Bony window repositioning without using a barrier membrane in the lateral approach for maxillary sinus bone grafts: clinical and radiologic results at 6 months.

PURPOSE: There has been considerable controversy about the placement of barrier membranes over osteotomy and graft sites in sinus bone grafts via the lateral window approach. Particularly when the sinus membrane remains intact, clinicians should consider the benefits and costs of a barrier membrane. This article presents clinical and radiologic findings following repositioning of a detached bony window without a barrier membrane in the lateral approach for maxillary sinus bone grafts. MATERIALS AND METHODS: After a complete 360-degree osteotomy on the lateral maxillary wall, a bony window was

Metastasis of maxillary carcinoma to the parapharyngeal space: rationale and technique for concomitant en bloc parapharyngeal dissection.

PURPOSE: En bloc resection of the primary tumor and regional lymph nodes is the classic method of surgery in cases of head and neck cancer, but it is not performed in cases of carcinoma of the maxillary gingiva or antrum for anatomic reasons. One of the reasons for the poor prognosis of patients with maxillary cancer and N+ stage necks is thought to be recurrence in the parapharyngeal space, which is out of the surgical field in radical neck dissection. The purpose of this study was to discuss the rationale and indication for en bloc resection and parapharyngeal dissection for maxillary cancer

Postoperative Pain Management in Orthognathic Surgery: Is There a Need for Opioids?

INTRODUCTION: Orthognathic surgery is traditionally considered painful, often prompting opioid use. In the context of rising opioid-related harm, this study assessed postoperative pain levels, factors influencing analgesic requirements, and the actual need for opioids after orthognathic procedures. METHODS: A retrospective analysis of 122 patients undergoing Le-Fort-I osteotomy, bilateral sagittal split osteotomy (BSSO), or bimaxillary surgery was performed. Pain levels (Numeric Analog Scale) were recorded 3 times daily for 5 postoperative days. Analgesic use was documented according to the W

Virtual Surgical Planning in Free Tissue Transfer for Orbito-Maxillary Reconstruction.

Since its introduction, virtual surgical planning (VSP) has been rapidly adopted as a part of reconstructive surgeon's armamentarium. VSP allows reconstructive surgeons to simulate resection, plan osteotomies, and design custom plates. These unique advantages have been especially beneficial for head and neck reconstructive surgeons as there is small room for error and high technical demand in head and neck reconstruction. Despite its popularity, most surgeons have limited experience in using VSP for orbito-maxillary reconstruction as tumors that involve the midface are relatively rare compared

Sinus Floor Elevation via an Osteotome Technique without Biomaterials.

According to classic Hirschfeld studies, the first teeth to be lost are the first and second maxillary molars. After the teeth are extracted and the alveolar process is developed, the maxillary sinus is reabsorbed and pneumatized with a decrease in bone availability in the posterior sector of the maxilla. This process often creates the need to perform regeneration techniques for the placement of implants in this area due to the low availability of bone. The most frequently used and documented technique for the elevation of the sinus maxillary floor is elevation by the side window, as proposed

A modified bone lid for osteoplastic surgery of the maxillary sinus.

Major problems associated with the use of a bone lid for osteoplastic surgery of the maxillary sinus, as first described by Lindorf, are the possibility of postoperative dislocation of the bony fragment into the sinus (floating bone), as well as inflammatory resorption or fracture of the very thin removed trap door fragment. We describe a technique that prevents dislocation of the bone lid and allows for very good visualization of the entire sinus by a simple extended design and the use of resorbable pins. With the use of this extended bone lid, the removal of severely dislocated teeth, implan

A novel trephine design for sinus lift lateral approach. Case report.

Various techniques are described in the literature, either by crestal or lateral approach. Sinus augmentation has a high percentage of success, but presents a number of intraoperative and postoperative complications. The most frequent complication is the Schneiderian membrane perforation with a percentage of perforations between 11% and 56% according to authors. The aim of this study is to describe another membrane approach technique for the sinus lateral wall osteotomy that minimizes the risk of Schneiderian membrane perforation. We present a case of a 50 year old patient attended the Univers

Surgical acrylic guide for distractor positioning.

Internal maxillary distraction with the Le Fort I osteotomy is a technique that can provide simultaneous skeletal advancement and gradual expansion of the soft tissue. For calibrating the vectors of the internal distractors and transferring the desired vectors to the patient, surgical guides may be fabricated before surgery on a stereolithographic model. The anterior nasal spine may be used as a critical anatomical landmark for applying this type of surgical guide.
