Evaluating Primary Care Referrals for Heavy Menstrual Bleeding in Adolescents: A Retrospective Medical Record Review.

OBJECTIVES: Heavy menstrual bleeding (HMB) is defined as excessive menstrual blood loss that interferes with physical, social, emotional, or material quality of life. Several societies, including the Society of Obstetricians and Gynecologists of Canada (SOGC) and North American Society for Pediatric and Adolescent Gynecology (NASPAG), have created algorithms to guide investigations and management. We aim to evaluate the extent to which primary care providers (PCPs) initiate first-tier guideline investigations and management before specialist referral. METHODS: A retrospective review of adoles

An Automated System for Tracking, Recording, and Reminding for Medical Implants (TRACER)-a Pilot Study with Ureteral Stents.

Background: There is an urgent need for a seamless clinical system for tracking implants deployed transiently in patients, to reduce the morbidity related to omission of timely removal. Using a ubiquitous implant of ureteral stents as a pilot project, we developed a novel system (Automated System for Tracking, Recording, and Reminding of Implants [TRACER]) that allows seamless tracking and deployment of reminders to clinicians, without the need for excessive infrastructural overhaul of existing hospital processes. Patients and Methods: Briefly, TRACER comprises the following: (1) a two-pronged

A Targeted Congenital Cytomegalovirus Screening Model Using a Customized Electronic Medical Record Dashboard Report.

OBJECTIVE: Targeted congenital Cytomegalovirus (cCMV) screening in newborns has been proposed to identify neonates who could benefit from early diagnosis and intervention. Oftentimes, implementation of targeted screening is variable and left to the discretion of individual institutions. This study investigates the utility of a centralized electronic medical record (EMR) dashboard to improve compliance of targeted screening in the inpatient setting. STUDY DESIGN: Single center prospective feasibility study SETTING: Tertiary medical center METHODS: A targeted cCMV screening dashboard report was

Digital health for Tuberculosis control: findings from the piloting of an electronic medical record in Luanda (Angola).

BACKGROUND: Digital health, like the use of electronic medical records (EMR), has been highlighted as a potential avenue to improve Tuberculosis (TB) surveillance and treatment outcomes. In Angola this technology remains largely untapped within TB health services, despite TB is a huge public health burden. A previous study described the development of a TB EMR system, and the current study fostered the piloting of the TB EMR system in three health facilities of Luanda from January 2022 to February 2023. METHODS: To assess the adequacy of the EMR system (accuracy and completeness) comparison of…

Retrospective analysis of the impact of electronic medical record alerts on low value care in a pediatric hospital.

OBJECTIVES: Hospital costs continue to rise unsustainably. Up to 20% of care is wasteful including low value care (LVC). This study aimed to understand whether electronic medical record (EMR) alerts are effective at reducing pediatric LVC and measure the impact on hospital costs. MATERIALS AND METHODS: Using EMR data over a 76-month period, we evaluated changes in 4 LVC practices following the implementation of EMR alerts, using time series analysis to control for underlying time-based trends, in a large pediatric hospital in Australia. The main outcome measure was the change in rate of each

Performance of an Electronic Medical Record-Based "Syphilis Flag" in Identifying At-Risk Patients in an Emergency Department.

BACKGROUND: The United States has seen a > 40% increase in syphilis cases since 2017. Early disease identification and treatment are crucial. This review sought to identify emergency department (ED) patients at risk for syphilis. METHODS: A 30-day retrospective review was conducted of visits to a single ED. Patient visits were assessed for predetermined syphilis "flags" to include a history of sexually transmitted infection (STI), current chief complaint or reason for visit (RFV) keyword(s) suggestive of potential STI or a positive pregnancy test result. Flagged charts were assessed for STI t

The (Re)-Relaunching of the DMP, the French Shared Medical Record: New Features to Improve Uptake and Use.

Health care delivery for patients with chronic conditions is complex and confusing. Care is often fragmented, less effective, and sometimes harmful. Given the potential for EHRs and other health IT systems to facilitate information flow between providers, patients, and settings, health IT-based measures are of particular interest for care coordination. Health information exchange (HIE) has the potential to improve the quality of healthcare by enabling providers with better access to patient information from multiple sources at the point of care. However, many barriers to HIE use have been repo

Hepatitis C: improving the quality of screening in a community hospital by implementing an electronic medical record intervention.

Both the Centers for Disease Control and Prevention (CDC) and the U.S. Preventive Services Task Force (USPSTF) have recommended that adults born between the years of 1945-1965 should receive one-time testing for Hepatitis C Virus (HCV). In fact, Governor Andrew Cuomo of the State of New York had signed a bill on October 23, 2013 which mandated NY hospitals and healthcare providers to offer HCV testing to all "Baby Boomers." For our project, we wanted to increase our community hospital's compliance with this law and improve the quality of patient care in doing so. An electronic medical record i

Low correlation between self-report and medical record documentation of urinary tract infection symptoms.

BACKGROUND: Correlations between symptom documentation in medical records and patient self-report (SR) vary depending on the condition studied. Patient symptoms are particularly important in urinary tract infection (UTI) diagnosis, and this correlation for UTI symptoms is currently unknown. METHODS: This is a cross-sectional survey study in hospitalized patients with Escherichia coli bacteriuria. Patients were interviewed within 24 hours of diagnosis for the SR of UTI symptoms. We reviewed medical records for UTI symptoms documented by admitting or treating inpatient physicians (IPs), nurses …

Enhancing uterine fibroid research through utilization of biorepositories linked to electronic medical record data.

BACKGROUND: Uterine leiomyomata (fibroids) affect up to 77% of women by menopause and account for $9.4 billion in yearly healthcare costs. Most studies rely on self-reported diagnosis, which may result in misclassification of controls since as many as 50% of cases are asymptomatic and thus undiagnosed. Our objective was to evaluate the performance and accuracy of a fibroid phenotyping algorithm constructed from electronic medical record (EMR) data, limiting to subjects with pelvic imaging. METHODS: Our study population includes women from a clinical population at Vanderbilt University Medical

Use of an Electronic Medical Record (EMR) to Identify Glycemic Intensification Strategies in Type 2 Diabetes.

BACKGROUND: Current treatment guidelines for type 2 diabetes (T2D) recommend individualized intensification of therapy for glycated hemoglobin (A1C) ≥ 7% in most patients. The purpose of this investigation was to explore the ability of an electronic medical record (EMR) to identify glycemic intensification strategies among T2D patients receiving pharmacologic therapy. METHODS: Patient records between 2005 and 2011 with documentation of A1C and active prescriptions for any diabetes medications were queried to identify potential candidates for intensification based on A1C ≥ 7% while on 1-2 oral
