Cardio-obstetrics de novo: a state-level, evidence-based approach for addressing maternal mortality and severe maternal morbidity in Georgia.

Georgia has a higher rate of severe maternal morbidity and mortality when compared with the rest of the United States. Evidence gained from the Georgia Maternal Mortality Review Committee identified areas of focus for high-yield clinical initiatives for improvement in maternal health outcomes. Cardiovascular disease, including cardiomyopathy, coronary conditions, and preeclampsia with or without eclampsia, is the most common cause of pregnancy-related death in non-Hispanic Black women in Georgia. The development of a cardio-obstetrics program is an initiative to advance health equity by decrea…

Obstetric transition in the World Health Organization Multicountry Survey on Maternal and Newborn Health: exploring pathways for maternal mortality reduction.

OBJECTIVE: To test whether the proposed features of the Obstetric Transition Model-a theoretical framework that may explain gradual changes that countries experience as they eliminate avoidable maternal mortality-are observed in a large, multicountry, maternal and perinatal health database; and to discuss the dynamic process of maternal mortality reduction using this model as a theoretical framework. METHODS: This was a secondary analysis of a cross-sectional study by the World Health Organization that collected information on more than 300 000 women who delivered in 359 health facilities in …

Risk factors for maternal death and trends in maternal mortality in low- and middle-income countries: a prospective longitudinal cohort analysis.

BACKGROUND: Because large, prospective, population-based data sets describing maternal outcomes are typically not available in low- and middle-income countries, it is difficult to monitor maternal mortality rates over time and to identify factors associated with maternal mortality. Early identification of risk factors is essential to develop comprehensive intervention strategies preventing pregnancy-related complications. Our objective was to describe maternal mortality rates in a large, multi-country dataset and to determine maternal, pregnancy-related, delivery and postpartum characteristics…

Maternal mortality is preventable in Lebanon: A case series of maternal deaths to identify lessons learned using the "Three Delays" model.

OBJECTIVE: To identify the lessons learned from women who died during pregnancy or childbirth in Lebanon between 2018 and 2020. METHOD: This is a case series and synthesis of maternal deaths between 2018 and 2020 that were reported by healthcare facilities to the Ministry of Public Health in Lebanon. The notes recorded from the maternal mortality review reports were analyzed using the "Three Delays" model to identify preventable causes and lessons learned. RESULTS: A total of 49 women died before, during, or after childbirth, with hemorrhage being the most frequent cause (n = 16). The possib…

Maternal mortality in women with pre-viable premature rupture of membranes: An analysis from the French confidential enquiry into maternal deaths.

INTRODUCTION: Pre-viable premature rupture of membranes (pre-viable PROM) is a rare event occurring in less than 1% of pregnancies. Nevertheless, it can be responsible for severe maternal complications, the risk of which needs to be balanced with the possibility to prolong the pregnancy up to viable gestational age. Maternal sepsis was reported in 1%-5% of women who received conservative management and prophylactic antibiotics, but information on maternal mortality is lacking. Our objective was to identify maternal deaths in women who had pre-viable PROM, describe the characteristics of the wo…

[Maternal mortality at the Charles Nicolle Hospital Maternity Department in Tunis between 1972 and 1975. With the exclusion of abortions (author's transl)].

The change in maternal mortality during the time of pregnancy, labour and the puerperium is similar in our department to that reported in many countries. The overall figure for maternal mortality between 1972 and 1975 is 0.54 per 1000 (54 per 100,000 deliveries. This is a halving of numbers as compared with our previous statistics. This improvement can be ascribed to several factors, which are increased in number and quality of medical and para-medical personnel, and the better environmental factors of the inhabitants of the region. On the other hand if mortality is looked at in relationship t…

Machine learning-based ecological prediction of maternal mortality ratio in malaria-endemic Africa.

BACKGROUND: Malaria in pregnancy remains a significant public health concern in sub-Saharan Africa. It may contribute to population-level maternal mortality through adverse maternal outcomes. This study aims to develop a machine learning framework to predict maternal mortality ratio (MMR) across African countries. METHODS: A supervised machine learning framework was applied to identified open source, country-level data across 52 African countries through (2000-2020). MMR was estimated using Support Vector Regression (SVR) model. Model performance was evaluated using a random 70/30 split of co…

Adverse childhood experiences and adverse community environments among maternal mortality cases in Arizona.

BACKGROUND: Adverse childhood experiences (ACE) and adverse community environments (ACoE) can have negative impacts on adult health and lead to premature death. Their association within the maternal mortality (MM) population remains unknown. OBJECTIVE: This study aimed to assess the association between ACEs and ACoEs with pregnancy outcomes and MM variables within the Arizona MM population. STUDY DESIGN: A secondary review of 146 maternal deaths in Arizona (2018-2019), originally reviewed by the Arizona Maternal Mortality Review Committee, was conducted to identify ACE and ACoE exposure, adv…

County-Level Structural Vulnerabilities in Maternal Health and Geographic Variation in Infant Mortality.

OBJECTIVE: To evaluate whether community factors that differentially affect the health of pregnant people contribute to geographic differences in infant mortality across the US. STUDY DESIGN: This retrospective cohort study sought to characterize the association of a novel composite measure of county-level maternal structural vulnerabilities, the Maternal Vulnerability Index (MVI), with risk of infant death. We evaluated 11 456 232 singleton infants born at 22 0 of 7 through 44 6 of 7 weeks' gestation from 2012 to 2014. Using county-level MVI, which ranges from 0 to 100, multivariable mixed e

[Maternal mortality due to suicide and other psychiatric causes in France 2016-2018].

This report, covering the period 2016-2018, confirms that psychiatric causes (largely dominated by suicides) are the leading cause of maternal mortality up to 1year after childbirth, a finding already made in the previous 2013-2015 report. There were 47 deaths from psychiatric causes in 3years, including 45 maternal suicides, giving a maternal mortality ratio (MMR) of 2.1 per 100,000 live births (NV) (95% CI: 1.4-2.6). The median time to suicide was 138days postpartum. This group represents 17.3% (16.5% for suicides) of all maternal deaths for the period. Maternal suicide is linked to an inter…

Obstetric transition: trends in maternal mortality in Somalia over the last two decades.

BACKGROUND: This paper sheds light on the trends of the maternal mortality ratio (MMR) and obstetric transition in Somalia over the last two decades. METHODS: This is a descriptive study comparing aggregate secondary data from the 2006 Multiple Indicator Cluster Survey and the 2020 Somali Health and Demographic Survey to show the transition. RESULTS: A 44% reduction of the MMR from 1044 to 692 per 100 000 live births was observed comparing the two surveys. CONCLUSIONS: Somalia has moved from stage I to stage II of the obstetric transition pathway spectrum and there is optimism that the ongo…

Peripartum Cardiomyopathy and Takotsubo Cardiomyopathy: Rare Contributors to Cardiac Causes of Maternal Mortality.

Peripartum cardiomyopathy (PPCM) and takotsubo cardiomyopathy (TCM) are cardiac conditions that can occur in the peripartum period. They have distinct characteristics and incidence rates; although rare, both contribute to the second leading cause of all-cause maternal mortality in the state of Missouri. PPCM can lead to heart failure, and TCM can cause acute arrhythmias leading to sudden cardiac death in otherwise healthy individuals. Our patient developed PPCM and TCM, which led to myocardial infarction, resulting in anoxic brain injury at 10 months postpartum. This case presentation demonstr…
