A case in which a tooth traumatically displaced into the floor of the mouth retained its vitality for 15 years is reported. Although the presence of the tooth and soft-tissue cuff which appeared to represent the original attached gingiva presented a source for complications, no significant pathologic changes were noted. The requirement for a thorough radiographic examination before definitive treatment of maxillofacial injuries is emphasized.

Immediate complications of trauma and shock include right heart failure, acute respiratory distress syndrome, coagulopathy, brain edema, small bowel dysfunction, renal failure, and immune dysfunction. Nosocomial infections are also common in trauma patients. These infections are the combined result of a compromised host defense and various iatrogenic risk factors, including the use of percutaneous drainage tubes, malnutrition, the injudicious use of prophylactic antibiotics, and treatment staff hygiene. The use of prophylactic antibiotics appears to be unwarranted in head, maxillofacial, and n

In an Austrian study of 14,654 patients with 35,129 injuries treated at a single Department of Cranio-Maxillofacial and Oral Surgery over a 15-year period, the following was seen.1 Older people were more prone to soft tissue lesions with a rising risk of 2.1% per year older, equal in men and women. Younger patients were at higher risk of dentoalveolar trauma with an increase of 4.4% per year younger, and this was 19.6% higher for women. The risk of sustaining facial bone fractures increased each year by 4.6% and was 66.4% times higher in men. A total of 2550 patients (17.4%) suffered 3834 conc

Children have differences in their physiology, anatomy and they are continuing to grow and develop. The management of pathology, trauma, deformity and upper airway obstruction in the young growing patient has to take into account these factors in the treatment planning for these conditions. It is important for Oral & Maxillofacial Surgeons to understand the potential impact of disease or injury on the developing facial skeleton and dentition. This chapter will provide an overview of the some of the major components of paediatric maxillofacial surgery and their management.

Maxillofacial trauma involving tongue injuries poses a significant challenge for airway management due to active bleeding, anatomical distortion, and the risk of obstruction and aspiration. We report the case of a 37-year-old man who presented with a self-inflicted tongue avulsion, likely in the context of an epileptic seizure. The patient was orotracheally intubated and transferred by helicopter under sedation and mechanical ventilation to a tertiary center, where the surgical team requested nasotracheal intubation to facilitate intraoral access. However, active bleeding and avulsion of tongu

UNLABELLED: The role of imaging in the management of maxillofacial trauma is to describe anatomical lesions and to detect complications and associated injuries. Plain films are still useful for minimal trauma, but CT-scan is the gold standard for complex trauma. Helical CT and multidetector row CT simplify the emergency imaging of horizontal struts (skull base, orbital floor, alveolar ridge and palate). The diagnosis, and sometimes the treatment of complications may require CT cisternography, MRI and angiography.

Injuries to the face may cause long-term defects both aesthetic and functional consequences when the treatment is delayed, inadequate or absent. The varieties of osteotomy techniques are applied to improve posttraumatic malposition of the maxillofacial bones and occlusal function. In this article, a 21-year-old female involved in a traffic accident presented a severe deformity in maxilla and treated by anterolateral osteotomy is presented.

Mandibular fractures are some of the most frequently occurring injuries in the maxillofacial region although pathological fractures state only 2% of them. Bone cysts are common lesions of mandible and maxilla. Their long asymptomatic development causes extensive deformation and bone damage. A case report presents fracture of mandible affected with massive odontogenic cyst. Presentation, diagnosis and management of fracture and intraosseus pathology is discussed.

Management of craniofacial injuries typically defaults to plastic, ophthalmology, and oral maxillofacial surgeons which can challenge these surgical subspecialists' capacity to care for both trauma victims and non-trauma patients. Evaluating the need to transfer patients to a higher level of trauma care for isolated craniofacial injuries warrants investigation. Our 5-year retrospective study measured the frequency of craniofacial injuries and subsequent surgical interventions in elderly trauma patients' ≥65 years old. Eighty-one percent of patients consulted with plastic surgeons and 28% with

The study aims to clarify the injury profile and the causes leading to missed injuries in isolated orbital fracture patients. The hypothesis was that orbital fracture patients are prone to underdiagnosis, especially regarding injuries to other body parts. Both injury profiles and causes leading to missed injuries in isolated orbital fracture patients were evaluated in a prospective cohort study. The patients were examined systematically by the oral and maxillofacial surgeon and an on-call orthopaedic traumatologist. The patients were also referred to ophthalmology for consultation. The study i

Facial penetrating gunshot wounds (GSWs) are seen in an assault, suicide, and accidental injury. They often carry high mortality given the important anatomical structures located within the neck. The foundations of maxillofacial GSWs are rooted in data from military combat, specifically the last world war. This type of injury is complex for reconstructive surgery due to significant soft tissue and bone loss. Management of maxillofacial GSWs is often challenging and has trended from serial debridement, immediate reconstruction, local tissue flaps, and distant free flap transfers depending on bu

A number of cephalometric analyses are presently being used in the assessment of dentofacial deformities. These cephalometrics are mostly based on hard tissue assessment alone, although a few methods using soft tissue only or partially hard and partially soft tissues exist. Most of the analyses use angular and linear measurements, although some are based mainly on measurements of relationships. When the various cephalometric analyses are compared, considerable inconsistency comes to light; so much so, that cephalometrics sometimes cannot be considered as a primary diagnostic tool. A combinatio

Injuries to the mental nerve are not an uncommon complication in maxillofacial surgeries. Manipulation close to the mental nerve poses a great risk of nerve injury from drills, bone cutting and trimming burs, and oscillating/reciprocating saws. Nerve injuries can be painful and affect the patient's quality of life. The accompanying complication of the nerve injury depends on the severity of the damage inflicted and can range from transient hypoesthesia to neuropathic pain or trigeminal neuralgia. It is considered that direct injury to the nerve may lead to permanent damage and more severe post
