disorders.

Endoscopic management of benign pancreaticobiliary disorders encompasses a range of procedures designed to address complications in gallstone disease, choledocholithiasis, and pancreatic disorders. Acute cholecystitis is typically treated with cholecystectomy or percutaneous drainage (PT-GBD), but for high-risk or future surgical candidates, alternative decompression methods, such as endoscopic transpapillary gallbladder drainage (ETP-GBD), and endoscopic ultrasound (EUS)-guided gallbladder drainage (EUS-GBD), are effective. PT-GBD is associated with significant discomfort as well as variable

Detailed Nephro-urological Management of a Case of Medullary Sponge Kidney with Distal Renal Tubular Acidosis and Obstructive Uropathy.

Medullary sponge kidney (MSK) is a rare renal malformation characterized by precalyceal tubular ectasia of the renal collecting ducts that clinically manifests as recurrent renal stones, distal renal tubular acidosis (dRTA), osteoporosis, and nephrocalcinosis. In this case report, we present the case of a woman with a severe form of MSK associated with dRTA. She had extensive nephrocalcinosis and obstructive uropathy caused by a large upper ureteric stone in the left kidney. The stone was disintegrated by flexible ureteroscopic laser lithotripsy. Her initial biochemical derangements were ident

Renal colic causing Boerhaave syndrome.

Boerhaave syndrome is the spontaneous rupture of the oesophagus, usually due to vomiting. The condition is rare but can be fatal. A 30-year-old male presented with vomiting and pain in his left flank and chest. Computed tomography scanning of the chest, abdomen and pelvic revealed a 4 mm left proximal ureteric stone and pneumo-mediastinum due to oesophageal rupture, consistent with Boerhaave syndrome. The patient underwent insertion of left ureteric stent, with staged left ureteroscopy and laser lithotripsy. The patient's oesophageal rupture was managed conservatively, and he made a full recov

Fluoroless Ureteroscopy: Experience in More Than 100 Patients.

BACKGROUND: Ureteroscopy is becoming the primary treatment for ureteral stones. As a standard of care, ureteroscopy is performed under the supervision of fluoroscopy. Recent advances in endourological technology make the need for fluoroscopy questionable. OBJECTIVES: To summarize our experience with a no-fluoroscopy technique for selected cases of ureteral stones. METHODS: Patients were considered suitable for fluoroless ureteroscopy if they had one or two non-impacted stones, in any location in the ureter, 5-10 mm size, with a normal contralateral renal unit and no urinary tract infection.

[Case of renal subcapsular hematoma caused by flexible transurethral lithotripsy].

A 39-year-old man with macroscopic hematuria was admitted to our hospital. A stone, 5 mm in diameter was detected in the right ureteropelvic junction after abdominal computed tomography and plain abdominal radiography. We performed flexible transurethral lithotripsy (f-TUL) and crushed the stone and extracted almost all stone fragments without any complications. However, almost immediately after the operation, the patient began to complain about pain in the right back. In the results of abdominal plain computed tomography right renal subcapsular hematoma was detected. Because active bleeding w

Acute Urinary Retention Secondary to Urethral Lithiasis in a 4-Year-Old Boy: How We Managed This Rare Case.

Urethral stone impaction (USI) is an extremely rare cause of acute urinary retention (AUR) in pediatric urology. Few case reports are available, and no consensus guidelines currently exist for managing this condition. We describe our management of such a case and provide a review of the relevant literature. A 4-year-old boy with no prior urological history presented to our emergency department with abdominal pain lasting 8 days. An ultrasound performed 5 days earlier identified a 7-mm stone at the ureterovesical junction. Symptomatic treatment with paracetamol and non-steroidal anti-inflammato

Robotic Flexible Ureteroscopy: A Step-by-Step Guide to the ILY® Platform.

INTRODUCTION: Robotic flexible ureteroscopy (rfURS) is emerging as an alternative to manual flexible ureteroscopy for the treatment of upper urinary tract stones. Contemporary guidelines and reviews describe its potential ergonomic and radiation-related advantages, as well as comparable surgical outcomes (1-5). Initial clinical and simulation studies with the ILY® platform have demonstrated feasibility, safety, and a short learning curve, although standardized implementation remains limited (6-10). This video presents a step-by-step workflow for the safe and efficient use of the ILY® platform.

Single-use, low-profile flexible ureterorenoscopy in paediatric lithiasis: Preliminary experience expanding the boundaries of ESPU recommendations.

INTRODUCTION: Single-use flexible ureterorenoscopy (fURS) is a well-established practice today in adult patients with urological lithiasis. Its application in children is becoming more widespread thanks to the availability of lower profile devices (<7Fr). Its indication remains limited in ESPU clinical guidelines. The aim of this study is to present our clinical experience and analyse the applicability of these guidelines in our practice. METHODS: A retrospective observational descriptive study was designed. Patients who underwent laser lithotripsy with single-use ureterorenoscopes during 202

The Impact of Ureteral Access Sheath Use on the Development of Abnormal Postoperative Upper Tract Imaging after Ureteroscopy.

PURPOSE: Ureteral access sheaths are commonly used during ureteroscopy to facilitate stone removal, improve visibility and maintain low intrarenal pressures. However, the use of a ureteral access sheath can cause ureteral wall ischemia and ureteral tears, potentially increasing the risk of postoperative ureteral stricture and obstruction. We studied the impact of ureteral access sheath use on postoperative imaging studies. Secondary objectives included studying the impact of other intraoperative parameters on postoperative imaging studies. MATERIALS AND METHODS: A retrospective chart review w

Antegrade removal of a knotted ureteric stent: Case report and review of literature.

Ureteral stents are routinely used in urological practice for many indications including obstruction of ureter, ureteral stricture, prior to treatment with extracorporeal shock wave lithotripsy, and to promote healing following ureteral injury. Complications reported with ureteric stents include stent migration, stent rupture, encrustation, ureteral perforation, erosion, and fistulation. Knotting of an indwelling ureteral stent is a very rare complication, with fewer than 30 cases reported in the literature. Techniques for managing this complication include using a holmium laser to cut the kno
