Magnetic stimulation of the brain: starting or stopping.

A very brief magnetic pulse is able to stimulate cortical neurons through the intact scalp. It can elicit movement of a limb by activation of pyramidal cells either directly or via interneurons. When making a voluntary movement, a corresponding program of the task is thought to be compiled beforehand. In patients with ideomotor apraxia who can not perform a task, a lesion of the left hemisphere is frequently found. A magnetic cortical stimulus given directly before a movement is executed is known to delay the onset without altering the pattern. When it will become feasible to deliver a localiz

Magnetic resonance imaging in the assessment of craniosynostosis.

Modern interest in craniosynostosis began when clinicians, regardless of their individual specialty but united in their interest in the problems raised by patients with this condition, first realised that if treatment was confined simply to opening up the fused sutures of the skull vault, success in terms of both function and the restoration of a more normal appearance was likely to be limited. Indeed, the grouping together of such specialists into formal craniofacial units was tacit recognition of this, as was the acceptance that many affected children had evidence of clinical problems that c

Magnetic resonance imaging in patients with hypopituitarism.

In patients with hypopituitarism, magnetic resonance (MR) imaging of the hypothalamus and pituitary has disclosed a high incidence of hypoplasia of the anterior pituitary lobe, attenuation or transection of the pituitary stalk, and formation of an "ectopic" posterior pituitary lobe at the base of the hypothalamus. These anatomic abnormalities may be associated with other congenital malformations of the central nervous system, or may be due to an in utero toxic or infectious insult, perinatal trauma, neonatal asphyxia and hypoxia, head injury, or hemorrhage into a pituitary adenoma. The progres

[Magnetic resonance in the staging of multiple myeloma].

The authors retrospectively reviewed the MR examinations of 46 patients with clinical and laboratory findings of monoclonal gammopathies (MG). All cases had been submitted to radiographic examination which had shown skeletal involvement in 22 cases and osteoporosis in 11, with rupture of the vertebral body in 3 patients. Scintigraphy had been performed on all patients and CT on 12; 36 patients were subsequently submitted to follow-up (at 6, 12 and 24 months). MR examinations were performed with dedicated coils and standard sequences for the subjects with skeletal localizations on X-ray images.

Magnetic resonance imaging in the detection of sacroiliitis.

The value of magnetic resonance imaging (MRI) in establishing the diagnosis of sacroiliitis was studied in 20 patients with established or suspected disease on conventional radiographs and in 10 healthy subjects. Coronal T1 weighted, axial T2 weighted and proton density MRI images of the sacroiliac joints in addition to plain film radiographs were obtained. All films were graded from 0 to 4 according to the modified New York criteria and independently for changes in joint width, erosions, sclerosis and ankylosis. Using the modified New York criteria, more abnormalities were detected by MRI tha

[Magnetic resonance tomography in pulmonary hypertension].

We examined 23 patients with pulmonary hypertension of varying aetiology by MRI and compared the results with those of right heart catheterisation. The best correlation was obtained between right ventricular mural thickness and mean pulmonary pressure (R = 0.91, p = 0.001). There was significant correlation (R = 0.85, p = 0.001) for the diameter of the inferior vena cava, which was dilated in all patients with pulmonary hypertension. There was no significant correlation between mean pulmonary pressure and the diameters of the superior vena cava or the main pulmonary artery branches (R = 0.55 a

Magnetic stimulation in the treatment of partial seizures.

We have recently demonstrated that Magnetoencephalographic (MEG) brain measurements in patients with seizure disorders show significant MEG activity often in the absence of conventional EEG abnormalities. We localized foci of seizure activity using the mapping technique characterized by the ISO-Spectral Amplitude (ISO-SA) on the scalp distribution of specified spectral components or frequency bands of the emitted MEG Fourier power spectrum. In addition, using an electronic device, we utilized the above recorded activity to emit back the same intensity and frequency of magnetic field to the pre

Magnetic resonance imaging of spinal metastases.

Sixty-six patients with spinal metastatic disease secondary to urological malignancy underwent magnetic resonance imaging. Twenty-eight patients had clinical evidence of spinal cord compression. MRI demonstrated metastatic cord compression in 16 cases, a benign cause in two cases, nerve root involvement in five cases. In seven patients MRI demonstrated spinal metastases which were not visible on plain radiographs and isotope bone scans. Nine patients had metastatic deposits involving the cauda equina and in a further 22 patients spinal metastases were an incidental finding during MRI for stagi

Magnetic resonance imaging in childhood epilepsy.

One hundred and seventy children of chronic seizures with strong clinical suspicion of an underlying intracranial lesion as its cause were studied by Magnetic Resonance Imaging (MRI). Maximum number of patients were between 6-12 years, males outnumbering females. Structural abnormalities were seen in 158 of the 170 patients. The study revealed tuberculoma as the commonest lesion in this series (n = 64) followed by cysticercosis (n = 27). Three patients were seen to have glioma. An interesting finding was disappearing lesion in 6 children. MRI proved to be an excellent modality in demonstrating

[Role of magnetic resonance in the diagnosis of cryptorchidism].

The paper reports the authors' personal experience regarding the use of magnetic resonance imaging in the diagnosis of cryptorchidism. The result of the study of 13 patients with non-palpable gonads, who all underwent surgical confirmation (with positive outcome in 66% of cases), encourage the use of this non-invasive method with a high anatomic resolution, which can be used to diagnose both the site, dimensions and possible structural alterations of the gonad.

Magnetic resonance imaging of Kernohan's notch.

Compression of the cerebral peduncle against the tentorial incisura contralateral to a supratentorial mass, the so-called Kernohan's notch, can be a cause of false localizing motor signs. The authors present a case of Kernohan's notch secondary to a traumatic extradural hematoma. The patient developed an oculomotor palsy and a dense motor deficit ipsilateral to the extra-axial hematoma. Magnetic resonance imaging in the postoperative period clearly showed the midbrain lesion. The motor deficit and 3rd nerve palsy subsequently resolved.

Magnetic resonance imaging in MELAS syndrome.

MELAS syndrome is a distinct clinical entity belonging to a group of mitochondrial encephalomyopathies characterized by the tetrad of myopathy, encephalopathy, lactic acidosis, and stroke-like episodes. Computed tomography (CT) and magnetic resonance (MR) findings are reviewed in a patient with MELAS. Serial CT studies demonstrated multiple "migrating" infarcts in various stages of evolution involving primarily the posterior temporal and occipital regions. MR was more sensitive than CT in demonstrating the number and extent of cortical lesions in this disease entity.

Moyamoya disease--imaging with magnetic resonance.

We report a case of moyamoya disease in a young adult female, in whom the initial clinical and radiological features were considered to be compatible with multiple sclerosis. The subsequent development of cerebral infarction led to the typical moyamoya features being found on carotid angiography. Magnetic resonance imaging (MRI) features of dilated collateral arteries were then recognized, and the initial MRI findings considered to represent demyelination were reinterpreted as areas of infarction. Although rare, moyamoya disease should be considered in the differential diagnosis in young patie

Magnetic resonance evaluation of mitral stenosis.

In order to study magnetic resonance (MR) abnormalities in mitral stenosis, electrocardiogram (ECG)-gated MR imaging was performed with a 2.0 T MR system in 41 patients with mitral stenosis before catheterization. Mean transverse diameter of the left atrium was 8.9 +/- 1.4 cm and anteroposterior diameter was 5.1 +/- 1.0 cm, indicating significant enlargement. Homogeneous or inhomogeneous flow-related signals in ventricular diastole were detected in the left atrial cavity in 32 cases (78%), and in the atrial appendage in 35 cases (85%). In 21 patients having a mean wedge pulmonary arterial pres

Magnetic resonance imaging of the paranasal sinuses.

MRI has been very helpful in evaluating tumors in the paranasal sinuses. MRI can differentiate retained secretions from tumors. The multiplanar capabilities of MRI are also very helpful in identifying tumor margins. Although a specific diagnosis can be suggested in most cases, demonstration of the extent of the disease and specifically the relationship to key surgical landmarks is more important. Structures of thin cortical bone are better seen with CT than MRI. For several reasons, CT is still the examination of choice for evaluation of the osteomeatal complex and inflammatory disease of the

Magnetic resonance imaging of unicornuate uterus.

Five patients with a hysterosalpingographic diagnosis of unicornuate uterus underwent magnetic resonance imaging (MRI) and subsequently laparoscopy/laparotomy to evaluate the ability of MRI to identify the various subclasses of this malformation. The method was demonstrated to be valid, since in all 5 cases (one subclass A1b, two subclass A2, two subclass B) were correctly diagnosed. Compared with laparoscopy, MRI is less expensive, less invasive, and can be performed in women in whom laparoscopic examination is risky. However, unlike laparoscopy, MRI can not detect the presence of minimal and
