Six-Year Survival of a Mini Dental Implant-Retained Overdenture in a Child with Ectodermal Dysplasia.

Patients with ectodermal dysplasia (ED) experience several problems caused by abnormal development and functioning of the head and neck region. In addition to developmental nasal cartilage abnormalities and absence of sweat glands, hair, and eyebrows, edentulism or developmental disorders of teeth (cone-shaped teeth) are commonly observed in these types of patients. ED is also characterized by underdeveloped alveolar ridges, a decreased occlusal vertical dimension, reduced salivary secretion, and dry oral mucosa, which make prosthetic rehabilitation difficult. Few studies of intraosseous denta

Effect of postoperative radiation therapy on mandibular reconstruction using a modular endoprosthesis - a pilot study.

The endoprosthetic system has been shown to be a potential option for mandibular reconstruction. The objective of this pilot in vivo animal study was to determine the effects of postoperative radiation using brachytherapy on bone and soft tissue healing in mandibles reconstructed with the endoprosthesis. Six adult Macaca fascicularis mandibles were reconstructed with a cemented endoprosthesis after segmental body resection. The animals were divided into two groups. The test group was subjected to radiation therapy 1 month postoperation while the control group did not receive any radiation. Res

Two-piece versus single-piece patient-specific titanium subperiosteal implants in atrophied edentulous mandibles: A finite element analysis.

STATEMENT OF PROBLEM: Subperiosteal implants (SPIs) are considered a viable treatment option in patients with highly resorbed ridges if extensive bone grafting or other more invasive procedures are not indicated. Traditionally, SPIs were designed in a single-piece fashion, while two-piece SPIs were introduced recently. However, there are no studies in the current dental literature that compared between the two designs. PURPOSE: This three dimensional finite element analysis (3D FEA) was conducted to compare the differences in the biomechanical behavior of two SPI designs: the single-piece and

CLINICAL MANAGEMENT OF IMMEDIATE IMPLANT PLACEMENT AND LOADING IN THE ESTHETIC ZONE WITH FINAL PROSTHETIC RESTORATION.

Immediate implant placement with immediate loading in the esthetic zone is a predictable treatment option when strict surgical and prosthetic protocols are followed. This case report describes the clinical management of a 50-year-old systemically healthy, non-smoking male treated with two immediately placed implants in the maxillary central incisor region (teeth 11 and 21). Following atraumatic flapless extraction due to extensive root caries (tooth 11) and a chronic periapical granulomatous lesion with insufficient residual root length (tooth 21), two tapered implants (3.5×13 mm; Hiossen) wer

Rapid Digital Conversion 2.0: A cast-less workflow for complete arch implant-supported restorations.

Accurate data acquisition is essential for the efficiency and predictability of complete arch implant-supported restorations. Conventional and hybrid workflows often require multiple analog steps, such as cast fabrication, mounting, verification, or the use of reference screws with repeated scans. These additional procedures may increase complexity, prolong treatment, and introduce errors through multiple dataset merges. Rapid Digital Conversion (RDC) 2.0 represents a fully digital, cast-less workflow that eliminates the need for reference screws, physical casts, and intraoral verification jig

[Surgical Procedure for Postoperative Reconstruction of Swallowing Function in a Very Elderly Patient with Advanced Tongue Cancer].

Surgical ablation and reconstruction are the first-line treatment strategies for squamous cell carcinoma of the tongue. However, locally advanced cases can be complicated by postoperative dysphagia. Here, we report a case of advanced tongue cancer in a very elderly patient who regained good swallowing function following a reconstructive surgery using a pectoralis major musculocutaneous flap with elevation of the hyoid bone. Case: An 89-year-old man diagnosed with advanced squamous cell carcinoma of the tongue(cT4aN2bM0, cStage ⅣA)underwent tracheostomy, right modified radical neck dissection t

The use of endoprotheses for patients with complex asymmetry: A clinical case study.

Facial asymmetry requires multidisciplinary management. In some cases, predictable problems due to congenital abnormalities can be corrected by early orthodontic treatment. Orthodontics is nevertheless often part of an orthosurgical treatment plan. The solutions are conventional and familiar: maxillomandibular rotation osteotomy and genioplasty. Many cases can be treated in this way, but there is often persistent residual asymmetry due to loss of volume that is not corrected by these techniques. The use of maxillo-acial endoprostheses designed on the basis of three-dimensional craniofacial ana

[Applications of autogenous outer table of the calvarial and mandibular bone in posttraumatic deformities of the nose].

OBJECTIVE: To study the applications of bone grafts from the cranium or mandible in the repair of posttraumatic deformities of the nose. METHODS: From 1997 to 2002, a total of 44 patients, who presented with secondary nose deformity as well as facial fractures, were reconstructed by autogeneous outer table grafts of the calvarial or mandibular bone after facial skeleton reposition. In this series, 32 cases used the calvarial bone, 12 cases used the mandible bone, which was harvested through a coronal incision and an oral approach respectively. RESULTS: The surgical incisions healed primarily

Impact of Complete Denture and Mandibular Advancement Device in the Management of Completely Edentulous Obstructive Sleep Apneic Individuals: A Systematic Review with Meta-Analysis.

STATEMENT OF THE PROBLEM: Obstructive sleep apnea (OSA) is an underdiagnosed and potentially serious disorder that is accentuated by edentulism. The overclosure of the mandible and a potential upper airway collapse during sleep creates challenges in treating edentulous sleep apneic patients. PURPOSE: To evaluate complete dentures and mandibular advancement devices as potential oral appliances in the management of sleep apnea in completely edentulous patients. MATERIALS AND METHOD: The study design was a systematic review with meta-analysis. The search criteria complied with the preferred rep

Long-term changes in soft tissue height on the facial surface of dental implants.

The success of osseous healing around dental implants has allowed for an increased emphasis on soft tissue healing and esthetic results. However, there is limited information profiling the long-term healing of the soft tissues following prosthesis placement. The purpose of this study was to assess the long-term changes in the position of the facial soft tissue margins following restoration of a one-stage implant system. One hundred and six one-stage ITI implants were evaluated in 39 patients. Implants were placed in maxillary and mandibular anterior regions. Clinical assessment of the soft tis

Delayed Infection of Porous Polyethylene Implants After Oncologic Maxillectomy and Reconstruction: 2 Case Reports and Review of Literature.

Medpor porous polyethylene implants are commonly used for facial skeletal reconstruction due to reported biocompatibility, fibrovascularization, and durability. While uncommon, late implant infections are an important consideration. We report delayed infections in 2 patients after unilateral total oncologic maxillectomy and reconstruction using Medpor implants for an ossifying fibroma and squamous cell carcinoma, respectively. In the first patient, annual interval computed tomography (CT) scans showed no recurrence of tumor or inflammatory changes. The second was lost to follow-up after adjuva

Surgical Management of Idiopathic Condylar Resorption: Orthognathic Surgery Versus Temporomandibular Total Joint Replacement.

Young females with retruded and hyperdivergent mandibles, class II openbite malocclusions, and steep occlusal planes with or without TMJ symptoms are at higher risk for Idiopathic Condylar Resorption (ICR). Such patients undergoing orthodontic and /or surgical treatment should be informed of possible relapse due to ICR. Orthognathic Surgery with Total joint replacement or Orthognathic surgery alone may both be acceptable options for management of the facial deformity and the malocclusion that ensues from ICR. Proper patient selection is key to achieving a successful outcome. Current trends and

Osteochondroma of condyle: case discussion and review of treatment modalities.

Temporomandibular joint (TMJ) forms a synovial articulation between the condyle and the cranium. It is a complex joint and shows hinge and gliding movements. Unlike other articulating heads, condyle grows with intramembranous ossification. TMJ is subjected to excessive loads throughout life as it supports essential functions such as mastication, deglutition, speech and respiration. Traumatic, neoplastic or non-neoplastic pathologies sometimes necessitate joint replacement therapy. Osteochondroma is one such benign tumour originating from condyle which requires surgical replacement of condyle w

[Therapy of the severely atrophied mandible].

The problems encountered in implant treatment of extensively resorbed mandibles concern poor residual height of the bone and the use of short implants in the least favorable prognosis, excessive corticalization of the bone making drilling difficult, poor vascularity of the implant site, and mechanical constraints sustained by implants bearing high prosthesis. This paper addresses these problems and suggests solutions, using author's own experience and the findings of other clinicians, published in professional literature. To solve poor vascularity of the implant site, the paper suggests prelim

Is Alloplastic Temporomandibular Joint Reconstruction a Viable Option in the Surgical Management of Adult Patients With Idiopathic Condylar Resorption?

PURPOSE: Idiopathic condylar resorption (ICR) presents diagnostic and therapeutic challenges to practitioners because of the rarity of the condition, progressive deformity, and simultaneous involvement of skeletal, occlusal, and articular disorders. The objective of this study was to report clinical outcomes after prosthetic replacement of the temporomandibular joint (TMJ) for the management of ICR. PATIENTS AND METHODS: A retrospective analysis of patients with ICR managed by bilateral total TMJ replacement and concomitant mandibular advancement with or without maxillary surgery was performe
