Relationship between the antitrichinellous effect of seven derivates of benzimidazolecarbamates and their chemical structure.

The efficacy of seven more recently developed anthelmintics of the benzimidazolecarbamate group, i. e., parbendazole, mebendazole, fenbendazole, oxibendazole, cambendazole, oxfendazole and albendazole, has been tested in a series of experiments on white mice artifically infected with Trichinella spiralis. Our results disclosed a relationship between their anthelmintic effect and their chemical structure. This finding might be of importance in a targeted synthesis of new, effective, derivates of benzimidazole, e. g., in the therapy of trichinellosis and in the choice of the most effective drug.

[Modern therapy of intestinal helminthiasis (author's transl)].

The modern chemotherapy of intestinal worm diseases is essentially based on niclosamide, thiabendazole and mebendazole. Thus an effective therapy with few side effects has become possible for these helminthiases. In infections with Taenia, Ascaris, Trichuris, Ankylostoma, Strongyloides, Trichostrongylus and Oxyuris, as the most frequently imported helminths, an elimination rate of over 90% of the parasites can be attained. Infections with nematode larvae as with Larva migrans and trichinoses are more problematic therapeutically because the anthelmintics are directed principally against the adu…

Common-source outbreak of trichinosis associated with eating raw home-butchered pork.

Four patients had trichinosis after consuming raw home-butchered pork. The patients had fever, myalgias, periorbital edema, and conjunctivitis. All of the patients had nausea, vomiting, or diarrhea (corresponding to the intestinal phase of the infection) seven to ten days before the onset of fever and myalgias. Laboratory findings included eosinophilia, elevated serum CPK and aldolase values, and seroconversion of Trichinella serology one month after onset of myalgias. The patients were treated with mebendazole and prednisone and recovered uneventfully.

Synthesis and anthelminthic activity of alkyl-(5-acyl-1-H-benzimidazol-2-yl) carbamates.

A series of alkyl-(5-acyl-1-H-benzimidazol-2-yl)-carbamates were prepared and screened for anthelminthic activity. Some of them were found to be fully active at low, atoxic oral dose levels against gastro-intestinal nematodes. The activity against Symphacia muria and Strongyloides ratta is indicated. From these studies methyl (5-benzoyl-1-H-benzimidazol-2-yl) carbamate (mebendazole) and methyl [5-(4-flourobenzoyl)-1-H-benzimiazol-2 yl]carbamate flubendazole) were selected for detailed investigation.

[Biliary ascaris-induced obstructive jaundice: a case of acute abdomen].

Ascaris lumbricoides is one of the most common helminthic diseases worldwide and it can cause various complications. The presence of this worm in the biliary tree, however, is a rare condition, especially in children. We describe a child with biliary ascariasis who was presenting clinical symptoms mimicking surgical acute abdomen. The diagnosis was made by ultrasound. The child was successfully treated with mebendazole and anti-spasmolytic. Biliary ascariasis should be added to the differential diagnosis of acute abdominal pain in patients, even in children, from endemic areas.

Changes in the acetylcholinesterase activity of the nematode Nippostrongylus brasiliensis following treatment with benzimidazoles in vivo.

Acetylcholinesterase (acetylcholine hydrolase, EC 3.1.1.7) activities of Nippostrongylus brasiliensis were measured following treatment of the hosts with oxfendazole and mebendazole. Drug treatment of N. Brasiliensis caused large, sustained increases in enzyme activity in male and female worms which coincided with the expulsion of the nematodes from the small intestine of the host. It is suggested that the increases in acetylcholinesterase activity may be due to an inhibition by the benzimidazole carbamates of the secretion of enzyme to the exterior of the worms which, in turn, leads to expuls…

The non-oral infection of larval Echinococcus granulosus induces immune and metabolic reprogramming in the colon of mice.

BACKGROUND: The intestinal tract serves as a critical regulator for nutrient absorption and overall health. However, its involvement in anti … ive genes, 44 immune response-associated genes, and 23 metabolic genes were annotated. Furthermore, mebendazole treatment could reverse the colonic histopathology induced by E. granulosus infection. CONCLUSIONS: Intraperitoneal infection with E. granulosus induced the pathological changes and functional reprogramming in the colon of mice, and mebendazole adm …

The Effect of Drug Pricing on Outpatient Payments and Treatment for Three Soil-Transmitted Helminth Infections in the United States, 2010-2017.

The price of certain antiparasitic drugs (e.g., albendazole and mebendazole) has dramatically increased since 2010. The effect of these rising prices on treatment costs and use of standard of care (SOC) drugs is unknown. To measure the impact of drug prices on overall outpatient cost and quality of care, we identified outpatient visits associated with ascariasis, hookworm, and trichuriasis infections from the 2010 to 2017 MarketScan Commercial Claims and Encounters and Multi-state Medicaid databases using Truven Health MarketScan Treatment Pathways. Evaluation was limited to members with conti…

Evaluation of eight serological tests in the diagnosis of human echinococcosis and follow-up.

The ELISA with use of IgG, IgA, and IgE, latex agglutination, indirect hemagglutination (IHA), total IgE, the radioallergosorbent test (RAST), and immunoelectrophoresis (IEP) were carried out to determine the preoperative diagnosis of infection due to Echinococcus granulosus in 131 patients. Eighty-nine patients received follow-up care for 42 +/- 22 months (mean +/- SD); 72 were treated surgically and 17 with mebendazole only. We analyzed 5 +/- 2 serum samples per patient and analyzed each serum sample using the eight tests. IgG ELISA was the most sensitive (up to 94%) and specific (up to 99%)…

Long-term benzimidazole treatment of alveolar echinococcosis with hematogenic subcutaneous and bone dissemination.

We report a case of advanced alveolar echinococcosis (AE) that presented like a malignant tumor. It was diagnosed histologically from a subcutaneous nodule with skin inflammation on the right leg. Additionally, the patient showed bone metastases in the lower thoracic spine and the left third toe. This is the first case with proven hematogenic spread of AE to a subcutaneous site. The patient was treated with albendazole and remained stable for 6 years. When progression of AE occurred the therapy was changed to mebendazole, resulting in a stable condition for further 4 years.

Progress in the pharmacological treatment of human cystic and alveolar echinococcosis: Compounds and therapeutic targets.

Human cystic and alveolar echinococcosis are helmintic zoonotic diseases caused by infections with the larval stages of the cestode parasites Echinococcus granulosus and E. multilocularis, respectively. Both diseases are progressive and chronic, and often fatal if left unattended for E. multilocularis. As a treatment approach, chemotherapy against these orphan and neglected diseases has been available for more than 40 years. However, drug options were limited to the benzimidazoles albendazole and mebendazole, the only chemical compounds currently licensed for treatment in humans. To compensate…

Medical treatment of pulmonary hydatidosis: complications and surgical management.

Pulmonary hydatidosis is more frequently encountered in children than in adults. Chemotherapy with oral administration of particular antihelminthic agents (mebendazole and albendazole) has proved to be effective. This treatment, however, may be associated with serious complications that require surgical management. The aim of this study was to define the limitations of medical treatment, the subsequent complications, and their management. During a 16-year period (1985-2001), 36 children with pulmonary hydatidosis (Echinococcus cysticus) were medically treated. Oral antihelminthic agents (meben…

Parasitic diseases. Other roundworms. Trichuris, hookworm, and Strongyloides.

Trichuriasis may be asymptomatic or, in heavy infection, lead to profuse, bloody diarrhea and rectal prolapse. Diagnosis is made by finding the distinctive barrel shaped eggs in the stool or in the heavily infested patient, by anoscopy and identification of worms attached to reddened and ulcerated rectal mucosa. Mebendazole is the drug of choice in treatment. Capillariasis, a parasitic infection encountered mainly in the Philippine Islands, is of interest in that the eggs may be confused with the eggs of trichuris. Hookworm disease is generally asymptomatic, but in heavy infection, leads to ir…

Diagnosis and management of hydatid disease of the liver. A 15-year North American experience.

Since 1967, 40 patients with hydatid disease of the liver have been treated at our hospital. Diagnosis was made using clinical criteria, serology, skin tests, and imaging techniques. Thirty-five patients were operated upon. In 18 patients the cyst was uncomplicated (Group I), and in 17 the cyst was infected or communicated with the biliary tract (Group II). Three forms of surgical treatment were used: A) cyst evacuation, scolicidal irrigation, and primary cyst closure, B) evacuation, irrigation, and external drainage, and C) complete or partial cyst resection. Mebendazole was used in six patie…

Helminthiasis. Tips for getting rid of unwelcome guests.

Helminthic parasites usually cause uncomplicated infections of the intestine. Complications can arise from nutritional competition between parasite and host, mechanical obstruction of the intestine or common bile duct (in ascariasis), and an overwhelming, fatal infection among immunocompromised hosts (in strongyloidiasis). Diagnosis is made by testing stool samples, except for Enterobius, which can often be seen in the perianal area. Treatment in uncomplicated cases is straightforward and easy. Several drugs are available, and the regimen depends on which parasites are present. However, none i
