Pneumocystis pneumonia in a prepaid care system caring for a Medicaid-covered population with AIDS.

Pneumocystis pneumonia (PCP) is the most common pneumonia in persons with acquired immunodeficiency syndrome (AIDS) and a frequent cause of hospitalization. The incidence of PCP in patients with AIDS can be substantially reduced when patients comply with standard prophylaxis protocols. However, achieving acceptable prophylaxis compliance in any patient population is difficult, particularly with intravenous drug users (IVDU), homeless, or medically disenfranchised patients. This study defines the rates and locations of treatment of PCP in a prepaid managed care program for a Medicaid-covered po…

Medicaid's effect on the elderly: how reimbursement policy affects priorities in the nursing home.

This article examines the social impact of Medicaid policy on the elderly in long-term care and identifies a previously unrecognized problem produced by Medicaid in New York State. Recent fieldwork in proprietary nursing homes in New York City shows that this state's Medicaid system results in a selection hierarchy on admissions and within nursing homes not only in terms of sponsor of payment but also in value, based on residents' functional level. Specifically, New York State Medicaid's Resource Utilization Groups (RUGs II) system is responsible for a new and startling phenomenon in long-term…

Rural-Urban and Racial-Ethnic Differences in Tele-Behavioral Health Access for Medicaid Beneficiaries.

BACKGROUND: Tele-behavioral health (TBH) expanded significantly during the COVID-19 pandemic, but the extent to which this addressed or reinforced existing rural-urban disparities remains unclear, especially in states with diverse and underserved populations. This study investigates the role of rurality and broadband access in TBH utilization among adult Medicaid beneficiaries in New Mexico-a highly rural and racially diverse state. METHODS: We analyzed New Mexico Medicaid claims data from March 2019 to March 2021 for outpatient behavioral health visits. Using logistic regression models, we a…

Prevalence of Colorectal Cancer Screening Among Latino Adults Following the Medicaid Eligibility Amendment Expansion.

IMPORTANCE: Cancer is the leading cause of death for Latino and Latina individuals in the US. Lack of insurance is one of the leading contributing factors to low prevalence of colorectal cancer screening. In 2021, the state Medicaid coverage programs in California and Oregon were amended to expand income-based eligibility for Medicaid to adults aged 50 years or older regardless of immigration status. OBJECTIVE: To evaluate whether expanding Medicaid eligibility regardless of immigration status to adults aged 50 years or older with low income was associated with improvements in the prevalence …

Medicaid coverage for fixed-dose combination antihypertensive medications - fifty states and Washington, D.C., 2022.

Nearly 93 million adults in the United States (US) have uncontrolled hypertension, a leading risk factor for heart disease and stroke. Increasing the use of fixed-dose combination antihypertensives (FDCs) has been proposed as a strategy to improve hypertension control. Using publicly available 2022 coverage information from 282 state Medicaid fee-for-service and managed care plans, we assessed coverage of FDCs in the 50 US states and Washington, D.C. No FDC had preferred coverage status in all plans. Losartan/hydrochlorothiazide had preferred status in 99.3% of plans. At least two FDCs-contain…

Racial and ethnic disparities in medication for opioid use disorder treatment among medicaid enrollees.

BACKGROUND: Racial and ethnic disparities in medications for opioid use disorder (MOUD) access exist, and more research is needed to understand the extent of national and cross-state variation in disparities in Medicaid, the largest payer for MOUD. METHODS: Using Medicaid claims (2017-2022) in 36 states, we conducted a repeated cross-sectional analysis to calculate opioid use disorder (OUD) occurrence and, among those with OUD, MOUD utilization, by enrollee race and ethnicity. Logistic regression models estimated the odds of MOUD receipt (any MOUD; buprenorphine; methadone; naltrexone) by enr…

Perinatal Quality Improvement Among Medicaid Populations in Hawai'i: Insights from a Statewide Learning Collaborative.

INTRODUCTION: Hawai'i reports favorable perinatal health outcomes compared to national averages (preterm birth rate: 9.8%; infant mortality rate: 4.6 per 1,000 live births), yet disparities persist. Native Hawaiian and Pacific Islander women experience higher rates of preterm birth (12.6%) and lower early prenatal care (62.7%). Med-QUEST, Hawai'i's Medicaid program, supported implementation of a Perinatal Quality Collaborative (PQC) to expand access to evidence-based maternal safety practices across birthing facilities. Hospitals implemented protocols (e.g., training, drills, and equipment cha…

Early School Medicaid Expansions and Health Services for Children With Parental Opioid Use Disorder.

IMPORTANCE: Children experiencing parental opioid use disorder are a growing population at heightened risk of physical and mental health issues over the life course. Yet these children are less likely to receive comprehensive, ongoing health care and their parents are more likely to report barriers to access health care for their children. School-based health services have potential to overcome some of these health care access barriers, including parental burden, transportation, time, costs, and health care discontinuity. In 2014, Medicaid revoked its longstanding free care rule, expanding the…
