A case is presented of a 25-year-old man with a submerged lower second deciduous molar and an impacted lower second premolar. Treatment was complicated by the relationship of the teeth to the inferior dental nerve, which necessitated extensive bone removal and mental nerve transposition to remove the submerged and impacted teeth. Early recognition and treatment of the submerging tooth might have prevented the need for such extensive surgery and morbidity at a later date.

Ectopic eruption can be defined as a tooth erupting in an abnormal position or orientation. The maxillary first permanent molar is the most commonly affected tooth. Correction of ectopically erupting permanent molars is critical for the development of a stable occlusion and is an important component of interceptive orthodontic treatment. This case report describes an unusual phenomenon where resorption occurred in the crown of an unerupted permanent molar. It was an incidental radiological finding. After eruption the tooth was extracted. Histological examination revealed resorption of enamel and dentine, and partial replacement by calcific material. The possible aetiology of the condition is discussed.

The diagnosis of molar pregnancy is now easy based upon a triple clinical, laboratory and ultrasonographic approach. Histology provides final confirmation. However treatment is more difficult since uterine evacuation may be very hemorrhagic or traumatic.
