[Frequency of lethal congenital malformations. Experience supported by 600 autopsies].

Data analysis of 600 neonatal necropsies performed in our Center from 1978 to 1984, has shown that in 200 cases one or more lethal malformations were observed: the most frequent concerned the central nervous system (33 cases), the urinary tract system (28 cases) and the cardiovascular system (26 cases). A genetic determinism was considered in 127 cases (63,5%). Usually a genetic counselling follows neonatal death in our Center: necropsy findings are then most useful: the authors advocate the interest of a close collaboration between pathologists and geneticists.

Characteristic Imaging Feature of Cortical Dysgenesis in Tubulinopathy.

Tubulin-related cortical dysgenesis or tubulinopathies include a wide range of brain cortical malformations that result from defects in genes responsible for the tubulin protein. Tubulin is an integral part of the cell cytoskeleton, and the development of the central nervous system depends on it. We present a case with characteristic imaging features that suggested tubulinopathy.

Unusually prolonged survival and childhood-onset epilepsy in a case of alobar holoprosencephaly.

Alobar holoprosencephaly is one of the most severe congenital malformations of the central nervous system. Most affected infants are stillborn or have a very short life-span. The survivors can present with neonatal seizures and/or infantile spasms. We report on an unusually long-lived patient with alobar holoprosencephaly and minor facial dysmorphism, who developed generalized epilepsy during childhood.

[Congenital brain malformations].

CLINICAL ISSUE: Malformations of the central nervous system belong to the most common developmental disorders in humans. The clinical presentation of brain malformations is nonspecific including developmental delay, hypotonia, and/or epilepsy. The great heterogeneity concerning etiology, mechanisms of development and morphology is challenging for diagnosis and classification of brain malformations. Thereby recognizing specific malformations is essential for optimal patient management and prognostic evaluation. The aim of this article is to give an overview of several clinically relevant brain

[Diabetic feto- and embryopathies].

The present article gives an analysis of available literature and the authors' own findings. Perinatal mortality of progeny of mothers suffering from diabetes mellitus may be caused both by embryopathy and antenatal fetopathy. The authors did not observe diabetic gameto- and blastopathies and no indications to their existance was found in literature, although some investigators admit the possibility of genetic implication in the pathology of progeny of parents with diabetes mellitus. In the progeny of mothers suffering from diabetes mellitus among congenital malformations which allow further i

Interferonopathies: From concept to clinical practice.

The horror autoinflammaticus derived from aberrant type I interferon secretion determines a special group of autoinflammatory diseases named interferonopathies. Diverse mechanisms involved in nucleic acids sensing, metabolizing or the lack of interferon signaling retro-control are responsible for the phenotypes associated to Aicardi-Goutières Syndrome (AGS), Proteasome-Associated Autoinflammatory Diseases (PRAAS), STING-Associated Vasculopathy with Infancy Onset (SAVI) and certain forms of monogenic Systemic lupus erythematosus (SLE). This review approaches interferonopathies from the basic im

Systemic lupus erythematosus due to C1q deficiency with progressive encephalopathy, intracranial calcification and acquired moyamoya cerebral vasculopathy.

We report a female with infantile onset of systemic lupus erythematosus secondary to C1q deficiency, in whom we identified a novel homozygous mutation in C1qB. The patient developed a progressive encephalopathy associated with spasticity, and suffered several arterial ischaemic strokes. Cerebral imaging demonstrated acquired intracranial calcification and a cerebral vasculopathy reminiscent of moyamoya. This case demonstrates overlap with some features of Aicardi-Goutières syndrome which, like C1q deficiency, is a monogenic cause of inflammation involving dysregulation of the innate immune sys

Vascular myelopathies-vascular malformations of the spinal cord: presentation and endovascular surgical management.

Spinal cord vascular malformations are often a cause of spinal myelopathy. Endovascular surgical approaches to spinal cord vascular malformations have become an important adjunct and often the primary treatment of these disorders. Spinal cord vascular malformations may be divided into several categories based on their anatomy and location. They may present with a wide range of symptoms far removed from the vascular pathology. Early recognition and treatment can effect a better outcome and often reverse presenting neurologic deficits. Emerging magnetic resonance imaging techniques allow visuali

Congenital malformations and maternal consumption of benzodiazepines: a case-control study.

This study assessed potential teratogenic properties of benzodiazepine (BZD) intake during early pregnancy. Four neonatal diagnoses of congenital malformations (embryopathy and fetopathy, unspecified; unspecified congenital malformations of the nervous system; cleft palate and cleft lip; congenital malformations of the urinary tract) were selected. The authors' previous clinical experience had shown these diagnoses to be characteristic of infants born to mothers with excessive intake of BZD in early pregnancy. The selected diagnoses were present in 25 of 10,646 liveborn infants (2.3 per 1000)

A missed case of hereditary hemorrhagic telangiectasia: A case report.

Hereditary hemorrhagic telangiectasia is a rare autosomal dominant disorder characterized by abnormal blood vessel formation. When an abnormal vascular architecture affects the lungs and central nervous system, serious complications can occur. We report a missed case of hereditary hemorrhagic telangiectasia with pulmonary arteriovenous malformations and cerebral arteriovenous malformations. A 22-year-old Chinese female was taken to the emergency room because of unconsciousness. Emergency head contrast-enhanced computed tomography and transthoracic contrast echocardiography showed that she had

Capillary Malformation-arteriovenous Malformation Type 2: A Case Report and Review.

Capillary malformation-arteriovenous malformation syndrome is a rare genodermatosis with cutaneous capillary malformations and a risk of associated fast-flow malformations. We describe here a four-generation family with a novel heterozygous pathogenic variant in the EPHB4 gene (NM_004444.5 (EPHB4): c.2224G>C, p.(Ala742Pro)). A review of the literature retrieved 127 patients with capillary malformation-arteriovenous malformation syndrome and confirmed variants in EPHB4. Multiple capillary malformations were present in 114 (89.76%) patients, and 12 (9.44%) patients had a solitary capillary malfo

Molecular neuropathology of epilepsy-associated glioneuronal malformations.

Glioneuronal malformations (malformations of cortical development [MCD]) include focal cortical dysplasias (FCD) as well as highly differentiated glioneuronal tumors (i.e. gangliogliomas) and constitute frequent findings in patients with pharmacoresistent focal epilepsies. Tailored resection strategies evolved as promising treatment options and allow a systematic neuropathologic and molecular biologic examination of the epileptogenic area in these patients. The histopathologic appearance and immunophenotype of glioneuronal lesions are, however, characterized by numerous similarities and sugges

Neuronal expression of the Ccm2 gene in a new mouse model of cerebral cavernous malformations.

Cerebral cavernous malformations are vascular defects of the central nervous system consisting of clusters of dilated vessels that are subject to frequent hemorrhaging. The genes mutated in three forms of autosomal dominant cerebral cavernous malformations have been cloned, but it remains unclear which cell type is ultimately responsible for the lesion. In this article we describe mice with a gene trap insertion in the Ccm2 gene. Consistent with the human phenotype, heterozygous animals develop cerebral vascular malformations, although penetrance is low. Beta-galactosidase activity in heterozy

[Prenatal sonographic diagnosis of abnormalities and the significance of prepartum therapeutic procedures].

Between 1981 and October 1985, prenatal ultrasound examinations revealed 19 malformations of the central nervous system, 13 intestinal malformations, 15 malformations of the urogenital tract, four non-immunological cases of hydrops fetalis, two sacral teratomas and one cardiac malformation. In only one of eleven cases of hydrocephalus were all preconditions fulfilled for considering an intra-uterine ventriculo-amniotic shunt insertion. In bilateral obstructive uropathy the dynamics of amniotic fluid volume both before and after intra-amniotic administration of an infusion is decisive for any f

Supratentorial abnormalities in the Chiari II malformation, III: The interhemispheric cyst.

OBJECTIVE: Our clinical observations noted an interhemispheric "cyst," a protrusion of the posterior superior third ventricle, in fetuses with myelomeningocele (MMC). The study analyzed the frequency of this observation and features that influence its visualization. METHODS: We searched for cases of fetal MMC sonographically detected between 1999 and 2007. Intracranial findings were retrospectively reviewed with attention to the interhemispheric cistern. Additionally, we reviewed 25 fetuses without a central nervous system anomaly and 10 fetuses with ventriculomegaly but no MMC. RESULTS: Amo

Fatal malformations in Jamaica.

Babies with major malformations were identified during the Jamaica Perinatal Morbidity and Mortality Survey. They were found in 96 (8.6%) of 1112 perinatal and neonatal deaths coming to necropsy and in 28 (2.6%) of 1085 no so examined. The central nervous system was most commonly affected, followed by the renal, gastrointestinal, and cardiovascular systems in decreasing order of frequency. Many infants had abnormalities in more than one system and 10 malformation syndromes/sequences were identified. Although at the present time, major malformations make only a small contribution to perinatal a

[The congenital cervical dermal sinus. A clinical case report and review of the literature].

Congenital dermal sinus represents a dysraphic state of the central nervous system. It can show up throughout a defect of the midline, of both the skull and the spine. Usually, the malformation presents a fistulous tract, connecting the skin with the underlying tissues (hypoderm, paraspinal muscles, vertebrae and, in some cases, also the dura mater and spinal cord). The incidence of the malformation is quite low, 1:2,500 births; the cervical and dorsal locations represent the less common region. The malformation can remain undetected at the clinical examination because of the scarce cutaneous

Non-Isolated Neural Tube Defects with Comorbid Malformations Are Responsive to Population-Level Folic Acid Supplementation in Northern China.

OBJECTIVE: Comorbid congenital malformation of multiple organs may indicate a shared genetic/teratogenic causality. Folic acid supplementation reduces the population-level prevalence of isolated neural tube defects (NTDs), but whether complex cases involving independent malformations are also responsive is unknown. We aimed to describe the epidemiology of NTDs with comorbid malformations in a Chinese population and assess the impact of folic acid supplementation. STUDY DESIGN: Data from five counties in Northern China were obtained between 2002 and 2021 through a population-based birth defect

A simple method for gross examination of the brain in abortuses and macerated fetuses.

The use of maternal serum alpha-fetoprotein screening and sonography has led to an increase in the prenatal diagnosis of major central nervous system (CNS) malformations. Therefore, it is important to correctly identify the type of malformation for proper counseling. We describe an autopsy method--freezing and sectioning of the fetal head--that should increase the accuracy of diagnosis after second-trimester abortion.
