Aseptic necrosis of the hamate: a case report.

The case of a 25-year-old man with osteonecrosis of the hamate is reported. He had pain and swelling in his right wrist. The diagnosis was accomplished with plain radiographs as well as with MRI. The case was treated surgically that included resection of the necrotic bone. The occured cavity was filled with autogenous cancellous bone graft. In addition, capito-hamate arthrodesis was performed. Histopathological examination following the operation demonstrated avascular necrosis of the hamate. The arthrodesis was obtained four months after the operation.

Atraumatic avascular necrosis of an index sesamoid.

Avascular necrosis of an index sesamoid is our topic. In literature we found a lot of papers published about Hallux and the thumb sesamoid. There were only few articles concerning index sesamoid pathology. But in contrast to these, there's no evidence for trauma, nor arthritis in the personal history of the case discussed. This pathology is rare. Diagnosis is difficult. Treatment by sesamoid resection was successful.

Incidence of oesophageal necrosis in an autopsy material.

The incidence of oesophageal necrosis was studied in 310 consecutive autopsies of patients more than 2 years old. The oesophagus and upper part of the stomach were excised and formalin-fixed in all cases suspicious of necrosis. Suspicious areas were taken out for microscopy. The necroses presented as dark coloured or black, punctate, striped or confluent areas, most frequently located in the lower third of the organ and usually stopping abruptly at the cardia. They were distinguished from autolytic changes by the presence of a vital reaction (i.e. fibrin exudation and presence of neutrophils).…

Diagnosis and treatment of acute tubular necrosis.

BACKGROUND: Acute tubular necrosis (ATN) is common in hospitalized patients, particularly in the intensive care unit. Over the past four decades, the mortality rate from ATN has remained at 50% to 80%. PURPOSE: To review recent studies of diagnosis and treatment strategies for ATN. DATA SOURCES: MEDLINE search for all clinical studies of therapies for ATN, supplemented by a review of the references of the identified articles. STUDY SELECTION: Prospective studies and major retrospective studies evaluating therapies for ATN. DATA EXTRACTION: Data on the study sample, interventions performed,…

[Air in the portal vein and necrosis of the ventricle].

BACKGROUND: Air in the portal vein is a serious sign, usually caused by enteric necrosis. METHODS: The case report presents a 69-year-old male who died shortly after arrival in our department. RESULTS: Abdominal CT scan showed hepatic portal venous gas, and thrombi in the superior mesenteric artery, the coeliac trunk and in the left iliac artery. Autopsy revealed a necrotic stomach but viable small bowel. INTERPRETATION: Differential diagnoses are discussed, and we conclude that the patient most probably died from ischaemic necrosis. In severely ill patients, hepatic portal venous gas is an…

Avascular necrosis of the hallux metatarsal head.

Avascular necrosis of the first metatarsal head is rare. Although idiopathic cases have been reported, AVN of the first metatarsal head is usually iatrogenic following surgical correction of hallux valgus using a distal metatarsal osteotomy with or without lateral soft tissue release. A thorough understanding of the delicate vascular anatomy of the first metatarsal head is essential when surgery is considered. Careful operative technique permits a safe combination of distal osteotomy and lateral soft tissue release. Because the intraosseous blood supply is completely disrupted with distal meta…

Avascular necrosis of the bone: the causes and the cures.

Injury to bone directly or indirectly influenced by the pathophysiologic effects of ischemia or various disease processes can have sensitive and dramatic effects upon intraosseous microvascular structures. The femoral head is a common area affected by avascular necrosis. The causes, cures, and staging management of this microcirculatory phenomenon will be presented.

The intraocular changes of anterior segment necrosis.

Anterior segment necrosis is an acute or chronic process occasioned by embarrassment of the blood supply of the anterior segment of the eye. In the acute form this vascular obstruction leads to severe corneal oedema, necrosis of anterior uvea, hypotony and cataract formation. Depression of aqueous humour formation accounts for severe reduction of glucose levels in corneal stroma and aqueous humour lasting for two days after cautery of the long posterior ciliary arteries (LPCA) in rabbits. Lactate levels are initially significantly elevated but return to normal after one week. Stromal hydration…

Avascular necrosis and vascular anatomy of the metacarpals.

Aseptic necrosis of the metacarpal head is extremely rare and has been described in association with systemic lupus erythematosus, steroid use, trauma, and other sites of bone infarction. The long metacarpal is the most commonly involved. The extrinsic and intrinsic vascular anatomy of the metacarpal is described in fifty metacarpal specimens. No definite conclusions can be drawn about anatomic variations in blood supply between the different metacarpals. However, in 35% of the specimens, a main arteriole in the distal epiphysis was absent, making these metacarpal heads solely dependent on sma…
