Nonaneurysmal subarachnoid hemorrhage (NA-SAH) constitutes a heterogeneous group of patients, both perimesencephalic (PMN-SAH) and non-perimesencephalic (nPMN-SAH). Despite many reports and case series, the etiology of NA-SAH remains uncertain. The differences in clinical course and outcome between PMN-SAH and nPMN-SAH are evident and have to be taken into consideration at the time of admission, as aggressive diagnostic evaluation and management are required for latter patient. In terms of diagnostic evaluation, the most important determination is to differentiate PMN-SAH from nPMN-SAH and ane

OBJECT: A precise understanding of the ambient cistern and its associated arachnoid membranes is helpful for accessing perimesencephalic lesions. However, few studies of the arachnoid membranes related to the ambient cistern have been published, and, additionally, some aspects of the ambient cistern also require further examination. The goal of this study was to reinvestigate and expound on the anatomical features of the cistern and membranes.
METHODS: The ambient cisterns and its associated arachnoid membranes were examined in 20 adult cadaveric brains using an operative microscope.

In the brainstem, lateral and vertical eye movements are controlled by separate structures, the former mainly in the pons and the latter in the midbrain. The abducens nucleus (VI) in the pons controls all ipsilateral eye movements, i.e., ipsilateral saccades as well as the horizontal vestibulo-ocular reflex (VOR). This nucleus contains the abduction motoneurons, but also the internuclear neurons involved in adduction, passing through the contralateral medial longitudinal fasciculus (MLF) before relaying in the third-nerve nucleus in the midbrain. Lesions affecting the abducens nucleus result i

Thalamic deep brain stimulation (DBS) has been demonstrated to be effective for the treatment of parkinsonian or essential tremor. To date, however, few data exist to support the application of this method to treat midbrain tremor. A 24-year-old right-handed man underwent radiosurgery and subsequent resection of a recurrently hemorrhaging cavernous angioma located in the left side of the midbrain. The surgery exacerbated severe choreoathetotic resting and action tremors of his right extremities and trunk. The patient underwent placement of a deep brain stimulator into the left ventral intermed

An autopsied case of postencephalitic parkinsonism of von Economo type with a 71-year duration is reported. Several cases of postencephalitic parkinsonism of von Economo type have been reported in Japan but this is the first reported case from western Japan. The patient was a Japanese man who was 74 years of age at the time of death. He developed encephalitis of unknown etiology at the age of 3 years. The first symptom was antisocial behavior, which developed at 30 years of age. At the age of 40 years, the patient showed progressive parkinsonism. Neuropathological findings disclosed marked neu

Three young patients developed strokes of rostral midbrain and thalamus shortly following cocaine abuse. Two had infarctions and one had a hemorrhage, but none had clear risk factors other than cocaine for this relatively uncommon type of stroke. Toxicologic analysis confirmed isolated cocaine use in each patient. In the two cases of infarction studied angiographically, one had normal findings and the other had focal narrowing of the P1 segments of the posterior cerebral arteries bilaterally. Since the P1 segment has a uniquely sparse perivascular sympathetic supply, we suggest that direct adr

Midbrain lesions may give rise to the most complex eye movement disorders observed in clinical neurology. Three main types of dysfunction may be delineated, which may be combined: 1) intra-axial fascicular syndromes of the third and fourth cranial nerves; 2) nuclear syndromes of the third and fourth cranial nerves; 3) prenuclear and supranuclear syndromes, in which vertical gaze dysfunction is the main abnormality, but frequently with associated horizontal gaze dysfunction. The fascicular syndromes of the third nerve may be diagnosed when the oculomotor palsy is associated with some specific n

Twenty-six embryos (6-11 mm) of stage 15 (approximately 33 days) were studied in detail and graphic reconstructions of three of them were prepared. Characteristic features of this stage include closed lens vesicles, presence of nasal pits, and retinal pigment. The neuromeric pattern is still visible. Each cerebral hemisphere is limited by the torus hemisphericus internally and by the di-telencephalic sulcus externally. The medial (diencephalic) eminence of the basal nuclei (previously misinterpreted by others as the lateral) had appeared in stage 14, and the lateral eminence, which is telencep

The combined morphological, biochemical, electrophysiological, and behavioral data summarized above show that implanted embryonic nerve cells in some cases can substitute quite well for a lost intrinsic neuronal system in mammals. The intracerebral implants probably exert their effects in several ways. The functional effects seen with grafts placed into one of the cerebral ventricles (such as those described in the studies of Perlow et al., Freed et al., and Gash et al.) are thus probably explained on the basis of a diffuse release of an active amine or peptide into the host CSF and adjacent b

Microfilaments, which are organized into bundles in the apical ends of neuroepithelial cells, are generally thought to play a major role in generating the driving forces for neural tube closure. Because of their proximity to the luminal surface, the contractile activity of these microfilament bundles results in conspicuous changes in the overall shape of neuroepithelial cells, most notably apical constriction and apical surface folding. In the present study, we have used morphometric methods and computer-assisted image analysis to reveal the distribution of microfilament-mediated forces in the

The objective of this study was to see what, if any, cellular changes occurred in the mouse embryo following a single injection of ethanol, a known teratogen in humans and animals, on day 9 of gestation. No changes were seen until 6 hours after injection, when many degenerating cells and necrotic fragments were seen in the neuroepithelium of the neural groove and of the neural tube. In addition, large clear vacuoles were seen in the cytoplasm of many cells and the pseudopodia at the luminal side of the neural groove appeared swollen. The cytoplasm of the latter also contained vacuoles. When tr

The object of this study was to determine whether the oculomotor nucleus and the extraocular muscles are present in the adult of a mutant mouse strain in which eye formation fails early in development (Chase and Chase, '41). Neuronal counts for the oculomotor (IIIrd) nucleus were obtained from serial paraffin sections made through the midbrains of seven mice of the anophthalmic mutant group (ZRDCT-An) and seven mice of the same strain in which eyes were present (ZRDCT-N). The orbital structures of the anophthalmic and control groups were reconstructed from 1-micrometer plastic serial sections.

On the basis of morphological examinations of 5 cases of primary and 15 cases of secondary hemorrhages to the brain stem histological characteristics of those hemorrhages are presented. A high frequency of preceding disorders of the brain stem circulation in the forms of infarctions at various stages of development, perivascular hemorrhages and cysts due to changes (mostly of hypertensive origin) in the cerebral arterie is noted. These changes are regarded as factors promoting the spreading, and, possibly, the development of primary hemorrhages to the stem, as well as aggravating the degree of
