Raised intracranial pressure in hepatic encephalopathy.

Intracranial hypertension secondary to cerebral edema is the cause of death in 50%-80% of patients with fulminant hepatic failure (FHF). This is rarely seen in chronic hepatic failure. The genesis of cerebral edema in FHF is poorly understood. The grade of encephalopathy and coagulopathy are the most important predictors of outcome in FHF. However, it is important to emphasize that intracranial pressure (ICP) may not reflect clinical course. Decerebrate posturing may be seen with ICP recording of 16 mmHg, while a quarter of the patients may have brain damage without clinical signs of raised IC

[Fractures of the sternum].

Sternal fractures almost solely occurs as a result of traffic accidents and are considered a marker of more serious injury, especially damage to the myocardium. Patients with an isolated sternal fracture are therefore often admitted to hospital for cardiac monitoring. In drivers and passengers restrained by safety belts sternal fracture is a benign injury: the complication rate is low and healing is usually uneventful. Routine admission and monitoring of patients with isolated sternal fracture is not indicated when the electrocardiogram and chest X-ray, apart from the fracture, are normal. A s

Assessing digital and quantitative EEG in clinical settings.

Assessment of clinical utility involves a series of steps based primarily on published peer-reviewed medical literature. Relevant publications usually use the scientific method, appropriate control groups, blinded reading, prospective design, and other study elements. Assessments are more credible when conducted by those who do not have a conflict of interest in the technique. A detailed assessment of digital and quantitative EEG was conducted recently by the American Academy of Neurology. The American Clinical Neurophysiology Society was a joint sponsor. This assessment concluded that digital

Severe stroke.

Severe stroke is an emergency and requires rapid neurological assessment and diagnosis. CT scan is the first diagnostic step with the aim of finding out the extent, localization and possible pathophysiology of ischaemia in order to direct specific diagnostic and therapeutic options. An intracranial haemorrhage must be excluded. Early CT signs, including the size of the hypodensity and brain swelling, are important prognostic markers. Extracranial and transcranial Doppler sonography are valid for primary assessment of vascular pathophysiology and haemodynamics in most cases. Cerebral angiograph

[Apparatuses for obtaining continuous 24-hour records of water intake and urinary output in dogs].

Two apparatuses were constructed to obtain continuous records of water intake and urinary output in a dog. As a validation of their accuracy, daily patterns of these measurements were observed on a beagle using these two apparatuses during non-treatment and treatment of the diuretic drug, flosemide. During the non-treatment, water intakes increased after feeding, but urinary outputs did not show a regular pattern. These patterns were the same as the previously reported patterns. During the treatment, urinary output increased within one hour after treatment. These results show these apparatuses

The sphinctometer: a new device for measurement of lower oesophageal sphincter function.

Current evidence suggests that lower oesophageal sphincter (LOS) competence depends upon both pressure and length. Existing devices can measure length and pressure, but not simultaneously. This methodological problem is a fundamental drawback to understanding LOS function and its role in oesophageal disease. A 'sphinctometer' has been developed, consisting of an oil-filled 3-mm diameter Silastic chamber, 6 cm in length, incorporating a pressure microtransducer designed to provide an integrated pressure reading as a function of length. Its performance has been tested in vitro using human cadave

Malignant hyperthermia.

Malignant hyperthermia (MH) is an acute syndrome that occurs in genetically susceptible persons who have been exposed to certain anesthetic agents. It can also be induced by catecholamines activated by stress. One in 200 people is at risk for developing malignant hyperthermia. The mortality rate may be as high as 15%. A thorough history obtained before surgery is of extreme importance to prevent an MH crisis and in the management of those susceptible to the disorder. If MH is suspected a diagnosis, which can be confirmed by muscle biopsy, should be ruled out before surgery.

Surface monitoring of cerebral cortical blood flow.

Techniques of monitoring surface blood flow in the brain allow observation of dynamic "real-time" changes in cortical blood flow (CoBF). These techniques have evolved from pial window observations that have not been quantitative and frequently are unreliable. Surface monitoring does not require the development of a clearance curve so changes in flow are seen immediately. On the other hand, the local vascular geometry may affect these techniques; therefore, large surface vessels must be avoided and the probe must be of large enough size so that some averaging effect of the cortical capillary be

[Analysis of the spread of ambulatory electrocardiography in Italy].

The diffusion of ambulatory monitoring (AM) in Italy was evaluated by a questionnaire sent to 102 centers known to be active in this field. The 70 replays obtained constitute this report. In Italy the first center for AM was activated in 1970, thereafter many other new centers started with the maximal increase between 1978 and 1983. During 1985 these centers performed 36223 examinations (mean 517) with a great variability in the number of examinations performed per year: 11 centers performed greater than 1000 examinations a yr, 21 greater than 500 less than 1000 and 38 less than 500. Electroretinogram as a possible monitor of anesthetic depth.

The effects of volatile anesthetics, i.e., methoxyflurane, halothane and enflurane, on the electroretinogram (ERG) were studied in 15 albino rabbits. The ERG was analyzed in terms of the a-wave, and the first oscillatory component (01) in the b-wave. The 01 peak latency showed a significant dose-related prolongation when anesthetic end-tidal concentrations were in excess of 0.8 minimum alveolar concentration (MAC). One MAC, a measure of anesthetic potency, is the end-tidal concentration of an anesthetic at 1 atmosphere that induces immobility in 50% of animals against a noxious stimulus. The a

Burns caused by monitoring equipment during anaesthesia.

Hazards of monitoring during anaesthesia are burns, cardiac arrest or ventricular fibrillation, electric shock and false alarms caused by interference with the traces. The causative and contributory factors and the measures for the prevention of burns are discussed.
