Immune-Array Analysis in Sporadic Inclusion Body Myositis Reveals HLA-DRB1 Amino Acid Heterogeneity Across the Myositis Spectrum.

OBJECTIVE: Inclusion body myositis (IBM) is characterized by a combination of inflammatory and degenerative changes affecting muscle. While the primary cause of IBM is unknown, genetic factors may influence disease susceptibility. To determine genetic factors contributing to the etiology of IBM, we conducted the largest genetic association study of the disease to date, investigating immune-related genes using the Immunochip. METHODS: A total of 252 Caucasian patients with IBM were recruited from 11 countries through the Myositis Genetics Consortium and compared with 1,008 ethnically matched c…

[Increased troponin T in a patient with myositis].

A 41-year-old woman presented to the emergency room for long-term dysphagia, with a loss of eight kilos in two months. Myositis or inflammatory myopathies were suspected. A marked increase in troponin T concentration was observed.

Nailfold capillaroscopy in myositis: A case series.

Nailfold capillaroscopy is well established in systemic sclerosis; however, abnormalities (scleroderma or scleroderma-like patterns) are identified in other connective tissue diseases, such as myositis, even in the absence of Raynaud's phenomenon. Although this may be known within the rheumatology world, myositis cases often present to other subspecialists who may be unfamiliar with the value of capillaroscopy. We present four cases of myositis that initially presented to non-rheumatology subspecialists where capillaroscopy played a role in confirming the diagnosis. Nailfold capillaroscopy is …

Myositis Ossificans: A Mimicker of an Intramuscular Tumour.

Teaching point: Myositis ossificans is a do not touch lesion of which the radiological findings can be misleading in the early stages due to possible features mimicking a malignant process.

Viral Myositis in an Eight-Year-Old.

Benign acute childhood myositis (BACM) is a pediatric syndrome characterized by mild self-limiting sudden onset of muscle pain during or following recovery from a viral illness. The case discussed in this report is of an eight-year-old female diagnosed with the common cold after presenting to her primary care physician. Five days later, the patient presented to the emergency department with lower extremity pain. The patient was sent home with supportive care and mild analgesics. Twelve hours later, the patient was seen again in the emergency department with severe bilateral lower extremities p…

Imaging Modalities in Myositis: A Clinical Review.

This review highlights the key role of imaging modalities in diagnosing and managing myositis. The authors underscore MRI's superiority in identifying muscle edema and fat infiltration, marking it as essential for evaluating disease activity and damage. They also suggest ultrasound's emerging significance for diagnosis and monitoring of the disease, valued for its ease of use, and real-time capabilities. Furthermore, PET scans' unique physiologic capabilities, especially useful for malignancy detection and assessing lung disease, are emphasized. Collectively, these imaging techniques offer a t…

Newer Autoantibodies and Laboratory Assessments in Myositis.

PURPOSE OF REVIEW: We aim to describe the immunoassays that have been used for myositis autoantibody discovery with a focus on newer methods. We describe recently identified myositis autoantibodies that do not yet form part of routine clinical testing, highlighting what is known about their associated clinical phenotype and potential clues as to their presence. RECENT FINDINGS: Novel approaches to autoantibody detection have been employed in recent years including chemiluminescent immunoassay, phage immunoprecipitation-sequencing and modifications to the more traditional immunoprecipitation t…

Patterns of recurrence in idiopathic orbital myositis.

PURPOSE: To characterise patterns of disease recurrence in idiopathic orbital myositis (IOM). METHODS: Multi-centre retrospective longitudinal study of IOM patients. Serial imaging was also analysed. Patients with incomplete clinical records and specific orbital myositis (e.g. thyroid-associated ophthalmopathy) were excluded. RESULTS: Thirty-three patients (Female: 18, mean age: 40.8 ± 16.8-years-old) presenting between 2001 and 2023. Twelve (36.4%) patients had disease recurrence. There was no difference between age or sex predilection of patients with and without recurrence (P = 0.107 and …

Aggressive atraumatic myositis ossificans in a toddler.

Myositis ossificans (MO) is a benign disorder where bone forms within muscles or other soft tissues. This condition usually follows trauma and is rare in pediatric patients. Here we present the case of a 2-year-old male who developed MO of his right elbow without obvious trauma to the area. Imaging of MO in the initial phase is highly unspecific and obtaining tissue samples through a biopsy can render misleading reports. In most cases MO is a self-limited process with complete resolution, however, some cases may present a diagnostic and therapeutic challenge.

Slicing and dicing myositis for cures and prevention.

Myositis is a group of conditions that vary greatly in risk factors, clinical manifestations, laboratory markers, presumed pathogenetic mechanisms, treatment responses and prognoses. Approaches to divide myositis into mutually exclusive and stable phenotypes are being considered, but are we thinking comprehensively enough in our attempts at classification?

Idiopathic orbital myositis in a pregnant woman.

A case of a 29-year-old woman who presented at 25 weeks' gestation with acute onset of painful diplopia and periorbital swelling is presented. Following further investigation, a diagnosis of idiopathic acute lateral rectus myositis was established. Her condition resolved following a 4-week course of oral prednisolone without recurrence. A healthy female was delivered at 40 weeks' gestation. The presenting features, differential diagnosis, treatment and course of orbital myositis are discussed.

Necrotising myositis - learnings for a plastic surgeon.

BACKGROUND: Necrotising myositis (NM) is a life-threatening emergency. Prompt treatment is associated with more favourable outcomes, but early diagnosis is challenging. The initial absence of cutaneous signs and symptoms coupled with delayed recognition commonly result in higher rates of morbidity and mortality. OBJECTIVES: Analyse data regarding demographics, epidemiology, aetiology, clinical manifestations, diagnosis and treatment of previously reported cases. This publication is intended for plastic surgeons in training to help them look out for this disease. SEARCH METHODS/CRITERIA: Publ…

[Muscle weakness and CK elevation: is it myositis?].

Rheumatologists are often confronted by patients with muscle weakness and elevated creatine kinase (CK) levels. Myositis cannot always be determined to be the cause of the complaints. This article presents two cases from our hospital where the diagnosis could only be determined by muscle biopsy. In the first case the patient presented with muscle weakness, pathological weight loss and a significant increase in CK levels. A muscle biopsy revealed an immune-mediated necrotizing myopathy (IMNM) caused by anti-3-hydroxy-3-methyl-gulatryl-CoA reductase (HMG-CoA reductase) autoantibodies due to the …

Ossifying myositis in the infant. About a case.

Myositis ossificans is a benign disorder characterized by the formation of heterotopic bone in skeletal muscle or soft tissues. It is extremely rare in children, <1% of cases occur in children under 10 years. We present a 17-day-old boy that, after 10 days of Intermediate Care Unit stay, was referred to our hospital for a developmental dysplasia of the hip. On clinical examination, he had swelling on the left thigh and increase in size compared to the contralateral one, therefore was admitted for studying. Imaging findings including plain radiographs, CT, MRI and bone scintigraphy, as well as …

Myositis and myopathy of sarcoidosis: A case report.

Muscular manifestations of sarcoidosis are commonly found on biopsy but rare on correlated imaging. We present a rare case of a 36-year-old male patient with sarcoid myositis and image findings of active myositis in the lower back and pelvic girdle musculature. This case suggests considering sarcoidosis as a differential diagnosis in the setting of chest findings and new lower back and lower extremity weakness.
