Macrolides are well-known antibiotics exerting antimicrobial as well as anti-inflammatory and immunomodulatory effects. Since the observation of a dramatic improvement in lung disease and survival in patients with diffuse panbronchiolitis, macrolides have been used over the long term in several chronic respiratory diseases. This review describes the results of trials that have evaluated long-term macrolides in the treatment of cystic fibrosis, non-cystic fibrosis bronchiectasis, and asthma, particularly focusing on the impact on children. It also provides new insights on the potential effects …

Didehydroroflamycoin (DDHR), a recently isolated member of the polyene macrolide family, was shown to have antibacterial and antifungal activity. However, its mechanism of action has not been investigated. Antibiotics from this family are amphiphilic; thus, they have membrane activity, their biological action is localized in the membrane, and the membrane composition and physical properties facilitate the recognition of a particular compound by the target organism. In this work, we use model lipid membranes comprised of giant unilamellar vesicles (GUVs) for a systematic study of the action of

Macrolide antibiotics, erythromycin, clarithromycin and azithromycin are commonly prescribed for upper respiratory infection, and their use has recently been further linked to immunomodulatory effects. With the widespread and expanded use of macrolides, special attention should be paid to their potential adverse effects. We reported two cases of end-stage renal disease (ESRD) patients who developed hallucinations such as vivid images of worms after taking clarithromycin. Similar to previous case reports of clarithromycin neurotoxicity, the visual hallucination resolved upon cessation of clarit…

Although the clinical relevance of antibiotic treatment in influencing the natural course of Mycoplasma pneumoniae-associated respiratory diseases is questioned by some physicians, most experts suggest that antibiotics should be systematically used in patients with M. pneumoniae respiratory infections, especially those involving the lower respiratory tract. Macrolides (MLs), tetracyclines (TCs) and fluoroquinolones (FQs) are the drugs of choice for M. pneumoniae infection, but only MLs are recommended for children. The main aim of this review is to analyse what is known about M. pneumoniae res…

This was a retrospective, multi-center study of patients admitted to hospital with community-acquired pneumonia, caused by Streptococcus pne … a, and mortality. High-level macrolide resistance remains rare among US patients failing outpatient macrolides. The majority of cases and virtually all of the mortality occurred in patients with low-level resistant strains.

From 2002 to 2003, a total of 381 consecutive S. pyogenes isolates were obtained from throat swabs (n = 337) and samples of pus (n = 31), sputum (n=10) and blood (n = 3) at Hacettepe University Hospital. The susceptibility of the isolates to erythromycin was tested by the agar dilution method. Erythromycin resistant strains were then tested for their MICs to azithromycin, clindamycin, and penicillin, their phenotype of resistance to macrolides-lincosamides-streptogramin B (MLSB) and for the presence of macrolide resistance genes. The rate of resistance to erythromycin was 6.8%. Constitutive (c…

The emergence of antibiotic resistance in mycobacteria involves the selection of mutant variants within a susceptible bacterial population. However, it is unclear whether antimycobacterial drugs act just as selective agents or can influence the rate of appearance of resistant mutants. The present study was initiated to address this issue by monitoring the effects of antimicrobial agents on the appearance and growth of clarithromycin (CLR)-resistant (CLR(r)) bacilli in broth cultures of Mycobacterium avium. Preexposure of M. avium to CLR had a significant dose effect on the emergence of resista

Azithromycin (AZM) preparations in fine granules and capsules were evaluated in 36 pediatric patients with various infections. In patients with pneumonia caused by Moraxella catarrhalis, Haemophilus influenzae or Mycoplasma pneumoniae, bronchitis, pharyngitis, scarlet fever, whooping cough, or campylobacter enteritis, AZM was found effective in 94.4% (34/36). As for the bacteriological efficacy of AZM, all of 12 strains identified were found eradicated by the treatment. Plasma T 1/2(24 approximately 48 hrs.) of AZM in fine granules, given two patients at 10 mg/kg body weight once daily for 3 d

The in vitro and in vivo antibacterial activity of A-56268 (TE-031), the 6-O-methyl derivative of erythromycin, was compared with those of erythromycin and other reference drugs. A-56268 had the same spectrum of antibacterial activity as erythromycin. A-56268 was generally 1 log2 dilution more potent or equal to erythromycin against all organisms except haemophilus influenzae and Propionibacterium acnes, for which A-56268 was 1 log2 dilution and 3 log2 dilutions, respectively, less potent. The MBC of A-56268 and erythromycin was not significantly different from the MIC against Streptococcus py

Macrolides are an antibiotic class widely used in both human and veterinary medicine, and function by interfering with protein synthesis. Regrettably, numerous strategies for evading the antibiotic properties of macrolides have been found in bacteria, including enzyme-mediated inactivation. These mechanisms are now widely disseminated among pathogenic, animal-associated, and environmental bacteria making them a One Health issue. Macrolide esterases, which hydrolyze the macrolactone's ester bond, confer one such resistance mechanism. Two types of macrolide esterases have thus far been identifie…

The prevalence of macrolide-resistant Mycoplasma pneumoniae in Japan is increasing once again. The therapeutic efficacy of quinolone garenoxacin was compared with minocycline, which is recommended as the first-choice drug in patients with macrolide-resistant M. pneumoniae pneumonia. Defervescence within 48 h after initiation of antibiotics was observed in 90.6% of garenoxacin and 90% of minocycline recipients. Our results indicated that garenoxacin can be considered as the first-choice drug for the treatment of macrolide-resistant M. pneumoniae pneumonia.

Covering: up to August 2024Macrolides, the core skeletons of numerous marketed drugs and bioactive natural products, have garnered considerable scientific interest owing to their structural diversity and broad spectrum of pharmaceutical activities. The formation of intramolecular ester bonds is a critical biocatalytic step in constructing macrolide skeletons. Here, we summarised enzymatic ester bond formation strategies in fungal polyketide (PK)-type, nonribosomal peptide (NRP)-type, and PK-NRP hybrid-type macrolides. In PK-type macrolides, ester bond formation is commonly catalysed by a trans…

CRISPR-Cas (clustered regularly interspaced short palindromic repeats-CRISPR associated) systems are common among enterococci and may prevent the acquisition of mobile genetic elements carrying antimicrobial resistance genes. In this study, we correlate the presence of CRISPR with genes associated with macrolide resistance and high-level resistance to aminoglycosides (HLR-A) among 216 Enterococcus faecalis and 82 Enterococcus faecium isolates. We used PCR to detect genes associated with macrolide resistance, HLR-A, and type II-A CRISPR elements. We used two-tailed Fisher's exact test to evalua

Lower respiratory tract infections caused by Mycoplasma pneumoniae are most often characterized by mild, self-limiting symptoms, including fever, cough, and shortness of breath. They mainly affect schoolaged children and young adults, in whom this germ is the third leading cause of community-acquired bacterial pneumonia. Infection with a macrolide-resistant strain of Mycoplasma pneumoniae (MR-MP) presents with a similar clinical picture; however, the duration of fever, cough, and hospitalization tends to be prolonged. The prolonged duration of symptoms is not associated with an increased risk

Since November 2023, an increase in M. pneumoniae respiratory infections has been observed in France. Macrolides are the reference treatment and are indicated as first-line therapy. Macrolide-resistant strains have been described in clinical isolates, especially in Asian countries. In the present manuscript, we report the persistence of an M. pneumoniae strain from an immunocompromised child with CCR2 deficiency on prophylactic azithromycin treatment. Antimicrobial susceptibility testing showed acquisition of macrolide resistance after treatment with azithromycin. Molecular typing using two ty…
