Major deployment-related amputations of lower and upper limbs, active and reserve components, U.S. Armed Forces, 2001-2017.

Major amputations of the lower and upper limbs are among the most life-altering and debilitating combat injuries. From 1 January 2001 through 31 October 2017, a total of 1,705 service members sustained major deployment-related lower and upper limb amputations. Lower limb amputations were far more common than upper limb amputations, with a total of 1,914 lower limb amputations, compared to 302 upper limb amputations. The greatest single-year number of amputations occurred in 2011, with a reported total of 273 service members who sustained 403 major limb amputations. The injured cohort mostly co

Help-seeking among airmen in distressed relationships: Promoting relationship well-being.

Although a substantial proportion of service members returning from a combat deployment report individual emotional and behavioral disorders as well as intimate relationship difficulties, previous studies indicate that only a minority actually seek mental health services. Little is known about factors that predict help-seeking in this population. We first review key findings from the literature on help-seeking in military and veteran populations, including mixed findings regarding the role of perceived stigma and attitudes toward mental health treatment. We then present data from a longitudina

Impact of coping style and PTSD on family functioning after deployment in Operation Desert Shield/Storm returnees.

The relationship between military combat and postdeployment family functioning difficulties has been frequently investigated in the literature, as has the relationship between types of coping and posttraumatic stress disorder (PTSD). Few studies, however, have examined these variables together, and no studies of which we are aware have examined the effect of coping on family functioning after combat exposure. This study examined coping style measured immediately after return from deployment, and PTSD symptoms and family functioning 18-24 months after return from deployment in a sample of Opera

Utility of the mild brain injury atypical symptoms scale as a screening measure for symptom over-reporting in operation enduring freedom/operation iraqi freedom service members with post-concussive complaints.

Evaluation of post-deployment conditions such as post-concussive syndrome (PCS) and posttraumatic stress disorder (PTSD) frequently relies upon brief, self-report checklists which are face valid and highly susceptible to potential symptom validity issues such as symptom exaggeration. We investigated the psychometric prope1rties of a 5-item measure of symptom exaggeration (mild brain injury atypical symptoms [mBIAS] scale) embedded in commonly used PCS and PTSD screening instruments in a sample of 403 patients seen in a brain injury clinic at a large military medical center. Exploratory factor

How and why Rapport Outperforms Persuasion in Military Interrogations.

The present study examined how rapport behaviors, operationalized through the HEAAR principles (Honesty, Empathy, Adaptation, Autonomy, Reflection), and persuasion techniques (alpha and omega) predict information disclosure during high-fidelity military interrogation training. Seventy-six hours of recorded interrogations (25 prisoners, 6 trainee interrogators) from a high-fidelity military exercise in Sweden were segmented, coded, and analyzed using Bayesian cumulative-probit regression with random intercepts for interrogator, detainee, and segment. Information yield was coded using ORBIT's or

Forward psychiatry - early intervention for mental health problems among UK armed forces in Afghanistan.

BACKGROUND: This observational study examined return to duty (RTD) rates following receipt of early mental health interventions delivered by deployed mental health practitioners. METHOD: In-depth clinical interviews were conducted among 975 UK military personnel referred for mental health assessment whilst deployed in Afghanistan. Socio-demographic, military, operational, clinical and therapy outcomes were recorded in an electronic health record database. Rates and predictors of EVAC were the main outcomes examined using adjusted binary logistic regression analyses. RESULTS: Overall 74.8% (n

Predictors of Veterans Health Administration utilization and pain persistence among soldiers treated for postdeployment chronic pain in the Military Health System.

BACKGROUND: Chronic pain presents a significant burden for both federal health care systems designed to serve combat Veterans in the United States (i.e., the Military Health System [MHS] and Veterans Health Administration [VHA]), yet there have been few studies of Veterans with chronic pain that have integrated data from both systems of care. This study examined 1) health care utilization in VHA as an enrollee (i.e., linkage to VHA) after military separation among soldiers with postdeployment chronic pain identified in the MHS, and predictors of linkage, and 2) persistence of chronic pain amon

A Participatory Exploration of Woman Veterans' Peer Support: An Analysis Guided by Ricoeur's Philosophy.

AIM: This study sought to understand the experience of women veterans transitioning from military to civilian life. DESIGN: A community-based research design and Ricoeur's hermeneutic phenomenology guided the philosophical framework. METHODS: Seventeen women veterans were recruited from a non-profit organisation. Data was collected in six virtual focus groups using Zoom in the fall of 2022 using dialogical engagement. Analysis of the data was done using hermeneutic phenomenology. Five participants in the research validated the data and recommended changes to their peer-to-peer support group.

Ethical considerations for psychedelic-assisted therapy in military clinical settings.

Psychedelic treatments, particularly 3,4-methylenedioxymethamphetamine (MDMA)-assisted and psilocybin-assisted therapies, have recently seen renewed interest in their clinical potential to treat various mental health conditions. Clinical trials for both MDMA-assisted and psilocybin-assisted therapies have shown to be highly efficacious for post-traumatic stress disorder and major depression. Recent research trials for psychedelic-assisted therapies (PAT) have demonstrated that although they are resource-intensive, their effects are rapid-acting, durable and cost-effective. These results have g

Incidence of Norovirus-Associated Medical Encounters among Active Duty United States Military Personnel and Their Dependents.

BACKGROUND: Norovirus is a leading cause of gastroenteritis episodes and outbreaks in US military deployments, but estimates of endemic disease burden among military personnel in garrison are lacking. METHODS: Diagnostic codes from gastroenteritis-associated medical encounters of active duty military personnel and their beneficiaries from July 1998-June 2011 were obtained from the Armed Forces Health Surveillance Center. Using time-series regression models, cause-unspecified encounters were modeled as a function of encounters for specific enteropathogens. Model residuals (representing unexpla…

The Challenges of Afghanistan and Iraq Veterans' Transition from Military to Civilian Life and Approaches to Reconnection.

Afghanistan and Iraq veterans experienced traumas during deployment, and disrupted connections with friends and family. In this context, it is critical to understand the nature of veterans' transition to civilian life, the challenges navigated, and approaches to reconnection. We investigated these issues in a qualitative study, framed by homecoming theory, that comprised in-depth interviews with 24 veterans. Using an inductive thematic analysis approach, we developed three overarching themes. Military as family explored how many veterans experienced the military environment as a "family" that

Rumination and depressive symptoms among aging Veterans: The moderating role of social support.

INTRODUCTION: Rumination has been linked to worsened mental health outcomes, but its specific association with depressive symptoms among aging Veterans remains unclear. While social support may buffer adverse effects of rumination on well-being, it is not yet well understood which sources of social support are most effective. This study explored whether different sources of social support (spouse/partner, family, military friends, and civilian friends) moderate the association between rumination and depressive symptoms within an aging Veteran sample. METHODS: Seventy U.S. Veterans aged at lea

Prevalence of sexually transmitted infections among military personnel: a systematic review protocol.

INTRODUCTION: Military personnel are a unique population with heightened vulnerability to sexually transmitted infections (STIs), often exhibiting higher prevalence rates than civilians due to demographic, environmental and occupational factors. These vulnerabilities underscore the need for global prevalence estimates to guide effective, evidence-based interventions. This study aims to quantify the global burden of STIs among military personnel, providing a comprehensive and up-to-date assessment. METHODS AND ANALYSIS: This systematic review will follow the Preferred Reporting Items for Syste

[Cross sectional study of odontological incidences on board the spanish Navy flagship Juan Carlos I during its first international peacekeeping mission].

OBJECTIVE: Oral health is an essential component of the general well-being and quality of life of the population. In Spain, access to dental services and emergency dental care for civilians is relatively straightforward. However, this is not the case for military personnel, especially those deployed on Navy ships that lack dental capabilities and in isolated areas where access to specialized healthcare services is complex and risky. The aim of this study was to evaluate the incidence and management of dental pathology among Spanish military personnel aboard the LHD 61 Juan Carlos I during its

Trend of Incidence of Ocular Injury in Service Members of the U.S. Armed Forces in 2016-2019.

PURPOSE: Ocular injuries pose a significant threat to performance of military functions by the U.S. service members. To estimate the burden of ocular injuries in service members, administrative health records from the Military Health System were collected and analyzed, inclusive of ocular injuries incurred during deployment, military training, or outside of duty hours. METHODS: Patient encounters which matched the predesignated ICD10 codes for ocular injury and complications were extracted and were longitudinally analyzed to categorize patients into complicated (documentation of surgical proc

Advanced bleeding control in combat casualty care: An international, expert-based Delphi consensus.

BACKGROUND: Hemorrhage from truncal and junctional injuries is responsible for the vast majority of potentially survivable deaths in combat casualties, causing most of its fatalities in the prehospital arena. Optimizing the deployment of the advanced bleeding control modalities required for the management of these injuries is essential to improve the survival of severely injured casualties. This study aimed to establish consensus on the optimal use and implementation of advanced bleeding control modalities in combat casualty care. METHODS: A Delphi method consisting of three rounds was used.

Formulating and improving care while mitigating risk in a military Ebola virus disease treatment unit.

This paper describes the development of the UK military's Ebola Virus Disease Treatment Unit (EVD TU) that was deployed to Sierra Leone as part of the UK response to the West African Ebola virus disease (EVD) epidemic in 2014 and 2015. It highlights specific challenges faced within this unique Field Hospital environment. The military EVD TU was initially established to provide confidence to international healthcare workers coming to Sierra Leone to assist in the international response to the EVD epidemic and formed a key part of the action plan by the UK's Department for International Developm

Impact of Pre-Existing Migraine and Other Co-Morbid or Co-Occurring Conditions on Presentation and Clinical Course Following Deployment-Related Concussion.

OBJECTIVE: To describe the clinical presentation and early clinical course of a sample of deployed U.S. military service members following concussion, underscoring the impact of pre-existing migraine and other co-occurring conditions. It is important to obtain a comprehensive clinical history to identify evidence of underlying migraine and other health conditions which may contribute to an individual's presenting symptoms influencing early management and outcomes following concussion. Early outcome measures assessed include headache treatment response and fitness for return to duty. BACKGROUN

Mild traumatic brain injury and posttraumatic stress disorder in returning veterans: perspectives from cognitive neuroscience.

A significant proportion of military personnel deployed in support of Operation Enduring Freedom (OEF) and Operation Iraqi Freedom (OIF) has been exposed to war-zone events potentially associated with traumatic brain injury (TBI) and posttraumatic stress disorder (PTSD). There has been significant controversy regarding healthcare policy for those service members and military veterans who returned from OEF/OIF deployments with both mild TBI and PTSD. There is currently little empirical evidence available to address these controversies. This review uses a cognitive neuroscience framework to addr

Is a Dental Health Risk Assessment predictive of dental health?

A complete dental examination is the basis for identifying the dental readiness of Air Force Reserve Command members. Historically, these were done on an infrequent basis. Recently, an annual dental examination has been mandated for all military members. Current dental personnel manning and resources are not sufficient to accomplish this dramatic increase in dental examination workload. To help alleviate this demand, a Dental Health Risk Assessment questionnaire was developed and tested to see if it could accurately screen out a dental problem severe enough to make a member non-worldwide deplo
