Magnesium in acute myocardial infarction.

Clinical studies on the use of magnesium in acute myocardial infarction have yielded contradictory results. While the exact reasons for these discrepancies are unclear, it appears that the timing of magnesium administration is crucial to the success or failure of therapy. Although some studies have shown a significant reduction in the odds of death with this agent, the exact reasons underlying this possible benefit are not yet fully understood. A reduction in arrhythmias, inhibition of thrombus generation, changes in coronary and peripheral hemodynamics, and the limitation of ischemic damage a…

Magnesium: deficiencies and therapeutic uses.

Hypomagnesemia, which may be difficult to diagnose, can complicate diabetes mellitus or a variety of cardiovascular diseases. In some patients, an infused multielectrolyte preparation is often the best solution.

[Magnesium--pathophysiology and clinical aspects].

Magnesium is one of the most wide-spread elements in nature. It is the fourth major cation in the human body (after Ca, Na and K), and the second most abundant within the cells (after K). More than 300 enzymatic reactions depend on the presence of magnesium, especially concerning the generation and use of adenosine triphosphate. Magnesium level is controlled by the kidneys and gastrointestinal tract. It appears closely linked to calcium, potassium and sodium balance. The fundamental role played by magnesium in different biological systems has been recognized during recent years, and disorders …

[Magnesium and ischemic heart disease].

Our knowledge about magnesium in health and disease has increased during the past ten years. Many authors have demonstrated possible magnesium depletion in the population as a whole and particularly among cardiac patients receiving diuretics. Evidence suggests that this magnesium depletion may play a role in the development of arteriosclerosis. It has been demonstrated that long term supplementation of the diet with magnesium reduces the frequency of heart disease. However, the matter is still not proven. It is postulated further that magnesium depletion aggravates the outcome of acute myocard…

Magnesium deficiency and sudden death.

A link between Mg deficiency and sudden death is suggested by a substantial number of studies published over the past three decades. Data come from epidemiologic, autopsy, clinical, and animal studies. They suggest that: (1) Sudden death is common in areas where community water supplies are Mg-deficient. (2) Myocardial Mg content is low in people who die of sudden death. (3) Cardiac arrhythmias and coronary artery vasospasm can be caused by Mg deficiency and (4) Intravenous Mg reduces the risk of arrhythmia and death immediately after acute myocardial infarction. Because of these data, Mg supp

[Congenital tubulopathy with magnesium loss].

The authors report on a 1-year old girl who presented with transient hypotonia and polydipsia related to renal-concentrating defect. Renal magnesium and calcium wasting were noted when the subject was 3.5 years old, in association with distal tubular acidosis and nephrocalcinosis. Hypocalcemia and hypomagnesiemia persisted when the patient was 9.5 years old. About 50 cases of tubular defects with renal magnesium loss have been reported in the literature and show that magnesium loss may be either isolated or associated with potassium and/or calcium wasting. This hereditary defect may be due to …

Magnesium status and digoxin toxicity.

1. Eighty-one hospital patients receiving digoxin were separated into groups with and without digoxin toxicity using clinical criteria. Serum digoxin, sodium, potassium, calcium, creatinine, magnesium and monocyte magnesium concentrations were compared. 2. Subjects with digoxin toxicity had impaired colour vision (P less than 0.0001, Farnsworth-Munsell 100 hue test) and increased digoxin levels (1.89 (1.56-2.21) vs 1.34 (1.20-1.47) nmol l-1, P less than 0.01) (mean (95% confidence limits], though there was considerable overlap between two groups. 3. Subjects with digoxin toxicity had lower lev…

Central nervous system magnesium deficiency.

The central nervous system concentration of magnesium (Mg++) appears to have a critical level below which neurologic dysfunction occurs. Observations presented suggest that the interchange of the Mg++ ion between the cerebrospinal fluid, extracellular fluid, and bone is more rapid and dynamic than is usually believed. This is especially so when the hypertrophied parathyroid gland is associated with significant skeletal depletion of Mg++ as judged by history rather than serum level. Magnesium, much like calcium, has a large presence in bone and has a negative feedback relationship with the para…

Magnesium metabolism in essential hypertension.

Hypertension is a complex, heterogeneous disorder of which the exact etiology is unknown. The difficulty in ascribing an independent role to a single dietary constituent in blood pressure regulation may be due to interactions among nutrients which influence blood pressure. The effect of any one nutrient, particularly magnesium, on hypertension should be considered within the context of overall nutrition in each patient. Clinical, experimental and epidemiologic studies support the role of magnesium in hypertension, whereas a few studies negate this role. Magnesium ions are important in arterial…

Ischemic heart disease and magnesium.

Globally, among mortality and morbidity indices for man, ischemic heart disease (IHD) ranks at the top of the list. In the industrialized world, IHD is the leading killer and accounts for approximately 35% of all deaths each year. This most common cause of death results from insufficient coronary blood flow. IHD is a general term used to describe a pathophysiologic state in which cardiac output is inadequate as a consequence of coronary blood flow deficits brought about when the latter cannot deliver enough O2 to meet the needs of the myocardium. This review is concerned with the etiology of I

The familial magnesium-losing kidney.

Magnesium deficiency is often secondary to existing disease. Over a period of 20 years, 17 cases of primary magnesium-losing kidney have been reported. This report describes two additional cases, and a comparison with previous cases is made. The familial magnesium-losing kidney appears to be caused by a congenital tubular defect in the reabsorption of magnesium. Its manifestations may be various but the diagnosis is established by the presence of hypomagnesemia with an unappropriately high urinary magnesium excretion in patients with no other renal disorder.
