There are also two approaches to the blocking of the maxillary nerve and the trigeminal ganglion under ultrasound guidance: the superazygomatic approach and the infrazygomatic approach. The infrazygomatic approach can be further divided into the anterior approach and the posterior approach. It is generally believed t …

Among 234 haemophiliacs (A and B) treated between 1948 and 1975 at the First Medical Clinic of the University of Vienna, 25 (10.8%) with 36 peripheral nerve lesions have been observed. This amounts to 15.3% or 2.4 lesions within 100 observation years. The average age of the patients at the time of development of lesion was 26.4 years. Correlations between severity of haemophilia and frequency of occurrence of lesions were found. Patients with mild haemophilia did not show peripheral nerve disturbances. The numbers of times individual nerves were affected are as follows: maxillary (1), radial (

Three case reports are presented to illustrate the existence and importance of reflex bradycardic responses that can occur during maxillofacial surgical procedures. All three patients were healthy young adults undergoing operations which did not include any manipulation of orbital structures. After the patients had been anaesthetized for some time and were haemodynamically stable, profound bradycardia or ventricular asystole occurred suddenly in response to manipulations of the bony structures of the maxilla or mandible, or dissection of, or traction on, the attached soft tissue structures. Pulpal inflammation was induced by cutting a class V cavity to within 0.1-0.3 mm of the pulp on the mesial aspect of maxillary and mandibular first molars at the cervical line. The exposed dentine was briefly acid-etched and left open to the mouth until the animals were killed. Histological examination of teeth 4 days after injury showed microabscesses, blood vessel dilation and increased numbers of terminal nerve sprouts around the lesion and in radicular pulp and dentine. Specimens at 7, 11, 21 and 35 days after injury showed progressive healing of the lesions with the formation of reparativ

Spread of herpes simplex virus to and within the mouse eye after inoculation of the cornea or the skin of the snout was examined by peroxidase-antiperoxidase (PAP) staining of viral antigen in flat mounts of the eye and by isolation of virus from nervous tissue. Following inoculation of virus at either site, viral antigen was found in ocular nerves. One to three days later antigen was also found in the iris, ciliary body and choroid/sclera suggesting that virus spread to these tissues occurred via their nerve supply. Viral antigen was also found in the retina of the uninoculated eye after corn

Recent evidence suggests an association between migraine and tension-type headache and local inflammation occurring in a maxillary nerve segment. This study was designed to evaluate the efficacy of topical ketoprofen for the prevention of migraine, tension-type, and posttraumatic headache. Patients with a headache frequency of at least once a week recorded the frequency, severity, duration, and type of headache for 60 days. After 30 days, patients applied the medication daily for the next 30 days to the periapical area of the maxillary molars on the symptomatic side(s). Headache medications an…

Transantral ligation of the internal maxillary artery is the most widely used surgical technique for control of intractable epistaxis. Although this technique is highly efficacious, significant complications may occur, including oroantral fistula, damage to the infraorbital nerve, and recurrent bleeding. An endoscopic transnasal approach for ligation of the terminal branch of the internal maxillary artery, the sphenopalatine artery, provides an alternative to transantral ligation. We have used endoscopic ligation of the sphenopalatine artery to treat 38 patients requiring surgical therapy for

Surgical treatment of mechanical trigeminal nerve injuries resulting in neuropathic pain varies with the type of injury (i.e., amputation or compression), presence of painful neuromas, and adjacent hard/soft tissues. Microsurgical principles that include access, preparation, and repair of the various nerve injuries are recommended for surgical treatment. The choice of surgical repair depends on the microscopic pathology and the presence or absence of adequate distal nerve tissue.

During a prospective study after separation of the nasopalatine nerve at the foramen incisivum during exposure or removal of impacted and palatal displaced maxillary canines, 59 patients were examined neurologically for 4 weeks postoperatively over an investigation period of 18 months. During the first week after the operation, subjective as well as objective sensory disorders were found in all of the patients, but after 4 weeks at the most no neurological deficit could be detected in any patient.

AIM: The aim of this study was to evaluate the efficacy, stability, and clinical outcomes of a bioresorbable plating system in the treatment of pediatric maxillary fractures.
MATERIALS AND METHODS: Twenty pediatric patients with maxillary fractures (13 males and 7 females) were included in this study. The 1.5- and 2.0-mm resorbable plates were used and secured with monocortical resorbable screws 6 mm and 7 mm in length. All patients were followed up for 6 months. Clinical parameters, such as pain, swelling, soft-tissue infection, malocclusion, nerve injury, and bite force for stability, were

AIM: The aim of this clinical trial was to compare the efficiency of the intraligamentary (periodontal ligament) injection with supraperiosteal injections in extraction of maxillary teeth, using pain during injection and extraction as the parameters.
MATERIALS AND METHODS: Thirty patients indicated for extraction of maxillary molars were randomly allocated into one of the following intervention groups (n = 15): intraligamentary injection and supraperiosteal injection. In both groups, anesthesia was given using a standard volume of 2% lignocaine with adrenaline 1:2,00,000 with a 27G needle. Pa

We report a 46-year-old woman with chronic inflammatory demyelinating polyneuropathy (CIDP) in whom swelling of the first branch of the trigeminal nerves in the bilateral orbits were observed as subcutaneous masses in the upper eyelids. The disease developed when the patient was 33 years old, and weakness of the four limbs, double vision, unilateral hypoglossal neuroparalysis, and unilateral facial paralysis frequently occurred during the course of the illness. On nerve conduction studies, conduction block was detected in the motor nerves. Steroid therapy and immunoglobulin treatment improved

To elucidate parasympathetic cerebrovascular innervation of the rat sphenopalatine ganglion (SPG), we injected wheat germ agglutinin-conjugated horseradish peroxidase into the SPG of one side and traced anterogradely labeled nerve fibers. Three days after the injection, the animals were killed and tissues including the SPG, major cerebral arteries, and ethmoidal arteries were reacted by the tetramethylbenzidine method. A number of cells and nerve fibers labeled with wheat germ agglutinin-conjugated horseradish peroxidase were observed in the SPG. The nasal mucous membrane, the periorbital soft

Introduction: Endoscopic medial maxillectomy (EMM) refers to surgical resection of the medial wall of the maxillary antrum. The primary indications for EMM are recalcitrant maxillary sinusitis, ablation of malignant and specific benign diagnoses such as Schneiderian Papilloma. Maxillectomy is potentially complicated by injuries to the orbital contents, lacrimal apparatus, optic nerve, ethmoidal arteries that may be accompanied by brisk bleeding.
Aim: The purpose of this paper is to conduct a retrospective analysis of patients treated with EMM sur

BACKGROUND: The use of the maxillary artery (MA) as a donor has increasingly become an alternative method for cerebral revascularization. Localization difficulties emerge due to rich infratemporal anatomical variations and the complicated relationships of the MA with neuromuscular structures. We propose an alternative localization method via the interforaminal route along the middle fossa floor.
METHODS: Five silicone-injected adult cadaver heads (10 sides) were dissected. Safe and effective localization of the MA was evaluated.
RESULTS: The MA displayed anatomical variations in relation to

Endodermal cysts of the cavernous sinus are extremely rare. We report the case of a 36-year-old man who presented with spontaneous pain in the ophthalmic (V1) and maxillary (V2) divisions of the trigeminal nerve. Based on the preoperative imaging findings, a cystic trigeminal schwannoma was suspected. The tumor was removed via an extradural middle fossa transcavernous approach, and histological examination confirmed the diagnosis of an endodermal cyst. Endodermal cysts should be considered in the differential diagnosis of cavernous sinus tumors.

The Sturge-Weber syndrome is a rare uncommon neurocutaneous disorders with angiomas involving the leptomeninges (Leptomeningeal Angiomas) and skin of the face, typically in the ophthalmic (V1) and maxillary (V2) distributions of the trigeminal nerve. The cutaneous angioma is called a Port-Wine Stain. It is commonly referred to as "Sturge-Weber syndrome after Sturge and Weber who first described this affliction in 1879. This article present a case of Sturge-Weber syndrome associated with gingival enlargement, its management and follow-up results.
