Behaviour of medial meniscus extrusion in patients with knee osteoarthritis during stair ambulation.

BACKGROUND: Evaluation of medial meniscus extrusion (MME) during motion can detect the behaviour of the MME including an increase in MME (ΔMME) and MME peak timing. These parameters reflect the pathology of knee osteoarthritis (KOA). Stair ambulation requires a greater knee flexion angle and produces higher mechanical stress on the meniscus; however, meniscus behaviour during stair ambulation remains unclear. OBJECTIVE: To reveal meniscus behaviour during stair ambulation in patients with KOA. METHODS: Twenty-five patients with medial KOA and 10 healthy controls were enrolled. The participan…

How Much Meniscal Translation Is Normal? A Pediatric Physiological Arthroscopic Meniscus Mobility Study.

BACKGROUND: Pediatric patients with a discoid meniscus often present with peripheral rim instability. Following saucerization, evaluation of peripheral rim instability is routine. However, the normal mobility of pediatric menisci when probed has yet to be studied. The purpose of this study is to establish normal age-based arthroscopic meniscal mobility and meniscal rim width parameters to aid the treating surgeon in identifying an unstable peripheral rim and determining the appropriate amount of saucerization. METHODS: Forty fresh-frozen pediatric cadaveric knee specimens under the age of 12 …

Biomechanics of horizontal meniscus tear and healing during knee flexion: Finite element analysis.

Meniscus horizontal tear is a common injury that mostly occurs in middle-aged and elderly people, and the effect of repair surgery directly affects the functional recovery of the knee joint and prevention of degenerative joint diseases. However, the stress concentration in a horizontal tear is not well understood. The primary objective of this study was to examine the reparative mechanisms involved in addressing horizontal tears of the meniscus and to elucidate the alterations in mechanical behavior throughout the subsequent postoperative healing stages. Based on clinical MRI scan data of norm…

Differences Between Traumatic and Degenerative Medial Meniscus Posterior Root Tears: A Systematic Review.

BACKGROUND: Intact meniscus roots are a prerequisite for normal meniscal function, including even distribution of compressive forces across the knee joint. An injury to the root disrupts the hoop strength of the meniscus and may lead to its extrusion and the development of osteoarthritis. A medial meniscus posterior root tear (MMPRT) is often thought to have a primary degenerative pathogenesis. However, there is mention of some cases of MMPRTs where the patients have a solely traumatic injury to a previously healthy meniscus. PURPOSE: To describe a subpopulation of patients with traumatic MMP…

Development of a Novel Rat Knee Osteoarthritis Model Induced by Medial Meniscus Extrusion.

OBJECTIVE: The medial meniscus extrusion (MME) is associated with increased stress on the knee joint, which leads to cartilage degeneration. To evaluate the etiology of knee osteoarthritis, it is extremely important to create animal models of the disease that more closely resemble actual clinical conditions in terms of symptomatology, molecular biology, and histology. This study aimed to create a clinically relevant model of MME in rats. DESIGN: Behavioral, molecular biological, and histological changes in the newly developed rat MME model were compared with those in sham and medial meniscus …

Current Reviews in Musculoskeletal Medicine: Current Controversies for Treatment of Meniscus Root Tears.

PURPOSE OF REVIEW: The role of the meniscus in preserving the biomechanical function of the knee joint has been clearly defined. The hypothesis that meniscus root integrity is a prerequisite for meniscus function is supported by the development of progressive knee osteoarthritis (OA) following meniscus root tears (MRTs) treated either non-operatively or with meniscectomy. Consequently, there has been a resurgence of interest in the diagnosis and treatment of MRTs. This review examines the contemporary literature surrounding the natural history, clinical presentation, evaluation, preferred surg…

The functional impact of home-based self-rehabilitation following arthroscopic meniscus root repair.

BACKGROUND: Corona virus infectious pandemic makes outdoors rehabilitation a potential hazard. Patient education to perform simple home-based exercises seems to be an interesting and sometimes a mandatory option. This study provides a comparison between the conventional and home-based virtual rehabilitation after surgical repair of medial meniscus root tears. METHODS: In this prospective study, all patients who underwent medial meniscus posterior root repair with a modified trans-tibial pull-out technique from March 2019 to March 2021 were evaluated. Those who underwent surgery after December…

Use of contralateral lateral meniscus for medial meniscal allograft transplantation: a cadaveric study.

PURPOSE: Meniscal allografts are a preferred alternative to menisectomy in cases of irrepairable meniscal tears in young patients. Biological meniscal allograft transplantation requires a cadaveric donor, limiting its availability for transplantation. We are exploring the possibility of using contralateral lateral meniscus for medial meniscal allograft transplantation, as it can be easily procured from proximal tibial cuts from total knee replacement. METHODS: Ten paired knees from five formalin-fixed Indian male cadavers were dissected. Outer and inner circumferences of the medial and menisc…

Significance of angiographic clot meniscus sign in mechanical thrombectomy of basilar artery stroke.

INTRODUCTION: The relationship between the occlusive clot morphology and the efficacy of mechanical thrombectomy (MT) in basilar artery occlusion (BAO) is not well known. Our aim was to evaluate the clinical significance of the clot meniscus sign in patients with acute BAO. METHODS: 89 patients with acute BAO who underwent MT were retrospectively analyzed. The clot meniscus sign was defined as meniscoid/tram-track like antegrade side-wall contrast opacification of the thrombus. Patients were assigned to two groups based on the presence of the clot meniscus sign. The treatment and clinical out…

Arthroscopic Medial Meniscus Root Repair With Soft Suture Anchor Without Posterior Portal Technique.

Medial meniscal root injury is known to cause an increase in tibiofemoral contact pressure and results in early osteoarthritis. There have been many reports on meniscal root repairing techniques, which can be categorized into 2 groups. One is transosseous suture, and the other is anchor suture repair. Both techniques show improvement in not only clinical performance, but also radiographic finding. However, the meniscal root repair procedure must be performed by experienced physicians. Most techniques require a posteromedial portal, which takes time and may even complicate the procedure. The te

Regeneration of lateral discoid meniscus after arthroscopic partial meniscectomy in an adult patient.

Arthroscopic partial resection is indicated for patients who have symptomatic discoid meniscus with overall satisfactory clinical outcomes. Reports regarding regeneration of discoid meniscus after arthroscopic surgery are limited. There are only two reports for children in the literature. To the authors' knowledge, the present study is the first report in the literature to report regeneration of discoid lateral meniscus after arthroscopic partial meniscectomy in an adult patient. The diagnosis was confirmed by both magnetic resonance imaging and arthroscopy. Surgeons should be aware that regen…
