Mastoid hyperpneumatization with atlantoaxial fistulization presenting as head rotation-induced aural fullness.

OBJECTIVE: To describe a unique presentation of mastoid hyperpneumatization with atlantoaxial air fistulization. PATIENTS: Case report, 30-year-old male patient. INTERVENTIONS: Descriptive case report. MAIN OUTCOME MEASURES: Subjective aural fullness, with objective tympanogram changes, induced by head rotation. RESULTS: The patient complained of left ear fullness exclusively and repeatedly during neck extension and head rotation to the right side. These head positional changes cause a change of pressure of the atlantoaxial intra-articular emphysematous air, and these fluctuations in press…

Mastoid bone fracture presenting as unusual delayed onset of facial nerve palsy.

Delayed-onset facial nerve paralysis is a rather uncommon complication of a mastoid bone fracture for children younger than 10 years. We routinely arrange a cranial computed tomography (CT) for patients encountering initial loss of consciousness, severe headache, intractable vomiting, and/or any neurologic deficit arising from trauma to the head. However, minor symptomatic cranial nerve damage may be missed and the presenting symptom diagnosed as being a peripheral nerve problem. Herein, we report a case of a young boy who presented at our emergency department (ED) 3 days subsequent to his acc…

Glial choristoma in the middle ear and mastoid bone: a case report.

Heterotopic brain tissue usually involves extracranial midline structures of the head and neck such as nose, nasopharynx, and oral cavity. Its occurrence in the non-midline structures, including middle ear, is rare. We described a 50-yr-old-man with heterotopic glial tissue in the middle ear and mastoid bone. The patient presented with progressive hearing loss for 8 yr. There was no history of congenital anomalies, trauma, or ear surgery. Computed tomography revealed a mass-like lesion with soft tissue density occupying the middle ear cavity and mastoid antrum. At the operation, a gray-white f…

Morphometric analysis of anatomical relationships of the facial nerve for mastoid surgery.

Surgical anatomical relationships of the facial nerve (FN) with several landmarks used in mastoid surgery were studied in temporal bone axial high resolution CT scans of 90 patients (180 ears). The shortest distances between the FN and external auditory canal (EAC), sigmoid sinus (SS), posterior fossa dural plate (PFD), and joint of the bony EAC with the lateral surface of the mastoid (M) were measured. These measurements were also analysed in respect of pneumatization and side differences. On average, it was found that FN-EAC was 2.9 mm, FN-SS was 10.5 mm, FN-PFD was 7.3 mm and FN-M was 15.3 …

Long-term outcome of Ménière's disease: endolymphatic mastoid shunt versus natural history.

This retrospective study evaluates the outcome of 38 patients with intractable Ménière's disease with a minimum of 7 years follow-up. Twenty underwent endolymphatic-mastoid shunt (EMS) and 18 were offered surgery but declined (natural history, NH, group). At the last control, 85% of the patients who were operated on (EMS group) and 74% of the NH patients had complete or substantial control of vertigo. The difference between the two groups was not significant. However, it was significant at 2 and 4 years follow-up. At 2 years, EMS patients had complete or substantial control of vertigo in 65% o…

The role of endolymphatic mastoid shunt surgery in the managed care era.

OBJECTIVE: Ongoing controversy regarding the surgical management of Meniere's disease has prompted us to review the effectiveness of the endolymphatic mastoid shunt procedure in the control of vertigo. In the current managed care environment in which outcome measures, cost effectiveness, and procedural efficacy must be demonstrated, the surgeon can no longer rely on anecdotal or empirical observations regarding the effectiveness of a treatment paradigm. STUDY DESIGN: Retrooperative case review. SETTING: A tertiary care center. PATIENTS: The records of 327 patients with presumed Meniere's-re…

[Plastic reconstruction of mastoid cavity in patients with chronic suppurative otitis media].

The authors describe a mastoidoplasty modification--repair of the retrotympanal cavity with a large osteomusculoperiosteal graft on the lower feeding pedicle. This intervention was conducted in 25 patients with otitis media purulenta chronica with cholesteatoma and in 6 patients with incomplete epidermization of the cavity after radical operation on the ear. Prior to and following the operation turundas containing mixed salts of acid ethers of cellulose, bound calcium ions and antibiotics were inserted into the external acoustic meatus and postoperative cavity. The treatment duration was short…

[Restoration of external auditory canal by filling mastoid cavity with carbon pellet].

To prevent infection of an open mastoid cavity after mastoidectomy and improve hearing, the external auditory canal had been maintained on 72 cases (73 ears) by filling mastoid cavity with carbon pellet. The average duration of dry ear after operation was 25 days with a dry ear rate being 98.6%. No phenomenon of rejection and falling out appeared within 1-7 years follow-up. Carbon pellet has advantages of chemical stability, nontoxicity, good histocompatability, anti-fatigue characteristic in body fluid, and anti-corrosive effect against acid, base and salt.

[Acoustic characteristics of external auditory meatus after tympanoplasty with open mastoid reconstruction].

This study investigated the effects of down-wall tympanoplasty (open technique included skeletonizing of mastoid) upon external auditory meatus resonance characteristics. The probe tube microphone measurement was performed twice pre and post operatively. The resonance frequency of external auditory meatus remained constant, but the peak amplitude was increased by 0.83dB SPL (P < 0.05). The results indicated that a tympanoplasty with standard open techniques did not affect the normal external auditory meatus resonance characteristics.

[Congenital cholesteatoma of the middle ear and mastoid. Apropos of 5 cases].

Five cases of congenital cholesteatoma of the middle ear and mastoid as defined by Derlacki's criteria were encountered over a 14 month period. They make up 5% of all cases of cholesteatoma managed over the same period of time. Three were young children and all presented with unilateral hearing loss. One had associated multiple congenital abnormalities of the external and middle ear. Only in one patient was the diagnosis made on initial otoscopic examination and the remainder diagnosed on CTscan, myringotomy and tympanotomy. All were operated on; three by the intact canal wall technique, one b…

Mastoid surgery at the Red Cross War Memorial Children's Hospital 1986-1988.

Eighty-three children between ages 0-12 years had mastoid surgery in the three year period 1986-88. The indications were acute mastoiditis (30 children), uncontrolled chronic suppurative otitis media (CSOM) (24 children), cholesteatoma (22 children) and a miscellaneous group (7 children). Forty-seven per cent of those with acute mastoiditis were under one year but the 27 per cent with cholesteatoma as the precipitating cause were 4-12 years. Twenty-three per cent had significant complications. Typmano-mastoid surgery was employed to resolve uncontrolled CSOM. Children were between 2-12 years a…

Endolymphatic mastoid shunt for treatment of Meniere's disease: a five year study.

The endolymphatic mastoid shunt seems to be an effective treatment for those patients whose Meniere's disease is refractory to medical and/or supportive treatment. This series is composed of 48 patients who were followed for up to 5 years postoperatively, with 1 year being the minimum follow-up period; 81% of these patients obtained a satisfactory relief of their vertigo. The morbidity of the procedure is low with no severe or total hearing loss reported in this series. Surgery should not be recommended until the symptoms have caused severe incapacitation; however, this delay must be tempered …

Mastoiditis With Pneumocephalus on CT: An Uncommon Clue to Underlying Bacterial Meningitis.

The combination of pneumocephalus and meningitis as a complication of mastoiditis is rare, with very few cases published in the literature. We report the case of a 33-year-old previously healthy male who presented with acute confusion, fever, agitation, and neck stiffness following a syncopal episode at work. CT imaging revealed opacification of the left mastoid air cells, suggestive of mastoiditis and pneumocephalus in the left frontoparietal region. Cerebrospinal fluid (CSF) analysis confirmed the diagnosis of pneumococcal meningitis. Empirical antibiotic treatment and steroids were initiate…
