BACKGROUND & AIMS: The impact of cirrhosis or prior liver transplant on maternal health during pregnancy has not been studied. We sought to characterize outcomes during labor and delivery among pregnant women with these 2 conditions.
METHODS: A population-based cohort study of women admitted for labor and delivery to US hospitals between 1998 and 2005 was conducted using the Nationwide Inpatient Sample database. We compared health outcomes between pregnant women with cirrhosis or liver transplant with those without known liver disease, adjusting for potential confounders.
RESULTS: The rates …

OBJECTIVE: We assessed the effect of prenatal and peripartum antibiotics on maternal morbidity and mortality among HIV-infected and uninfected women.
METHODS: A multicenter trial was conducted at clinical sites in 4 Sub-Saharan African cities: Blantyre and Lilongwe, Malawi; Dar es Salaam, Tanzania; and Lusaka, Zambia. A total of 1558 HIV-infected and 271 uninfected pregnant women who were eligible to receive both the prenatal and peripartum antibiotic/placebo regimens were enrolled. Pregnant women were interviewed at 20-24 weeks of gestation and a physical examination was performed. Women wer

BACKGROUND: Maternal health is affected by infant's sleep problems like night awakening or bedtime difficulties. Previous studies indicated that infant's sleep problems are affected by particular parenting behaviour (hereinafter referred to as parenting). However, the relation between maternal sleep and infant's sleep has been unclear and there is only limited information on the interaction between parenting, infant's sleep, maternal sleep, and maternal health. The purpose of this study was: 1. to examine the relationship between infant's sleep problems and maternal sleep and health; and 2. to…

BACKGROUND: Better health for populations is linked to the adequate access to health services. This is particularly important in developing countries, where a number of economic, social, and geographic barriers exist. Pregnant women are particularly at risk, as they have many health needs and problems. In addition, the use of maternal health services is particularly influenced by economic and socio-cultural factors. Therefore, we intended to appreciate the association between poverty, socio-cultural factors and use of maternal health services in Ivory Coast.
METHODS: For this study, our data …

OBJECTIVE: Inpatient conditions that might be avoided through improved outpatient services are called Ambulatory Care Sensitive Indicators, and they include pyelonephritis in nonpregnant adults. No such indicators have been developed for pregnant women. We examine whether hospital-specific rates of gestational pyelonephritis may serve as a measure of the quality of ambulatory maternal care.
METHODS: The California Department of Health Services provided an administrative data set linking maternal and newborn delivery records for 1997 with antepartum hospital admissions. We created a "low-risk"

Recent studies in humans suggest that alterations in the activity of the neuroendocrine system mediate the effects of psychosocial stress on fetal development and birth outcome. Chronic maternal distress compromises the normal regulation of hormonal activity during pregnancy and elevates free circulating corticotrophin-releasing hormone (CRH), probably of placental origin, before the normal increase occurs at term. Excess CRH, and other hormones like cortisol and met-enkephalin that pass through the placenta, could precipitate preterm labor, reduce birth weight and slow growth rate in prenatal

As part of a community health assessment, variations in maternal and child health-related vital statistics data in Ishikawa Prefecture, Japan, from 1984 to 1993 were studied. Annual vital statistics reports published by the Prefectural government were used for the analysis. The following rates by public health district were calculated: abortion rate, early fetal mortality rate, late fetal mortality rate, perinatal mortality rate, neonatal mortality rate, and infant mortality rate. Maternal mortality rates were calculated for the Prefecture because of the small number of events. A few northern

India has an excellent infrastructural layout for the delivery of MCH services in the community through a network of subcenters, primary health centers, community health centers, district hospitals, state medical college hospitals, and other hospitals in the public and private sectors. However, the health pyramid does not function effectively because of limited resources, communication delays, a lack of commitment on the part of health professionals, and, above all, a lack of managerial skills, supervision, and political will. The allocation of financial resources for the delivery of health ca

A household health interview survey on MCH services utilization was carried out in 22 villages of a rural district of Arsi region, Ethiopia, before the launching of an integrated MCH programme. Coverage of antenatal services was 26%, and 61% of the women who received antenatal care reported having had 3 or more visits. Antenatal care was positively associated with living within 10 km of the Health Centre. Twenty-eight percent of the mothers attended the under-5 clinic and most returned for 3 or more visits. In addition, 99% reported having breast-fed their last child but more than 25% started

The determinants of the use of family planning, prenatal care, childhood immunizations and oral rehydration salts (ORS) were studied with survey data of 8000 women in Metro Cebu, the Philippines. Polytomous logistic regression methods were used. The level of maternal education was the most consistent and important determinant of use of these four health services in both urban and rural areas. For example, the estimated odds of using modern contraception increased by six and eleven per cent for each additional year of schooling in urban and rural strata respectively. Economic status and access

A study of maternal health care and outcome of pregnancies in a rural area of Nigeria is presented. Over 80% of women sought maternal health care from the traditional midwives. The chance of survival of the child born was found to decrease with parity. In 95% of families child spacing was between 1-3 years. The peak incidence of multiple pregnancy occurred earlier and the rate was found lower than has been reported elsewhere. The health institutions in the urban area were found to differ in the quality of maternal services provided. The best service was not accessible to a larger population. T…

Examining the causal nexus between health services and gender equality is of paramount significance in policy formulation and academic inquiry. This paper concentrates on maternal, sexual, and reproductive health, offering a critical narrative review of empirical research exploring the causal relationship between enhanced women's health, stemming from either overall healthcare amelioration or specific interventions, and broader gender equality objectives. A conceptual framework is devised to elucidate the causal pathways between health and gender equality across various dimensions. The final r

PURPOSE: In Northwest Territories (NWT), Canada, infection rates for chlamydia, gonorrhea, and syphilis are three to ten times higher than the national averages and have been steadily increasing. However, data are scarce on the socioeconomic, psychological, and behavioural factors associated with the proliferation of sexually transmitted infections (STIs) in NWT. To this extent, this study explores Indigenous mothers' perspectives on sexual health, STI risk factors and healthcare-seeking behaviours in NWT.
METHODS: Quantitative and qualitative data were collected using a semi-structured quest

BACKGROUND: Maternal mental health conditions are associated with unmet Social Determinants of Health (SDOH) needs and can impede access to mental health and substance use disorder (SUD) treatment, leading to poor maternal and newborn health outcomes. A text/phone-based maternal mental health screening and referral to treatment intervention, Listening to Women and Pregnant and Postpartum People (LTWP), has demonstrated improved rates of screening, screening positive for mental health concerns, referral to and attendance of mental health and SUD treatment compared to usual care (i.e., in-person

BACKGROUND: Every human being has the right to affordable, high-quality health services. However, mothers and children in wealthier households worldwide have better access to healthcare and lower mortality rates than those in lower-income ones. Despite Somalia's fragile health system and the under-5 mortality rate being among the highest worldwide, it has made progress in increasing reproductive, maternal, and child health care coverage. However, evidence suggests that not all groups have benefited equally. We analysed secondary 2006 and 2018-19 data to monitor disparities in reproductive, mat

BACKGROUND: Maternal mental health (MMH) problems, such as perinatal depression, maternal anxiety, suicide ideation and puerperal psychosis among others, have a significant impact on maternal morbidity and mortality, as well as the health and development of children. One in every four pregnant women and one in every five postpartum women in low-income countries, suffer from maternal mental health (MMH) problems. Despite this, MMH screening, diagnosis, and reporting remain scanty in Uganda. Consequently, this study aimed to investigate the knowledge, practices, and impediments that maternity ca
