Mucosal metastases in malignant melanoma.

BACKGROUND: We present the case of a patient with malignant melanoma stage IV according to the American Joint Committee on Cancer (AJCC) classification and an unusual pattern of metastasis to the mucosa of the esophagus, the stomach, the bladder and the palatine tonsil. CASE REPORT: A 38-year-old male patient with metastatic malignant melanoma of stage III (AJCC) was admitted for initiation of adjuvant therapy. 4 months earlier a primary melanoma of the left upper leg had been excised and 2 months later the patient had undergone a left inguinal lymph node dissection revealing 2 metastatic lym

Endonasal dacryocystorhinostomy with mucosal flaps.

PURPOSE: To describe a new endonasal dacryocystorhinostomy (DCR) technique and assess its efficacy. DESIGN: Prospective nonrandomized interventional case series. METHODS: A prospective series of 44 consecutive endonasal DCRs performed from January 1999 to December 2000 was entered into the study. The new technique involved creation of a large bony ostium and mucosal flaps to create an anastamosis between the lacrimal sac mucosa and nasal mucosa. Patients presenting with naso-lacrimal duct obstruction based on symptomatic, clinical, and radiologic grounds were included in the study. Patients

Experience with registered mucosal vaccines.

Most pathogens gain access to their host through mucosal surfaces. It is therefore desirable to develop vaccination strategies that lead to mucosal immune responses. Ideally, a vaccine should be administered mucosally in order to elicit mucosal protection. Several attenuated live viral and bacterial pathogens are registered as oral vaccines for human use, including the oral polio vaccine (Sabin) as well as attenuated strains of Salmonella typhi and Vibrio cholerae. These attenuated bacterial live vaccines-S. typhi Ty21a as well as V. cholerae CVD 103-HgR-are employed as vaccines against typhoi

Mucosal melanoma of the sinonasal tract.

Mucosal melanoma of the sinonasal tract is an uncommon clinical entity, which frequently presents in advanced stages and follows an unpredictable course. We describe a case of a 97-year-old white female who presented with a 5-month history of painless, intermittent epistaxis and who was found to have melanoma involving the right inferior turbinate. She required operative intervention because of chronic epistaxis that had resulted in anemia and weakness. The case is presented along with a review of the pertinent literature. The dilemmas involved in the clinical decision-making process and treat

Cutaneous and mucosal pain syndromes.

The cutaneous and mucosal pain syndromes are characterized by pain, burning sensation, numbness or paraesthesia of a particular part of the skin or mucosal surface without any visible signs. They are usually sensory disorders, sometimes with a great deal of psychologic overlay. In this article various conditions have been listed and are described. The possible causative mechanisms are discussed when they are applicable and the outline of their management is described.

Regulation of experimental mucosal inflammation.

Studies conducted over the past 10 years have provided ample evidence that many types of inflammations arising from basic abnormalities of immune regulation are ultimately 'funneled' through a Th1 or Th2 T cell-mediated immune reaction. Thus, by understanding these types of reactions and, in particular, by identifying their natural checkpoints, one can control the inflammation regardless of its more basic causes. A case in point is the inflammatory disease of the intestine known as Crohn disease, a disease now thought to be due to one or more abnormalities leading to an excessive immune respon

Mucosal immunization with DNA vaccines.

The mucosal surface area of the gastrointestinal, genitourinary and respiratory tracts is more than 200 times greater than that of the skin and is the primary site of transmission of numerous diseases. The entry of pathogenic organisms at mucosal surfaces can be prevented by mucosal, but not systemic immunity. Vaccines which are delivered by intramuscular (IM) or subcutaneous (SC) injection induce strong systemic responses but generally no mucosal immunity. In contrast, vaccines delivered at mucosal surfaces trigger both mucosal (at local and distant sites) and systemic responses (1,2). Other

The mucosal phase of Listeria infection.

Listeria monocytogenes is an enteroinvasive bacterial pathogen of man and animals. Listeriae have been shown capable of infecting the host by translocating from the intestinal lumen through Peyer's Patches (PP), however, results of experiments now indicate that these facultative intracellular parasites may also translocate through PP-independent routes. With regards to this, on occasion we observed that listeriae were absent from the PP of mice inoculated intragastrically with L. monocytogenes, but were present in the mesenteric lymph nodes of these same mice. These observations suggested that

[Acute gastroduodenal mucosal lesion].

AGML (acute gastric mucosal lesion) is now recognized as one of the important causal disease for gastrointestinal bleeding. If patients have sudden onsets of epigastralgia, epigastric discomfort, vomiting, hematemesis and melena following probable causes, it seems quite reasonable to make diagnosis of AGML by endoscopy with findings of gastric erosion, hemorrhagic gastritis and gastric ulcer. There are a variety of causes for AGML such as psychological and physical stress, drugs (NSAIDs, antibiotics, adrenal corticoid steroid, anti cancer drug), alcohol, serious organ failure of liver, kidney,

Oesophageal mucosal diseases in the elderly.

Diseases of the oesophageal mucosa are particularly prevalent in the elderly population and may present with various symptoms, such as heartburn, dysphagia or chest pain. Recent technological advances, such as endoscopy, ambulatory pH monitoring and radiological imaging, have allowed for a more accurate diagnosis and assessment of disease severity. Depending on the leading presenting symptom, empirical medical therapy or stepwise diagnostic tests may be used. Elderly patients with mild reflux symptoms respond well to over-the-counter antacids, acid suppressants or prescription prokinetic drugs

Colonic mucosal pH in humans.

Mucosal pH was measured at specific anatomic segments within the colon using a flexible pH probe in patients prepared for colonoscopy. The data revealed similar pH measurements along the length of the colon, irrespective of the presence or absence of colorectal neoplasia. Patients exhibited a relatively acidic right colon; a more alkaline transverse, left, and sigmoid colon; and a relatively acidic rectum. There were no apparent gender- or age-related effects on colonic mucosal pH.

The gastrointestinal mucosal immune system.

The mucosal immune system, consisting of lymphoid tissues associated with the lacrimal, salivary, gastrointestinal, respiratory and urogenital tracts and lactating breasts, quantitatively contains the majority of the lymphoid tissue of the body. There are a number of important features of the gastrointestinal mucosal immune system: the mucosal immune system contains specialized structures, such as the Peyer's patches, where immune responses are thought to be initiated; there is a pattern of relatively specific recirculation of lymphoid cells to the mucosa, known as mucosal homing; subsets of l

A case of mucosal prolapse syndrome.

A case of mucosal prolapse syndrome in the rectum is reported. A 61-year-old female presented complaining of bloody discharge and constipation for 2 months. Colonoscopy revealed an elevated lesion on the left anterior wall of the rectal canal. Biopsy showed glandular proliferation and fibromuscular obliteration of the lamina propria, findings characteristic of mucosal prolapse syndrome. It is a rare disorder affecting the rectum because the lesion looks macroscopically like a protruding adenoma.

[Mucosal bridge of the vocal fold].

Mucosal bridge of the vocal fold is a newly recognized laryngeal disease need to be differentiated from the functional voice disorder. A 16-year-old woman with deteriorating hoarseness presented for indirect laryngoscopy and laryngofiberoscopy of just slight swelling of the bilateral vocal folds. Since the laryngeal stroboscopy revealed the decrease of wave formation at the swelling portion, we performed an endolaryngeal microsurgery and discovered a sulcus in a side of the vocal fold and a mucosal bridge in the other side. Resection of the bridge followed by the voice therapy increased the vo

Mucosal reactions to amalgam restorations.

Amalgam restorations have been implicated both in contact sensitivity reactions and in lichenoid reactions. This appears to be related principally to the mercury content, although other metals cannot be discounted. The cases of two patients are reported who showed features of lichenoid reactions of the oral mucosa, in addition to features of a contact hypersensitivity to mercury. The mucosal lesions resolved following replacement of the amalgams with non-metallic restorations. Consideration is given to the selection of materials and procedures currently available for treatment of these patient

NSAID-induced mucosal injury.

Nonsalicylate, nonsteroidal anti-inflammatory drug (NSAID)-induced injury to the gastroduodenal mucosa may be responsible for significant ulceration and bleeding. The pathophysiology of gastroduodenal injury is reviewed and risk factors predisposing to NSAID injury are discussed. Finally, data are presented on the concurrent use of prostaglandin analogues and sucralfate in preventing mucosal injury.
