[Syncopal ventricular tachycardia complicating paroxysmal atrial fibrillation in severe myocardial ischemia].

The authors report two cases of spontaneous paroxysmal atrial fibrillation closely followed by syncopal ventricular tachycardia resulting in cardiac arrest requiring defibrillation. Both patients, men aged 62 and 64 years, had a past history of myocardial infarction without arrhythmias; atrial fibrillation occurred during severe myocardial ischaemia; coronary arteriography showed tight stenoses of the left main coronary artery with normal left ventricular function. Ventricular tachycardia (or fibrillation) during spontaneous paroxysmal atrial fibrillation is a rare occurrence. This sequence of

The Value of Myocardium and Kidney Histopathological and Immunohistochemical Findings in Accidental Hypothermia-Related Fatalities.

Background and Objectives: The post-mortem diagnosis of hypothermia is challenging in forensics. The aim of our study was to detect the kidney and heart histopathological changes that occurred in a group of hypothermia-related fatalities. Materials and Methods: The cohort included 107 cases identified in the database of our department between 2007 and 2021, which have been associated with extreme cold stress. Demographic and clinicopathological data were collected from the medico-legal reports. Archived tissue samples were evaluated to identify the histopathological features, in routine haemat

Association of electrostimulation with cell transplantation in ischemic heart disease.

BACKGROUND: Until now, cell therapy has constituted a passive therapeutic approach; the only effects seem to be related to the reduction of the myocardial fibrosis and the limitation of the adverse ventricular remodeling. Cardiac resynchronization therapy is indicated in patients with heart failure to correct conduction disorders associated with chronic systolic and diastolic dysfunction. The association of electrostimulation with cellular cardiomyoplasty could be a way to transform passive cell therapy into "dynamic cellular support." Electrostimulation of ventricles following skeletal myobla

Preclinical cardiovascular safety assessment of pharmacology-toxicology relationship for a set of novel kinase inhibitors.

Cardiovascular toxicity is one of the more common causes of attrition in preclinical and clinical drug development. Preclinical cardiovascular safety assessment involves numerous in vitro and in vivo endpoints which are being continually reviewed and improved to lower the incidence of cardiovascular toxicity that manifests only after the initiation of clinical trials. An example of notable preclinical toxicity is necrosis in the papillary muscle of the left ventricle in dogs that is induced by exaggerated pharmacological effects of vasodilators or positive inotropic/vasodilating off-target dru

Ultrasound Visualization and Recording of Transient Myocardial Vibrations.

OBJECTIVE: A new visualization and recording method used to assess and quantitate autogenic high-velocity motions in myocardial walls to provide a new description of cardiac function is described. METHODS: The regional motion display (RMD) is based on high-speed difference ultrasound B-mode images and spatiotemporal processing to record propagating events (PEs). Sixteen normal participants and one patient with cardiac amyloidosis were imaged at rates of 500-1000/s using the Duke Phased Array Scanner, T5. RMDs were generated using difference images and spatially integrating these to display ve

Simultaneous biventricular pressure-volume analysis in rats.

The pressure-volume (PV) analysis is used for an accurate assessment of load-independent cardiac function and is important for the study of cardiovascular diseases and various therapeutic modalities. PV analysis is often performed on one of the ventricles, or on both ventricles but sequentially. Since both ventricles interact with each other and their functions are mutually interdependent, especially in various disease conditions such as pulmonary hypertension or heart failure, it is important to quantify the function of both ventricles at the same time. Therefore, our aim was to describe a st

A novel cardiovascular systems model to quantify drugs effects on the inter-relationship between contractility and other hemodynamic variables.

The use of systems-based pharmacological modeling approaches to characterize mode-of-action and concentration-effect relationships for drugs on specific hemodynamic variables has been demonstrated. Here, we (i) expand a previously developed hemodynamic system model through integration of cardiac output (CO) with contractility (CTR) using pressure-volume loop theory, and (ii) evaluate the contribution of CO data for identification of system-specific parameters, using atenolol as proof-of-concept drug. Previously collected experimental data was used to develop the systems model, and included mea

Timing the opening and closure of the aortic valve using a phonocardiogram envelope: a performance test for systolic time intervals measurement.

OBJECTIVE: This work explored the reliability of using points on the heart sounds envelope as indicators of the opening and closure of the aortic valve (AVO, AVC) to measure the pre-ejection period (PEP) and the left ventricular ejection time (LVET). APPROACH: 36 phonocardiograms (PCGs) from healthy subjects and cardiovascular disease subjects were denoised using single-channel independent component analysis (SCICA) and, from the Hilbert envelopes, the positions of the S1 and S2 peaks were detected (pS1, pS2). Complementarily, the positions of the local maxima of S1 and S2 (mS1, mS2) and the

Invasive Hemodynamics of Myocardial Disease: Systolic and Diastolic Dysfunction (and Hypertrophic Obstructive Cardiomyopathy).

Heart failure is a clinical diagnosis that is supported by various laboratory, imaging, and invasive hemodynamic measures. There is no single diagnostic test. A variety of structural and/or functional myocardial abnormalities can lead to the inability of the heart to fill or eject blood. Despite ejection fraction being the most commonly assessed measure of systolic function in clinical practice, it is a poor measure of contractility because it is susceptible to loading conditions and chamber size. Invasive hemodynamic assessment remains of great importance in the evaluation of patients with my

Mitochondrial adaptations to physiological vs. pathological cardiac hypertrophy.

Cardiac hypertrophy is a stereotypic response of the heart to increased workload. The nature of the workload increase may vary depending on the stimulus (repetitive, chronic, pressure, or volume overload). If the heart fully adapts to the new loading condition, the hypertrophic response is considered physiological. If the hypertrophic response is associated with the ultimate development of contractile dysfunction and heart failure, the response is considered pathological. Although divergent signalling mechanisms may lead to these distinct patterns of hypertrophy, there is some overlap. Given t

Case report: transesophageal echocardiography detected severe regional wall motion abnormalities signifying failed reimplantation of an anomalous left main coronary artery.

PURPOSE: Anomalous coronary arteries comprise a spectrum ranging from benign to fatal. The most ominous lesion that occurs is the left main coronary artery (LMCA) originating from the opposite aortic sinus. This defect usually presents as sudden death in a healthy young adult immediately after exercise. We report a case of reimplantation of an anomalous LMCA, discovered in a 15-yr-old girl during investigations for atypical chest pain. CLINICAL FEATURES: The intraoperative transesophageal echocardiography (TEE) confirmed that the anomalous LMCA originated from the right aortic sinus. The anom

Pharmacology of bipyridine phosphodiesterase III inhibitors.

The bipyridine phosphodiesterase III inhibitors amrinone and milrinone form a new class of positive inotropic vasodilator agents that are beneficial in the treatment of acute or decompensated heart failure. These agents inhibit the intracellular hydrolysis of cyclic adenosine monophosphate, thereby promoting cyclic adenosine monophosphate--catalyzed phosphorylation of sarcolemmal calcium channels and activating the calcium pump. They have a wider therapeutic index than do the cardiac glycosides. They also have vasodilator and lusitropic actions and are devoid of the central stimulant actions t

[Echocardiographic evaluation of hemodynamics and myocardial function in children under 1 year of age following surgery under conditions of artificial blood circulation].

Left and right ventricular myocardial contractility was assessed, using two-dimensional echocardiography in the earliest postoperative period after cardiopulmonary bypass surgery in 60 children under 1 year of age. It has been shown that a hemodynamically smooth postoperative period is characterized by a 36 to 40% decrease in left ventricular ejection fraction depending on the type of heart valve defect and the variant of its correction. Such a decrease in contractility required a slight inotropic support in the first 24 hours after the operation; in this case cardiac index exceeded 2.5 l.min-

Usefulness of echocardiographically determined mitral leaflet motion for diagnosis of mitral valve prolapse in 17- and 18-year-old men.

Mitral leaflet motion during systole was studied by echocardiography in 102 healthy young men. Mean posterior maximal leaflet motion was 2 +/- 1 mm behind the CD line on 2-dimensionally (2-D) directed M-mode examination. On the apical 4-chamber cross-sectional view a mean area of 0.34 +/- 0.24 mm2 was contained by the mitral leaflets above the plane of the mitral anulus. Elevated values on the M-mode view (greater than or equal to 4 mm) compared with the 2-D 4-chamber view (greater than or equal to 0.70 mm2) were discordant, with 90% (18 of 20) of the elevated values found in 1 view only. Thus

Tetanus in the mammalian heart: studies in the shrew myocardium.

The purpose of the present study was to test the possibility that tetanus can occur in the shrew myocardium, in which the ventricular action potential is similar to that of skeletal muscle. Action potentials and tension were recorded from the right ventricular muscle using glass microelectrodes and a force transducer. When the regular twitch (at a cycle length of 2000 ms) was followed by a train of stimuli having an internal cycle length of less than 50 to 60 ms, unfused and fused tetanus readily developed. Action potentials were generated at the frequency of the stimuli within the train. With
