Haemophagocytic syndrome after liver transplantation in adults.

The haemophagocytic syndrome is defined as a proliferation of phagocytic macrophages in the bone marrow, lymph nodes and spleen. Clinically, it is characterised by fever and pancytopenia. We present here a case of haemophagocytic syndrome after liver transplantation in a 63-year-old man who had undergone transplantation for autoimmune hepatitis. One month after liver transplantation, he developed ascites, fever and progressive pancytopenia. Bone marrow biopsy showed proliferation of non-neoplastic histiocytes, demonstrating phagocytosis of haemopoietic cells. No infectious or neoplasm-associat

Liver involvement in patients with solid tumors of nonhepatic origin.

Metastatic disease represents the most common hepatic neoplasm in the Western world. The most common primary malignancies to spread to the liver are those that originate in the gastrointestinal tract. Of non-gastrointestinal malignancies, breast, lung, and melanoma malignancies are most likely to develop hepatic metastases. Some solid tumors, such as renal cell carcinoma, may cause liver-related abnormalities in the absence of hepatic metastases, presumably by way of cytokine-mediated mechanisms. Physical examination, laboratory testing, histologic evaluation, and various radiographic studies

β-Catenin mutations in 2 nested stromal epithelial tumors of the liver--a neoplasia with defective mesenchymal-epithelial transition.

Nested stromal epithelial tumor of the liver is a rare neoplasm of early childhood and adolescence with a characteristic nested morphology of spindle and epithelioid cells. Histogenesis and pathogenesis of this neoplasm are, however, still unclear. Because the characteristic nested morphology with spindle mesenchymal and epithelioid cells is suggestive of altered mesenchymal-epithelial transition and β-catenin mutations are rather common in other liver tumors such as hepatoblastomas, we investigated the β-catenin gene in 2 nested stromal epithelial tumors of the liver and analyzed additional f

Intracholecystic papillary-tubular neoplasm diagnosed by peroral cholangioscopy.

Intracholecystic papillary-tubular neoplasm (ICPN) is a preinvasive neoplasm arising from the gallbladder mucosa, with a better prognosis than conventional gallbladder carcinoma; however, accurate preoperative differentiation based solely on imaging remains challenging. When complicated by infection, inflammatory changes often mimic tumor invasion, making it difficult to accurately evaluate the tumor extent. We report the case of an 83-year-old male with a liver abscess and suspected advanced gallbladder carcinoma. Initial imaging suggested invasion of the liver and right hepatic artery, and t

Pancreatic cancer and hypercalcemia associated with von Recklinghausen's disease.

A 44-year-old man with von Recklinghausen's disease was admitted to our hospital presenting with left hypochondralgia. Computed tomography showed a large mass at the body and tail of the pancreas, with metastatic liver tumors. Endoscopic retrograde pancreatography revealed an obstruction of the main pancreatic duct at the body. In the course of the illness, the patient's serum calcium concentration increased gradually to 13.6 mg/dl, although bone scan with 99mtechnetium demonstrated no accumulation in the bones. Serum levels of parathyroid hormone related-protein and tumor necrosis factor were

Immunological and ultrastructural characterization of true histiocytic lymphoma in the northern pike, Esox lucius L.

An epidemic of a malignant neoplasm occurs in northern pike, Esox lucius L., from the Aland Islands of Finland. The neoplasm is morphologically similar to other pike hemic tumors reported in other areas of the world. Pike normal tissues showed evolutionary conservation with the mammalian intermediate filament proteins cytokeratin, desmin, vimentin, neurofilament protein, and glial fibrillary acidic protein; tumor cells are positive for vimentin, suggesting that the neoplasm is of mesenchymal origin. Hemic tissue mononuclear cells undergo polyclonal stimulation by the known mammalian T- and B-l

Malignant retroperitoneal PEComa: A case report with emphasis on radiological findings.

A perivascular epithelioid cell tumor (PEComa) is a rare mesenchymal neoplasm with distinctive perivascular epithelioid cells that usually demonstrates myomelanocytic differentiation. PEComas can arise in various organs and generally are benign. Uncommonly PEComas have been documented to be malignant with metastasis most frequently to the lung, liver, lymph nodes, and bone. Here, we present the case of a 59-year-old male with a malignant retroperitoneal PEComa with confirmed metastasis to the femur and suspected metastasis to the liver and lung. The purpose of this case study is to present the

Malignant histiocytosis in a patient presenting with hepatic dysfunction and peliosis hepatis.

In this article, we report the case of a 36-yr-old patient presenting with manifestations of portal hypertension, hepatic dysfunction, and fever who proved to have peliosis hepatis on liver biopsy. A thorough work-up revealed no obvious etiology. At autopsy, malignant histiocytosis of the liver and bone marrow was diagnosed. This case represents the first report of the association of peliosis hepatis with this rare histiocytic neoplasm and exemplifies the need for persistence in the search for malignancy, particularly hematological malignancy, in the patient with unexplained peliosis. The clin

Mixed neuroendocrine-non-neuroendocrine neoplasm of the ampulla of Vater: a case report.

BACKGROUND: Mixed neuroendocrine-non-neuroendocrine neoplasms of the ampulla of Vater are rare and heterogenous, making it difficult to achieve a definitive preoperative diagnosis. Herein, we describe a patient in whom a provisional diagnosis of mixed neuroendocrine-non-neuroendocrine neoplasm of the ampulla of Vater was made preoperatively. CASE PRESENTATION: Computed tomography revealed an enhancing periampullary tumor in a 69-year-old man with obstructive jaundice. Subsequent duodenoscopy revealed an ulcerated lesion in the swollen ampulla of Vater, from which six biopsies were collected.

Non-Hepatosplenic Extramedullary Hematopoiesis Masquerading as Metastatic Malignant Melanoma.

Non-hepatosplenic extramedullary hematopoiesis (NHS-EMH), the formation of blood cellular elements outside the medulla of the bone marrow and outside the liver and spleen, has been noted among patients with myeloproliferative neoplasms and other serious hematological diseases. However, NHS-EMH is rarely identified among individuals without concurrent hematological disease. Since the radiologic features of NHS-EMH are nonspecific, lesions may be mistaken for metastatic disease when observed in patients with known solid tumors. We report an unusual case of a patient with a simultaneous presentat

Genomic characterization of a well-differentiated grade 3 pancreatic neuroendocrine tumor.

Pancreatic neuroendocrine neoplasms (PanNENs) represent a minority of pancreatic neoplasms that exhibit variability in prognosis. Ongoing mutational analyses of PanNENs have found recurrent abnormalities in chromatin remodeling genes (e.g., DAXX and ATRX), and mTOR pathway genes (e.g., TSC2, PTEN PIK3CA, and MEN1), some of which have relevance to patients with related familial syndromes. Most recently, grade 3 PanNENs have been divided into two groups based on differentiation, creating a new group of well-differentiated grade 3 neuroendocrine tumors (PanNETs) that have had a limited whole-geno

A Case of Malignant PEComa of the Uterus Associated with Intramural Leiomyoma and Endometrial Carcinoma.

Perivascular epithelioid cell tumors (PEComas) refers to a family of mesenchymal neoplasms composed of angiomyolipomas, clear cell "sugar" tumors of the lung, and lymphangioleiomyomatoses. These tumors have a distinctive and common component of perivascular epithelioid cells that show an association with blood vessel walls and immunohistochemically display myomelanocytic differentiation. The unique neoplasms have been shown to have an expanded range through a variety of case reports, including visceral, intra-abdominal, soft tissue, and bone tumors. The retroperitoneum, abdominopelvic region,

Immature T cell neoplasms in three young cattle.

Immature T cell neoplasms in three young Holstein cattle with neoplastic involvement of the thymus are described. Case 1, with a precursor T lymphoblastic leukemia (calf form of leukosis), was an 86-day-old female calf. The leukemia was characterized by replacement of the bone marrow and spleen by leukemia cells, but preservation of epithelial frameworks throughout the thymus. The other two neoplasms were thymic γδ T cell lymphomas, which were observed in a 246-day-old steer (case 2) and a 16-month-old heifer (case 3). Histological examination revealed obliteration of the normal thymic archite

Malignant hepatic perivascular epithelioid cell tumor (PEComa) - Case report and a brief review.

Perivascular epithelioid cell tumors (PEComas) are rare mesenchymal neoplasms which can arise from almost any location in the body. Diagnosing them pre-operatively is difficult as they mimic features of other hepatic neoplasms including hepatocellular carcinoma (HCC), fibrolamellar HCC, and focal nodular hyperplasia (FNH) among others. The unique feature of these tumors is the coexpression of muscle and melanocytic markers. These are identified immunohistochemically by the expression of Human Melanin Black-45 (HMB-45), Melan-A and Smooth Muscle Antigen (SMA) which are seen in the majority of t

Intraductal papillary mucinous tumor of the pancreas associated with autosomal dominant polycystic kidney disease.

A 43-year-old male with a history of autosomal dominant polycystic kidney disease (ADPKD) was admitted to our center with severe abdominal pain and was diagnosed with acute pancreatitis. CT showed multiple cysts in the liver and both kidneys along with ADPKD and a cystic mass, 4 cm in diameter, in the pancreatic head. The main pancreatic duct was dilated to 1 cm in diameter. The patient was diagnosed with acute pancreatitis due to intraductal papillary mucinous tumor (IPMT), and pancreatoduodenectomy was performed. Histologic examination revealed a multiloculated cystic tumor filled with mucin

Analysis of misdiagnosis of pancreatic acinar cell carcinoma with significantly elevated alpha-fetoprotein: a case report and literature review.

Acinar cell carcinoma (ACC) is a rare malignant exocrine tumor of the pancreas, often presenting with nonspecific symptoms that lead to diagnostic confusion with other pancreatic neoplasms. This article reports a case of a 72-year-old female ACC patient who was misdiagnosed as Solid pseudopapillary neoplasms (SPN) by an external hospital. Laboratory investigations upon admission revealed a markedly elevated serum alpha-fetoprotein (AFP) level of 2321.18 ng/mL, while contrast-enhanced CT of the upper abdomen demonstrated a mass in the pancreatic head, with the liver showing normal parenchyma an

Unusual cases of penile vascular tumors in dogs: report of two cases.

Vascular tumors affecting the reproductive tract in male dogs, including hemangiomas and hemangiosarcomas, are rarely reported in veterinary medicine. However, they are more frequently observed in other anatomical sites in dogs such as the skin, spleen, and the liver. Despite their low incidence in the genital region, these neoplasms should be considered when compatible clinical signs are observed. This case report describes the clinical and pathological findings of two penile vascular neoplasms in dogs, highlighting their differing prognoses. The first case was classified as hemangiosarcoma a

Epstein-Barr virus-associated smooth muscle tumour in an immunosuppressed patient: a rare clinical scenario.

Epstein-Barr virus-associated smooth muscle tumours (EBV-SMTs) are rare neoplasms typically affecting immunosuppressed individuals such as organ transplant recipients. This case follows a woman in her 30s who presented with multi-organ masses 5 years after a kidney transplant. While initial suspicion focused on common post-transplant infections or malignancy, a liver biopsy revealed a smooth muscle neoplasm positive for EBV by EBV-encoded RNA in-situ hybridisation, consistent with EBV-SMT. Managing EBV-SMT is challenging as no standardised treatment regimens exist. When reducing immunosuppress

Gastric neuroendocrine neoplasms: a primer for radiologists.

Gastric neuroendocrine neoplasms are uncommon tumors with variable differentiation and malignant potential. Three main subtypes are recognized: type 1, related to autoimmune atrophic gastritis; type 2, associated with Zollinger-Ellison and MEN1 syndrome; and type 3, sporadic. Although endoscopy alone is often sufficient for diagnosis and management of small, indolent, multifocal type 1 tumors, imaging is essential for evaluation of larger, high-grade, and type 2 and 3 neoplasms. Hypervascular intraluminal gastric masses are typically seen on CT/MRI, with associated perigastric lymphadenopathy

[Rectal neuroendocrine tumors: surgical therapy].

The incidence of rectal neuroendocrine tumors (NET) has increased in recent years. Most of these neoplasms are asymptomatic and are diagnosed by colonoscopy screening, which could be one of the reasons for the increasing occurrence. As less than 1 % of rectal NET produce serotonin they are practically never discovered due to a carcinoid syndrome. The current guidelines of the European (ENETS) and North American (NANETS) Neuroendocrine Tumor Societies support clinicians with useful diagnostic and treatment algorithms. The most important criteria for therapy are tumor size and histopathological
