The Significance of Weight and Volume of the Resected Fibrofatty Tissue Specimen in Neck Dissection: A Preliminary Report.

International guidelines suggest the presence of more than 18 lymph nodes in the neck dissection (ND) specimen in order to characterize the procedure as adequate. However, the lymph node yield depends not only on the surgeon, but also on the pathologist and on patient factors. Surgeons skeletonize the neck structures after resection of the surrounding fibrofatty tissue. We investigated whether measurement of the weight and volume of the resected fibrofatty tissue could be used to isolate the surgeon's part and investigate its significance. In subjects undergoing ND for head and neck cancer, we…

Diagnostic Challenges of Aortic Dissection at 5200m- A Case Report Presenting as Neck and Back Emphysema.

BACKGROUND: cute Aortic Dissection (AD) is of great concern due to its high mortality rate. The probability of young patients without underlying diseases developing acute aortic dissection is relatively low. In extreme regions such as high-altitude areas, for patients presenting with atypical chest pain, it is necessary to not only consider life-threatening diseases such as aortic dissection and acute coronary syndrome, but also to rule out the interference of emphysema in the diagnosis. This case provides experience in the diagnosis, evacuation, and treatment of aortic dissection patients in

Complications of neck dissections in papillary thyroid carcinoma. A modified procedure to reduce parathyroid morbidity.

Lymphatic metastasis develops in approximately 20% to 80% of patients with papillary thyroid carcinoma (PTC). Clinically evident pathologic nodes should be treated with therapeutic neck dissection. The extent of lymph node dissection, based on predictable drainage patterns from PTC, includes central compartment dissection with nodal clearance of level VI, and modified radical dissection of the lateral compartment with removal of levels II through V. Neck dissection, though a reliably safe procedure, carries certain potential complications. The most significant morbidity following therapeutic ne…

Significance of neck dissection for the treatment of clinically-evident medullary thyroid carcinomas: A systematic review.

OBJECTIVE: The survival benefit of prophylactic lateral neck dissection in medullary thyroid carcinomas remains unclear; thus, recent clinical guidelines have deferred the recommendation of lateral neck dissection. This review is to assess the role of lateral neck dissection in treatment of clinically overt medullary thyroid carcinoma. METHODS: A meta-analysis was performed on full-text publications written in English sourced using Embase and Medline databases. Publications with data regarding lateral neck dissection and clinical outcome were included. RESULTS: The frequently performed centr…

A clinical review of 38 cases of cervical tuberculous lymphadenitis in Japan - The role of neck dissection.

OBJECTIVES: After tuberculous pleurisy, lymphadenitis arising from cervical lesion is the second most common form of extrapulmonary tubercul … chemotherapy alone. The three patients resistant to chemotherapy were successfully treated through neck dissection. Thirty-six cases (94.7%) were cured by chemotherapy or chemotherapy and surgery. CONCLUSION: Local therapy could prove effective in cervical tuberculous lymphadenitis patients who exhibit an inadequate response to drugs. The role of neck dissectio …

Perioperative evaluation of glycaemic status in neck dissection: a retrospective analysis at a single hospital centre.

Diabetes mellitus is generally considered a risk factor for impaired wound healing. This study aimed to evaluate the glycaemic status of patients undergoing neck dissection and describe its impact on postoperative outcomes, especially wound healing. A retrospective analysis was performed of the preoperative, intraoperative, and postoperative glycaemic data obtained from the medical charts of 60 adult patients who had undergone 64 neck dissections. Nine of the 64 procedures were performed in diabetic patients (14.1%). The average glucose values were: preoperative 5.99 ± 1.25 mmol/l, intraoperat…

Selective neck dissection as a therapeutic option in management of squamous cell carcinoma of unknown primary.

Carcinoma of unknown primary of the neck (CUP) is a metastasis presenting in one or more cervical lymph nodes, with no primary mucosal site identified. Retrospective case notes review of 25 consecutive patients (median age 55, 72% males) diagnosed as CUP who underwent neck dissection in a UK tertiary referral comprehensive cancer centre between 2000 and 2011. Median follow-up was 33 months. Nineteen patients underwent comprehensive neck dissections (six extended), six patients had selective neck dissection. Five year disease specific survival and regional recurrence free survival were 76 and 8…

Modified deep plane rhytidectomy with a lateral approach to the neck: an alternative to submental incision and dissection.

BACKGROUND: A "composite" type subplatysmal dissection with lateral advancement of the platysma-skin flap to lift the neck during rhytidectomy has been advocated by several authors. However, the limited subplatysmal dissection described in these techniques restricts the degree of mobilization of the flap. Therefore, most investigators rely on supraplatysmal dissection and medial plication of the platysma to achieve satisfactory cervical contours, especially in patients with difficult necks. METHODS: To allow for greater mobilization of composite neck flaps, the senior author (T.A.M.) extends

Lymphocyte-depleted thymic remnants: a potential diagnostic pitfall in the evaluation of central neck dissections.

The head and neck region is a frequent site for potentially confusing incidental benign findings not related to the primary pathologic process and having no impact on patient prognosis. Several lesions are thymic in origin. We report 3 cases of lymphocyte-poor thymic remnants identified in central cervical (level VI) lymph node dissections for unrelated benign and malignant pathology. In each, the rests were nearly completely composed of bland epithelial cells with rare admixed lymphocytes. These cells were immunophenotypically similar to thymic epithelial cells, although they differed in the

Is central neck dissection a safe procedure in the treatment of papillary thyroid cancer? Our experience.

BACKGROUND AND AIMS: The role of central neck dissection in the treatment of papillary thyroid carcinoma is debated. This retrospective investigation was undertaken to assess whether it augments total thyroidectomy morbidity. PATIENTS/METHODS: A total of 305 consecutive patients who had undergone total thyroidectomy for papillary thyroid carcinoma were divided into three groups: group A (n = 64) showed evidence of node metastases and received therapeutic bilateral central node dissection; group B (n = 93) showed negative nodes and received prophylactic ipsilateral central node dissection; gro…

Vertebral artery dissections after chiropractic neck manipulation in Germany over three years.

Vertebral artery dissection (VAD) has been observed in association with chirotherapy of the neck. However, most publications describe only single case reports or a small number of cases. We analyzed data from neurological departments at university hospitals in Germany over a three year period of time of subjects with vertebral artery dissections associated with chiropractic neck manipulation. We conducted a country-wide survey at neurological departments of all medical schools to identify patients with VAD after chirotherapy followed by a standardized questionnaire for each patient. 36 patient

A case of rupture of the internal jugular vein caused by postoperative infection of functional neck dissection.

A 68-year old diabetic man with gingival cancer of the lower jaw underwent resection of the mandible and functional neck dissection. Swabs of a postoperative wound infection revealed methicillin-resistant Staphylococcus aureus (MRSA). The wound was irrigated, and antibiotics administered. The pathogens isolated were sensitive to the antibiotics used, but the infection failed to respond to treatment. Bleeding ensued on the 14th postoperative day (#14POD), when the wound was opened to reveal thrombosis and rupture of the right internal jugular vein. The patient's condition improved after ligatio…

Selective neck dissections for squamous carcinoma of the upper aerodigestive tract: patterns of regional failure.

BACKGROUND: Surgeons have been using selective neck dissections in the treatment of squamous carcinoma of the upper aerodigestive tract for over 20 years. To date, no data is available that can answer the question "What are the patterns of failure in the neck following a selective neck dissection and is a selective neck dissection a reliable procedure for metastatic disease?" METHODS: To answer this question, the medical records of all patients with squamous carcinoma of the oral cavity, oropharynx, larynx, and hypopharynx treated at The University of Texas M. D. Anderson Cancer Center from J…
