Early-Stage Hodgkin Lymphoma.

Combined modality therapy (CMT; chemotherapy with consolidative radiation) has led to excellent long-term outcomes in patients with early-stage Hodgkin lymphoma. However, despite high cure rates, CMT can lead to significant long-term toxicity in a typically young patient population; as a result, there has been a push in the field to develop novel treatment strategies that can maintain robust patient survival while minimizing the risk of treatment-related complications. This has been primarily explored by 2 mechanisms: first, the use of response-adapted treatments based on interim FDG-PET/CT; a…

Classical Hodgkin Lymphoma Today.

Classical Hodgkin Lymphoma is one of the most curable neoplasms worldwide, particularly among young adults. Significant advances have been made to maximize treatment efficacy and minimize acute and long-term toxicities, including infertility, cardiovascular complications and secondary primary malignancies. Pretreatment prognostic stratification and interim PET response are the cornerstones of personalized treatment strategies. Circulating tumor cell-free DNA represents an investigational tool for genotyping Hodgkin Lymphoma at diagnosis and monitoring treatment response. An abbreviated course …

Conjunctival lymphoma: case report.

OBJECTIVE: To report the diagnosis and therapeutic approach in the case of a patient with conjunctival lymphoma. CASE PRESENTATION: A 54-year-old caucasian female presented to the hospital with a painless conjunctival mass in the right eye of at least three months duration. The ophthalmological examination showed a "salmon patch" conjunctival lesion in the inferior fornix. An incisional biopsy of the conjunctival lesion was performed, and the histopathological examination confirmed the diagnosis of extranodal marginal zone B-cell conjunctival lymphoma. The CT scan showed no systemic involveme…

Primary pulmonary follicular lymphoma.

Primary pulmonary follicular lymphoma is an extremely rare subset of extra-nodal non-Hodgkin's lymphoma. We present a successful work-up and surgical management of this disease entity. The patient is a 74-year-old man who presented with an enlarging ground glass opacity on his computed tomography scan. Percutaneous biopsy was not diagnostic for malignancy, but given the underlying malignant potential, he underwent definitive operative resection rather than additional invasive diagnostic testing. Our case highlights challenges in the management of nondiagnostic preliminary pathology as well as …

Update on follicular lymphoma.

The past two decades have seen remarkable progress in both biological understanding and optimizing treatment of follicular lymphoma. Historically considered an incurable disease, long-term follow-up of several induction approaches demonstrates that up to 40% of patients enjoy remission durations of 10 or more years, and risk of dying of lymphoma continues to fall. This update will focus on progress in follicular lymphoma over the past 3 years, which has included refinements in staging and prognosis, novel immunotherapy treatment approaches for relapsed and refractory disease, and long-term fol…

Duodenal-Type Follicular Lymphoma.

This case report presents the clinical findings, diagnostic evaluation, and treatment options for a 71-year-old female patient with a medical history of hypertension, dyslipidemia, and recurrent urinary tract infections. The patient presented with chronic abdominal pain as the sole symptom. Despite normal laboratory investigations, esophagogastroduodenoscopy and ileocolonoscopy were performed. Biopsies were obtained from the duodenum and histopathological analysis confirmed a diagnosis of duodenal-type follicular lymphoma. This rare condition typically presents with minimal clinical symptoms a…

Breast lymphoma: General review.

BACKGROUND: Lymphoma of the breast can be classified as either primary breast lymphoma (PBL) or secondary to systemic lymphoma (SBL). PBL is a rare disease with Diffuse Large B cell Lymphomas (DLBCL) being the most common subtype. OBJECTIVES: In the current study, we represented eleven cases diagnosed with breast lymphoma in our trust; two of them had PBL and nine had SBL. We focused mainly on the clinical presentation, diagnosis, management and outcomes. METHODS: We did this retrospective review for all breast lymphoma patients who were diagnosed in our trust during the periods from 2011-20…

Treatment of indolent lymphoma.

Treatment of indolent lymphoma has improved significantly in recent decades since the advent of rituximab (anti-CD20 monoclonal antibody). Although, some patients with limited disease can be cured with radiation therapy alone, most patients experience disease progression and recurrence during follow-up despite early initiation of treatment. Thus, watch-and-wait is still regarded the standard for asymptomatic patients. Patients with indolent lymphoma have a significant heterogeneity in terms of tumor burden, symptoms (according to anatomical sites) and the need for instant therapy. Therefore, t…

HHV-8 associated lymphoma.

PURPOSE OF REVIEW: The discovery of Kaposi sarcoma herpesvirus (KSHV) / human herpesvirus 8 (HHV-8) led to recognition of primary effusion lymphoma (PEL) as a distinct lymphoproliferative disorder. Subsequently, two other types of lymphoma have been shown to be associated with HHV-8 : HHV-8 positive diffuse large B cell lymphoma not otherwise specified and germinotropic lymphoproliferative disorder. The pathogenesis of PEL is unique as in most cases the HHV-8+ tumoral cells are coinfected with the Epstein-Barr virus (EBV), suggesting an interplay between these two herpesviruses. This article re…

Pediatric classical Hodgkin lymphoma.

Over the past century, classical Hodgkin lymphoma (HL) has been transformed from a uniformly fatal disease to one of the most curable cancers. Given the high cure rate, a major focus of classical HL management is reducing the use of radiation therapy (RT) and chemotherapy agents such as procarbazine and doxorubicin to minimize long-term toxicities. In both North America and Europe, an important philosophy in the management of classical HL is to guide the intensity of treatment according to the risk category of the disease. The main factors used for risk classification are tumor stage, bulk of …

Early Progressing Follicular Lymphoma.

PURPOSE OF REVIEW: Follicular lymphoma is an indolent lymphoma which does not limit life expectancy in most patients; however, approximately 20% of patients will experience progression of disease within 24 months (POD24) of diagnosis and have inferior survival outcomes. To date, no clinical, genetic, or tumor microenvironment prediction models have been able to definitively predict which patients will experience POD24 which limits the ability to alter frontline management of patients suspected to be at high risk of early progression. Here, we review recent literature regarding novel prediction…

Follicular T-cell lymphoma.

Follicular T-cell lymphoma is a recently described, rare neoplasm with the true incidence still unknown. It is a lymph node-based tumor in which the node shows a follicular pattern of growth. Immunohistochemistry confirms the cells of origin to be follicular helper T-cells and thus plays an important role in the diagnosis of the tumor. We report a case of follicular variant of peripheral T-cell lymphoma, which was erroneously reported as Hodgkin lymphoma on fine-needle aspiration, and follicular lymphoma on hematoxylin and eosin.

Non-Hodgkin Lymphoma Metabolism.

Non-Hodgkin lymphomas (NHLs) are a heterogeneous group of lymphoid neoplasms with different biological characteristics. About 90% of all lymphomas in the United States originate from B lymphocytes, while the remaining originate from T cells [1]. The treatment of NHLs depends on the neoplastic histology and stage of the tumor, which will indicate whether radiotherapy, chemotherapy, or a combination is the best suitable treatment [2]. The American Cancer Society describes the staging of lymphoma as follows: Stage I is lymphoma in a single node or area. Stage II is when that lymphoma has spread t…

Lymphoma: Diagnosis and Treatment.

Lymphoma is a group of malignant neoplasms of lymphocytes with more than 90 subtypes. It is traditionally classified broadly as non-Hodgkin or Hodgkin lymphoma. Approximately 82,000 new U.S. patients are diagnosed with lymphoma annually. Any tobacco use and obesity are major modifiable risk factors, with genetic, infectious, and inflammatory etiologies also contributing. Lymphoma typically presents as painless adenopathy, with systemic symptoms of fever, unexplained weight loss, and night sweats occurring in more advanced stages of the disease. An open lymph node biopsy is preferred for diagno…
