60 kidney biopsies from patients with minimal changes were investigated by light-, electron- and immunofluorescent microscopy. The clinical picture in 41 patients was nephrotic syndrome; 10, 6 and 3 patients had mixed, hematuric and latent forms, respectively. In 45 of 60 biopsies focal or diffuse deposits of immune complexes in basal membrane and glomerular mesangium were found. 15 cases proved to be immunonegative. Depending on various combinations of G, A, M-immunoglobulins, C3-fraction of complement, their location in the glomerulus and type of the immune complex fluorescence, one can say

In 22 (13 females, 9 males) of 224 patients presenting with nephrotic syndrome, onset occurred during the second decade of life. Lipoid nephrosis and its variants (nil, focal segmental, or focal global glomerulosclerosis and mesangial proliferative) accounted for the disease in the majority of patients (13 patients). In all with nil change (7 patients) and one of the 4 with mesangial proliferation the disease completely resolved with steroids. Forty-one percent of patients had nephrotic syndrome as the result of other forms of glomerulonephritis: 1 patient had nephrotic syndrome secondary to p

The problems in the diagnosis and especially the pathogenetic mechanism of IgA nephropathy are discussed and suggestions are made that this entity may not be a renal disease of primary renal immunogenetic origin but may be a disease of disturbed mesangial transport mechanism for IgA. Suggestions are made for intensive studies on the pathogenesis and differential diagnosis of focal glomerulosclerosis vs. minimal change disease (lipoid nephrosis) especially by immunohistology and T Cell sub-set abnormalities. It is suggested to study minimal change disease from the viewpoint of a T Cell immunode

This communication describes the electron microscopy (EM) study of the small arterial vessels in renal biopsies from 32 patients with a variety of glomerular and tubulointerstitial diseases. With the possible exception of lipoid nephrosis, all types of glomerular and tubulointerstitial diseases were associated with abnormal arterial vessels. The vascular abnormalities included variable thickening and irregularity of the basement membranes (sclerosis), excessive elastic tissue and collagen fibers, atrophic or necrotic smooth muscle cells, and electron-dense deposits. The EM of glomeruli and tub

The podocyte is a critical component of the glomerular filtration barrier. Injury to these specialized cells results in podocytopathies. A kidney biopsy will reveal focal segmental glomerulosclerosis or minimal change disease. These diseases can have variable clinical presentations and biopsy features, which will help classify them into primary or secondary podocytopathies and further guide treatment. Contrary to secondary focal segmental glomerulosclerosis, primary focal segmental glomerulosclerosis and minimal change disease require immunosuppression since spontaneous remission is rare and c

The 20th century unfolded many mysteries regarding renal pathology with the advent of ancillary techniques such as immunofluorescence (IF) and electron microscopy (EM) studies. EM plays a major role in confirming the routine and IF findings or may uncover new and unsuspected features. The aim of the study is to elucidate the role of ultrastructural findings in patients with medical kidney diseases on whom kidney biopsy was performed at the American University of Beirut Medical Center, between November 2018 and June 2019. A total of 188 renal biopsies were examined during the study period. EM c

AIMS: A subset of IgA nephropathy (IgAN) patients exhibiting minimal change disease (MCD) like features present with nephrotic-range proteinuria and warrants immunosuppressive therapy (IST). However, the diagnosis of MCD-like IgAN varied by reports. We aimed to identify the key pathological features of MCD-like IgAN.
METHODS: In this cohort, 228 patients had biopsy-proven IgAN from 2009 to 2021, of which 44 without segmental sclerosis were enrolled. Patients were classified into segmental (< 50% glomerular capillary loop involvement) or global (> 50%) foot process effacement (FPE) groups. We

Idiopathic nephrotic syndrome is the most frequent glomerular disease in children in most parts of the world. Children with steroid-sensitive nephrotic syndrome (SSNS) generally have a good prognosis regarding the maintenance of normal kidney function even in the case of frequent relapses. The course of SSNS is often complicated by a high rate of relapses and the associated side effects of repeated glucocorticoid (steroid) therapy. The following recommendations for the treatment of SSNS are based on the comprehensive consideration of published evidence by a working group of the German Society

Human brucellosis is considered a great example of the complexity of clinical manifestations possibly affecting multiple organs or systems. Renal manifestations of human brucellosis have been documented in few case reports and one case series. Herein, we present a case of Nephrotic syndrome (NS) due to minimal change disease in the course of acute brucellosis. A 53-year-old male farmer was admitted to our department with acute brucellosis and NS. Renal biopsy revealed minimal change disease. Combined treatment with prednisone (1 mg/kg), rifampicin (600 mg/day), and doxycycline (200 mg/day) was

The coinciding suffering of two or more autoimmune diseases has been occasionally reported during the last years. We report a multiple sclerosis case in a young male patient who presented minimal-change nephrotic syndrome with frequent relapses seven months later. New relapses have been prevented with cyclosporine therapy in a dose of 3,8 mg/kg bw/day associated to prednisone 10 mg/day during eighteen months. Documentation and investigation of such cases will help to the best understanding of the immunopathogenesis of those diseases.

A 14-year old girl with lupus erythematosus (LE) who initially showed as discoid lupus and followed with nephrotic syndrome one year later. Concomitant piroxicam and indomethacin were prescribed for her arthritis, which preceded two months before the onset of nephrotic syndrome. Histology from renal biopsy showed minimal-change nephrotic syndrome (MCNS). After continuous treatment for six months, nephrotic syndrome resolved completely and no signs of relapse were noted over 14 years of follow-up. For the children, it is rare to have both LE and MCNS simultaneously; sequela of nonsteroidal anti

Our study is aimed to reveal the frequency and clinical significance of the coincidence of two widely spread entities, e.g. minimal change disease (MCD) and IgA nephropathy (IgAN), claimed to be found in an overwhelming number in some Asian regions. We retrospectively analyzed clinical and histological data from 627 renal biopsies, performed in our department from January 2002 to January 2005 and completed electron microscopy in 112 specimens diagnosed as IgAN. The coincidence of IgAN and MCD was found in 8 patients (7.1%). The coincidence of IgAN and minimal change nephrotic syndrome (MCNS) c
