Cardiac involvement in patients with myotonic dystrophy: characteristic features of magnetic resonance imaging.

Structural alterations of the myocardium, such as fibrosis and fatty infiltration, were observed in post-mortem examinations in patients with myotonic dystrophy, a familial multisystem neuromuscular disease with frequent cardiac involvement. To identify structural and anatomical abnormalities of the heart, 14 patients, aged 45 +/- 14 years, belonging to seven families, suffering from myotonic dystrophy were studied. Twelve-lead ECG, high resolution signal-averaged ECG, 24 h Holter monitoring, bidimensional echocardiography and cardiac magnetic resonance (MRI) were performed in all patients. At…

Fetal akinesia-hypokinesia deformation sequence (FADS) in 2 siblings with congenital myotonic dystrophy.

Two premature siblings described herein had clinical features comparable to the fetal akinesia-hypokinesia deformation sequence (Pena-Shokei … tivity for actin and desmin. Subsequent molecular genetic analysis revealed a maternal diagnosis of myotonic dystrophy. The retarded growth and maturation of skeletal muscle observed in the presented cases correspond with previous findings in neonatal myotonic dystrophy. A well-defined myopathy can thus result in the fetal akinesia-hypokinesia deformation sequence.

Myotonic dystrophy and hyperparathyroidism: association with neurofibromatosis and multiple endocrine adenomatosis type 2A.

Four patients with hyperparathyroidism associated with myotonic dystrophy have been identified. All were females aged between 2 and 45 years. They were from three separate families, with two related patients being mother and daughter. In addition, one patient had medullary carcinoma of the thyroid and was diagnosed as having multiple endocrine adenomatosis, type 2A; another had an unspecified thyroid carcinoma; a third patient had neurofibromatosis. Our data suggest that myotonic dystrophy may somehow be associated with one or more of these disorders of neural crest origin.

Unmasking Brugada ECG Pattern in Myotonic Dystrophy Type 2 With an ANK2 Variant.

BACKGROUND: Brugada syndrome has been reported in myotonic dystrophy type 1, whereas its association with myotonic dystrophy type 2 (DM2) remains largely unexplored. CASE SUMMARY: We report a patient with genetically confirmed DM2 who presented with an electrocardiogram showing a Brugada type-2 ECG pattern. Sodium-channel blocker testing unmasked a diagnostic type 1 Brugada pattern. Genetic analysis excluded SCN5A variants and identified a rare heterozygous ANK2 missense variant, a gene involved in cardiac electrical stability. CONCLUSION: This case expands the spectrum of Brugada phenotypes…

Modified Polycyclic Compounds Rescue Mis-splicing in Myotonic Dystrophy Type 1 Disease Models.

Myotonic dystrophy type 1 (DM1) is an autosomal dominant multisystemic disorder with no approved therapeutics targeting the disease mechanism. DM1 is caused by the expression of expanded CUG repeat RNA (CUGexp), which sequester the muscleblind-like (MBNL) family of RNA binding proteins leading to dysregulated alternative splicing and a host of downstream impacts. While previous studies showed that diamidines rescued DM1 dysregulated alternative splicing events, their potential was limited by toxicity and off-target effects. A new class of modified polycyclic compounds (MPCs), based on diamidin…

[Two cases of congenital myotonic dystrophy type 1 caused by DMPK gene variants].

Patient 1 was a male neonate who, at 3 hours of life, presented with respiratory distress, hypotonia, and ventilator dependence. Genetic tes … testing showed a DMPK gene CTG repeat expansion (12/>83). Both cases were diagnosed with congenital myotonic dystrophy type 1. Congenital myotonic dystrophy type 1 is a rare and severe genetic disorder with low survival. When characteristic clinical manifestations appear, genetic testing and family counseling should be performed as early as possible to guide future …

Bottlebrush polymer conjugates for enhanced antisense oligonucleotide therapy in myotonic dystrophy type 1.

Oligonucleotides are a promising class of genetic medicine for myotonic dystrophy type 1 (DM1), the most common adult-onset muscular dystrophy. However, poor muscle distribution of nucleic acid drugs following systemic administration has hindered drug development, and no curative treatment currently exists. DM1 pathology requires drug localization to the nucleus, where pathogenic mutant RNA is sequestered, posing additional challenges after cellular internalization regarding endosomal escape and nuclear uptake. Here, we show that a locked nucleic acid oligonucleotide targeting mutant CUG repea…

HSP90 inhibition partially rescues alternative splicing dysregulation in cell models of myotonic dystrophy.

Myotonic dystrophy type 1 (DM1) and type 2 (DM2) are multisystemic diseases caused by the expression of toxic expansion RNAs that sequester muscleblind-like (MBNL) proteins, resulting in extensive alternative splicing dysregulation. Given that there are no current disease-modifying treatments for DM, we sought to identify compounds that rescue the underlying splicing dysregulation. A medium throughput splicing screen utilizing DM1 patient-derived fibroblasts was developed and used to screen 1584 compounds from the NIH NCI Diversity Set VI, leading to the identification of macbecin II, an HSP90…

Therapeutic advances in type 1 myotonic dystrophy complicated with type 2 diabetes mellitus.

Myotonic Dystrophy (DM) is a hereditary muscle disorder characterized by progressive muscle weakness, myotonia, and multi-system dysfunction. Based on clinical and genetic features, DM can be classified into Type 1 (Type 1 Myotonic Dystrophy, DM1) and Type 2 (Type 2 Myotonic Dystrophy, DM2), with DM1 being the most common subtype in adulthood. Diabetes, a metabolic disease, is defined by persistent hyperglycemia, typically resulting from insufficient insulin secretion or impaired insulin action. Among the various forms of diabetes, Type 2 Diabetes (T2DM) has the highest prevalence, accounting …

Characterizing white matter hyperintensities in myotonic dystrophy type 1 through IVIM derived metrics.

Myotonic dystrophy type 1 (DM1) is a genetic disease which affects multiple systems including the central nervous system. One of its key neuroimaging biomarkers are white matter hyperintensities (WMHs). The clinical relevance of WMHs remains unclear, and treating them as a uniform entity may overlook their heterogeneous composition and spatial distribution. To assess WMHs heterogeneity we applied intra-voxel incoherent motion (IVIM) modelling, an advanced magnetic resonance imaging technique, to a cohort of 23 patients with DM1 (age [mean ± std]: 44.1 ± 11.9 years; M/F = 8/15). WMHs were disti…

Electrocardiogram vs Electrophysiological Study and Major Conduction Delays in Myotonic Dystrophy Type 1.

IMPORTANCE: For the prevention of sudden cardiac death in myotonic dystrophy type 1 (dystrophia myotonica; DM1), professional practice guidelines recommend pacemaker implantation in asymptomatic patients with a PR interval greater than or equal to 240 milliseconds and/or QRS duration greater than or equal to 120 milliseconds on electrocardiogram (ECG), or a His-ventricular (HV) interval greater than or equal to 70 milliseconds during electrophysiological study (EPS), as class IIa indications. OBJECTIVE: To determine which of these strategies-ECG or EPS based-is more effective in predicting ma…

microRNA-mRNA expression profiles in the skeletal muscle of myotonic dystrophy type 1.

OBJECTIVES: Myotonic dystrophy type 1 (DM1) is the most common muscular dystrophy in adults, yet there are currently no disease-modifying treatments. Disrupted miRNA expressions may lead to dysregulation of target mRNAs and dysfunction involved in DM1 pathogenic mechanism. METHODS: We used microarray platforms to examine the miRNA/mRNA expression profiles in skeletal muscle biopsies derived from DM1 patients and matched controls. Bioinformatics analysis and dual-luciferase reporter assay were conducted to provide insight into miRNA-mRNA regulatory networks altered in DM1. RESULTS: Twenty-thr…

Prevalence of cardiovascular implantable electronic devices in children with type 1 myotonic dystrophy.

INTRODUCTION/AIMS: Type 1 myotonic dystrophy (DM1) is a neuromuscular disorder of multiple organ systems with important electrophysiologic (EP) manifestations, leading to a cumulative incidence of sudden death of 6.6%. Due to genetic anticipation, there is a pediatric subset of this patient population. However, most EP research on DM1 patients has been in adults, making cardiac care for pediatric patients difficult and directed by adult guidelines which often leads to cardiovascular implantable electronic device (CIED) implants. We sought to investigate the prevalence of CIEDs in the pediatric…

Calcium handling abnormalities increase arrhythmia susceptibility in DMSXL myotonic dystrophy type 1 mice.

BACKGROUND: Myotonic dystrophy type 1 (DM1) is a multiorgan disorder with significant cardiac involvement. ECG abnormalities, including arrhythmias, occur in 80 % of DM1 patients and are the second-most common cause of death after respiratory complications; however, the mechanisms underlying the arrhythmogenesis remain unclear. The objective of this study was to investigate the basis of the electrophysiological abnormalities in DM1 using the DMSXL mouse model. METHODS: ECG parameters were evaluated at baseline and post flecainide challenge. Calcium transient and action potential parameters we…
