The influences of mandible-first versus maxilla-first sequencing on postoperative occlusion.

The aim of this study was to evaluate the influences of surgical sequencing on the postoperative occlusion in bimaxillary osteotomies. Patients who underwent bimaxillary surgery without maxillary segmentation, based on a virtual surgical plan were enrolled. The primary outcome variable was occlusal shift, defined as the difference between the surgical accuracy of the mandible and maxilla. The primary predictor variable was the sequence of surgery, with secondary predictors including inferior maxillary repositioning, counterclockwise rotation and gender. 145 patients were enrolled, 68% female, …

Atypical fibro-osseous lesion of maxilla: possibility of brown tumour or hyperparathyroidism.

The non-neoplastic fibro-osseous lesions of the facial skeleton are rare and are usually considered to be fibrous dysplasia or ossifying fibroma. Brown tumour of the maxilla, on the other hand, is extremely rare with primary hyperparathyroidism (HPT) as its most common cause. This is a report of an atypical fibro-osseous lesion of the upper jaw (in her late thirties) that presented with painful smooth swelling around the premaxilla, adjoining hard palate and floor of the nose. With atypical radiology and variegated consistency, a complex diagnostic workup was undertaken, including FNAC, blood …

Integrating mandibular evidence to assess morphological variation of the Australopithecus afarensis maxilla.

Geometric morphometric analyses are used to explore variation of maxillary dental arcades of Australopithecus afarensis, expanding on the work of Hanegraef and Spoor, 2025 (Morphological variation of the Australopithecus afarensis maxilla. Journal of Human Evolution, 201, 103651) by integrating evidence from a large sample of virtually reconstructed mandibles. Size and shape of maxillary dental arcades can be predicted accurately from mandibular landmarks based on strong covariation between occluding upper and lower dentitions, and a novel method was developed to correct for reduced shape vari…

Comparison of conventional and digital impression approaches for edentulous maxilla: clinical study.

BACKGROUND: The expectations for the clinical and laboratory phases of tissue-supported complete dentures (TSCDs) are changing. Currently, there is a trend towards fast, comfortable, reliable, and low-cost methods. In TSCD impressions, simplified impression and digital impression methods involving the use of an intraoral scanner (IOS) are becoming preferable. Given this situation, this study aims to compare different conventional and digital impression methods used in TSCDs. METHODS: Patients with maxillary complete edentulism and healthy oral mucosa were included in this study. In the digita…

Calcifying Epithelial Odontogenic Cysts of the Anterior Maxilla: Report of Two Cases.

This article is a discussion of two cases of young adults with lesions in similar locations in the anterior maxilla, i.e., the canine-to-canine region, similar history, and comparable radiology. Both cases were histologically diagnosed as calcifying odontogenic cysts. Case 1 was a male aged 28 years with diffuse, firm left malar area facial swelling with pain in associated teeth for a month. Intraorally, he had a gingivo-vestibular swelling also extending palatally in the anterior left maxillary region extending from the distal surface of the left maxillary central incisor to the mesial surfac…

Alveolar Bone Morphologic Predictors for Guided Bone Regeneration Outcome in Anterior Maxilla.

OBJECTIVES: This study aimed to explore the influence of alveolar bone morphologic variables on the outcome of guided bone regeneration (GBR) in the anterior maxilla region. METHODS: Twenty-eight patients who received single maxillary anterior tooth delayed implant placed simultaneously with GBR were recruited. Baseline data including age, gender, implant site, implant brand, and bone graft materials were recorded. The resorption rate of the grafted bone (RRGB), labial bone width at 0 mm, 2 mm, and 4 mm apical to the implant platform at Tn (LBW0Tn, LBW2Tn, LBW4Tn), implant angulation (IA), ma…

Surgical ciliated cyst of the maxilla after orthognathic surgery: A case report.

Surgical ciliated cysts are rare benign cystic lesions that generally occur a number of years after invasive surgical procedures or trauma involving the maxilla. The appearance of this cyst after orthognathic surgery is a complication that has rarely been reported. It usually shows as a well-defined radiolucency in the maxilla in young adults mimicking other maxillary cysts. Therefore, an exhaustive clinical-radiological diagnosis is needed to establish its differential diagnosis and appropriate treatment. The present study describes the case of a surgical ciliated cyst that appeared 20 years …

Managing the severely atrophic maxilla: Farewell to zygomatic implants and extensive augmentations?

BACKGROUND: Dental rehabilitation of severe atrophic upper jaws remains challenging. A new generation of subperiosteally placed and rigid multi-vector bone-anchored patient-specific implants proposes an innovative line extension in implant dentistry. This single-center retrospective study focused on treating severely atrophic maxillae using these implants. METHODS: All patients who were treated with a patient-specific implant (IPS Implant® Preprosthetic, KLS-Martin, Tuttlingen, Germany) at Hannover Medical School due to severe atrophy of the maxilla who had no history of malignancy, cleft lip…

Complex odontoma at the upper right maxilla: Surgical management and histomorphological profile.

BACKGROUND: Odontomas are hamartomatous developmental malformations of the dental tissues. Usually asymptomatic, their presence is often revealed on routine radiographs. The study aimed to establish the efficacy of this conventional approach in treating odontomas, analysing clinical outcome, follow-up, and histomorphological profile. CASE REPORT: A case is presented with a review of the international literature. The patient, aged 8 years, had a complex odontoma localised on the front upper jaw. She was treated following the conventional surgical procedure. Post-operative course and healing we

Primary diffuse large B-cell lymphoma in the maxilla: A case report.

RATIONALE: Lymphomas are the second most common non-epithelial malignant tumors in the oral and maxillofacial region. Non-Hodgkin's lymphoma (NHL) develops at extranodal sites, and cases involving the maxilla account for less than 1% of all NHLs. We describe a case of diffuse large B-cell lymphoma (DLBCL) in the maxilla, and highlight the clinical signs, symptoms, differential diagnosis, and appropriate treatment of DLBCL in the oral cavity and maxillofacial region. PATIENT CONCERNS: A 67-year-old woman was admitted to our surgical department with pain and swelling in her right upper posterio…

Actinomycotic Osteomyelitis of Maxilla Presenting as Oroantral Fistula: A Rare Case Report.

Actinomycosis is a chronic granulomatous infection caused by Actinomyces species which may involve only soft tissue or bone or the two together. Actinomycotic osteomyelitis of maxilla is relatively rare when compared to mandible. These are normal commensals and become pathogens when they gain entry into tissue layers and bone where they establish and maintain an anaerobic environment with extensive sclerosis and fibrosis. This infection spreads contiguously, frequently ignoring tissue planes and surrounding tissues or organ. The portal of entry may be pulpal, periodontal infection, and so fort…

Exact three-dimensional skull-related repositioning of the maxilla during orthognathic surgery.

The LeFort I osteotomy is standard for corrective repositioning of the maxilla, but intraoperative, skull-related, three-dimensional repositioning of the maxilla remains unsolved. Different ways of improving the accuracy of intraoperative, preoperatively planned, skull-related, correcting movements have been described, including the measurement of vertical maxillary shift, use of positioning devices (such as a face bow or a halo frame), and computer-assisted navigation. Nevertheless, intraoperative three-dimensional control of maxillary shift is not standard. Intraoperatively adjusted position…
