Myelopathy is an infrequently reported complication of bacterial meningitis. We report the case of a preterm neonate with group B streptococcal meningitis who developed acute transverse myelopathy with subsequent spinal cord cavitation noted by MRI. The destruction of the cerebellum and obstructive hydrocephalus resulted in a posterior fossa cyst.

A 42-year-old female was admitted to our hospital on October 1, 1990 because of one week history of back pain, weakness of her right lower extremity and sensory disturbance of her left lower extremity. Physical examination revealed swollen hands, Raynaud's phenomenon, sclerodactyly and heliotrope rash. The body temperature was 37.0 degrees C. Neurological findings included weakness in the right lower extremity, left hypalgesia and thermohypesthesia below Th4, hyperreflexia on the right lower extremity and right extensive plantar response. Laboratory data showed leucopenia (3,700/mm3) and hyper

Longitudinally extensive transverse myelitis (LETM) is a rare but severe neurological complication of systemic lupus erythematosus (SLE). The existing literature contains only limited information about this condition. We present a case of a 38-year-old female with SLE who presented with quadriparesis. Magnetic resonance imaging (MRI) of the brain and spinal cord showed T2-weighted high signal intensity involving the brainstem, bilateral middle and inferior cerebellar peduncles, and C1-C7 spinal cord segments. Early intervention with high-dose methylprednisolone and cyclophosphamide was initiat

A 74-year-old man was admitted to our hospital with complaints of weakness in the lower extremities, urinary retention for 10 days, and generalized vesicular rash for 7 days. Spinal magnetic resonance imaging showed contrast enhancement at the Th12-L1 level of the spinal cord and cauda equina. Serum and cerebrospinal fluid varicella-zoster virus (VZV)-immunoglobulin (Ig) G antibody titers were markedly elevated, and VZV-IgM was detected in cerebrospinal fluid. The patient was diagnosed with VZV transverse myelitis and cauda equina syndrome with subsequent varicella and was treated with acyclov

Subacute presentation with gait preservation is rare in the initial presentation of transverse myelitis (TM) in children. Lyme TM is poorly described in the literature. Here, we present the case of a 10-year-old boy who presented with neck pain with irradiation in the upper limbs for 13 days, accompanied by a right latero-torticollis. Magnetic resonance imaging (MRI) of the spine showed a hypersignal in the centromedullary T2 weighted image (WI) between C1 and C7, which was suggestive of cervical TM. A lumbar puncture revealed pleocytosis and proteinorachia. The test results of Borrelia IgG in

Spinal cord involvement, either isolated or together with brain, in Behçet's disease (BD) has been reported. In these cases the existence of the disease was previously known or the classical triad of disease such as oral and genital ulcers with uveitis/iritis was present. Here we describe a 22-year-old man in whom acute transverse myelitis diagnosed with MRI was the first finding of BD.

CASE REPORT: A 25-year-old man with Behçet's disease was admitted because of weakness of the lower limbs and difficulty in urination. He had received a rabies vaccination 2 months previous because he had been bitten by a dog.
FINDINGS: Clinical and laboratory findings supported acute transverse myelitis. A hyperintense lesion and expansion at the level of conus medullaris was detected on spinal magnetic resonance imaging.
CONCLUSION: Although neurologic involvement is one of the main causes of mortality and morbidity in Behçet's disease, the factors that aggravate the involvement of the nerv

A 30-year-old Caucasian male with systemic lupus erythematosus suffered acute catastrophic-onset transverse myelitis. Two years earlier aseptic meningitis, another rare CNS lupus manifestation, had been diagnosed. MRI showed involvement of the medulla oblongata and the entire spinal cord. Therapy with intravenous high-dose methylprednisolone and intravenous cyclophosphamide is discussed.

Transverse myelitis is an inflammatory disorder of the spinal cord that can result in significant neurologic morbidity. Diagnostic evaluation is particularly challenging in immunocompromised patients because of a broad differential diagnosis that includes infectious, neoplastic, vascular, and treatment-related etiologies. We report the case of a 64-year-old Filipino male with a history of parotid carcinoma treated with surgery and radiotherapy, acute myeloid leukemia treated with hematopoietic stem cell transplantation, and chronic immunosuppression who presented with acute-onset progressive b

BACKGROUND: Acute transverse myelitis (ATM) has been reported as a potential association between COVID-19 vaccination. In this study, we aimed to investigate the association between the COVID-19 vaccination and ATM.
METHODS: A self-controlled case series study was performed using a large database that combine the COVID-19 vaccine registry and the national claims database. The COVID-19 vaccination data included information on individuals aged 18 and above who received COVID-19 vaccination from February 26, 2021, to August 31, 2022. The claims database covered the entire Korean population for t

We report a case of acute transverse myelitis (ATM) following severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection in a 59-year-old man. This patient had been treated for ulcerative colitis (UC) for 4.5 years with several medications, including a tumor necrosis factor (TNF)-α inhibitor. Because this patient developed ATM almost concurrently with the onset of coronavirus disease 2019, SARS-CoV-2 infection was considered the likely cause of ATM, although TNF-α inhibitor therapy and the autoimmune condition UC might also have played a role.

Widespread white matter changes, including those in the corticospinal tract (CST), have been observed in patients with neuromyelitis optica spectrum disorder (NMOSD). However, whether these alterations originate within the brain or result from spinal cord damage remains unclear. To investigate the CST alteration in AQP4-IgG positive NMOSD with longitudinally extensive transverse myelitis (LETM), and evaluate its relevance to LETM via fixel-based analysis. AQP4+ LETM patients and healthy controls were from ongoing clinical and imaging patterns of neuroinflammation diseases in China cohort betwe

INTRODUCTION: Fibrocartilaginous embolism causing spinal cord infarct is rare, and a high index of clinical suspicion is needed for accurate diagnosis.
CASE PRESENTATION: A 65-year-old woman presented to our hospital with back pain, paraparesis, and neurogenic bladder. Magnetic resonance imaging showed a T4-T7 signal abnormality that was misdiagnosed initially and treated as autoimmune myelitis.
DISCUSSION: Fibrocartilaginous spinal cord infarction is rare and remains a clinical diagnosis with supportive imaging findings. The imaging findings may be nonspecific, and other etiologic diagnosti

INTRODUCTION: Acute transverse myelitis is an uncommon inflammatory, intramedullary, disorder of the spinal cord. Spastic paraplegia, impaired sphincter functions, and sensory loss, with sensory level, are the clinical manifestations of this devastating disorder. The utilization of magnetic resonant imaging (MRI) contributes to the surge in the diagnosis of more ATM cases. Although the causes of ATM are numerous, both Mycoplasma pneumoniae and Schistosoma mansoni are uncommon causes and their co-existence in the same patient has not been reported before in Saudi Arabia.
CASE: We report a 25-y

INTRODUCTION: Respiratory viral illnesses are associated with diverse neurological complications, including acute transverse myelitis (ATM). Among the respiratory viral pathogens, the Coronaviridae family and its genera coronaviruses have been implicated as having neurotropic and neuroinvasive capabilities in human hosts. Despite previous strains of coronaviruses exhibiting neurotropic and neuroinvasive capabilities, little is known about the novel severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) and its involvement with the central nervous system (CNS). The current pandemic has hi

Concomitant acute transverse myelitis (ATM) and Guillain-Barre syndrome (GBS) is described as GBS and ATM overlap. Its presentation varies greatly, thus making the diagnosis difficult. Overlap syndrome is more commonly associated with acute motor axonal neuropathy (AMAN) subtype of GBS. However, we present a case of a middle-aged gentleman with combined ATM and acute motor and sensory axonal neuropathy (AMSAN) subtype of GBS. This combination is quite rare, and only a few cases have been reported so far.

Varicella-zoster virus (VZV) is a human neurotropic herpes virus that causes chickenpox in children. After becoming latent in dorsal root ganglia, it can reactivate to cause dermatological manifestations, the most common one being shingles or herpes zoster. Severe neurologic dysfunctions can occur in immunocompromised patients such as encephalitis, meningitis, myelitis and neuropathy. Longitudinal extensive transverse myelitis (LETM) is an unusual neurological complication mainly described in immunocompromised patients, with very few cases described in immunocompetent ones.
