[The problem of acalculia: a study of a case].

1)The Authors describe a case of acalculia and emphasize some peculiar aspects, the study of which was facilitated by being the patient a mathematics teacher. 2) The patient was unable to perform arithmetic operations (addition and subtraction) but was able to carry our more complex and algebric operations. The acalculia was associated with constructional apraxia. 3) These findings are discussed and the hypothesis is suggested that simple mental arithmetic calculations need spatial operations which are not required for more complex mathematical reasoning and that the ability for calculation ma

What is the role of a full physical examination in the management of asymptomatic patients with late syphilis?

According to the British Association for Sexual Health and HIV guidelines, a full physical examination is recommended in patients with possible late syphilis. The aim of this audit was to review all cases of late syphilis diagnosed at our centre since 1994 to see if a full cardiovascular and neurological examination was documented and also to see what a full examination contributed to the management of asymptomatic patients. Of the 480 medical notes audited, 295 patients were asymptomatic of whom 288 (98%) had normal physical examinations; the rest were either not documented, declined or defau

[Neurofibromatosis type 1 in children. Experiences of the Gdansk Paediatric Oncohaematology Centre. Preliminary results].

INTRODUCTION: Neurofibromatosis type 1 (NF1) is a frequent genetic disorder of autosomal-dominant pattern. The incidence is about 1 per 3000 live births. Patients with NF1 are predisposed to malignancies including soft tissue sarcomas and leukaemias. The aim of the study was assessment of the most frequent symptoms on the basis of long term observation of children with NF1 and presentation of implemented diagnostic and therapeutic procedures. MATERIAL AND METHODS: In our department there are 149 children (71 boys and 78 girls) aged from 7 months to 18 yrs with diagnosed or suspected NF1. Each

A practical guide for examining and treating the diabetic foot.

Physicians can perform a quick, complete examination of the feet of patients with diabetes to prevent serious complications. The examination should focus on circulation, nerve function, musculoskeletal problems, and the skin. All patients should be urged to wear supportive, comfortable shoes, and to wash, moisturize, and examine their feet every day.

Neurological evaluation of the head injury patient.

Neurological evaluation of the head-injured patient should be rapid and as straightforward as possible. It should be performed in a manner consistent with all other evaluations of patients with similar injuries, so that complete, pertinent information can be obtained prospectively and retrospectively. The data should be recorded in a manner that ensures maximum accuracy and reliability of the initial and subsequent observations--a necessary feature if changes in the patient's status are to be recognized by the various treating personnel. The preceding sections give the essential elements of th

Retinal Vasculitis as an Initial Presentation of Atypical Neurosarcoidosis With Occult Central Nervous System Inflammation.

Purpose: To describe a case of retinal vasculitis as a presenting sign of atypical neurosarcoidosis with occult central nervous system involvement. Methods: A case report and literature review are presented, highlighting the role of the ophthalmic examination and the importance of early neurologic workup for diagnosis and treatment. Results: A 27-year-old woman presented with monocular blurry vision, central scotoma, and headache. Ophthalmic examination demonstrated retinal vasculitis bilaterally, and systemic steroid treatment was initiated. Further neurologic workup with brain magnetic reson

Clinical Characteristics and Delayed Diagnosis of Pediatric Patients Presenting to the Emergency Department With a Newly Diagnosed Central Nervous System Tumor: A Single Institutional Experience.

BACKGROUND: Due to the varied symptomatology and inconsistent features on neurologic exam, central nervous system (CNS) tumors are difficult … by demographics, location, or histologic subtype. Moderate degree of concordance was observed among neurologic examinations performed by ED physicians and neurologists. Delayed diagnosis (median delay = 3.5 [1-7] months) was seen in 14% of patients, 13 with primary brain tumors (11 hemispheric, 2 brain stem). Six children with delayed diagnosis of low-grade glial tumors …

Focal neurologic symptoms in panic attacks.

Among 350 referrals to a neurology service, 19 (5%) had panic attacks manifesting as focal neurologic symptoms. Diagnostic clues included multiple symptoms, normal results on neurologic examination, personal stress, dietary extremes, age 20-50 years, and symptom reproduction by hyperventilation. Focal neurologic symptoms should not exclude a diagnosis of panic attack.

Reassessing hourly neurochecks.

Following acute brain injury, frequent neurological examinations ("neurochecks") are commonly prescribed and form the cornerstone of many care protocols and guidelines (e.g., for intracranial hemorrhage). While these assessments are intended to identify and mitigate secondary injury, they may unintentionally contribute to additional injury related to neurocheck-associated sleep disruption. Data are lacking to define patterns of neurological decline following acute brain injury, as are data to define the short- and long-term consequences (e.g., neuropsychological sequelae) of frequent and prolo

Vestibular reflexes in essential tremor: abnormalities of ocular and cervical vestibular-evoked myogenic potentials are associated with the cerebellum and brainstem involvement.

This study utilized cervical vestibular-evoked myogenic potentials tests (cVEMP) and ocular vestibular-evoked myogenic potentials tests (oVEMP) to investigate the vestibulocollic and vestibuloocular reflex arcs and to evaluate cerebellar and brainstem involvement) in essential tremor (ET). Eighteen cases with ET and 16 age- and gender-matched healthy control subjects (HCS) were included in the present study. Otoscopic and neurologic examinations were performed on all participants, and both cervical and ocular VEMP tests were performed. Pathological cVEMP results were increased in the ET group …

CANVAS, a sensory neuronopathy to look for in ataxia.

Sensory neuronopathies name the degeneration of peripheral sensory neurons in dorsal root ganglia. Among the genetic causes, CANVAS could be the most frequent. CANVAS is a clinical entity associating cerebellar ataxia, sensory neuronopathy and vestibular areflexia due to biallelic expansions in RFC1. This study reports the 18 individuals with sensory neuronopathy tested for RFC1 expansion in our center. The clinical picture showed that chronic cough was a frequent sign beginning before the onset of other symptoms. CANVAS is an underestimated cause of late-onset sensory and cerebellar ataxia th

New, diagnostic flicker test for optic neuritis shows specific stages following disease onset.

BACKGROUND: Previously, results of the digital flicker test (DFT) have shown distinct patterns in acute optic neuritis (ON) and healthy eyes. We aimed to examine the diagnostic potential of the DFT in acute ON and to investigate the temporal development of the DFT response following ON while comparing with visual evoked potentials (VEP). METHODS: The DFT examines the subjective brightness of a flickering field, varied in 11 different frequencies from 0 to 60 Hz, compared to a steady field. Previous studies have indicated a pronounced darkness enhancement at medial frequencies in acute ON eyes

A Clinical Approach to Disease of Peripheral Nerve.

Peripheral neuropathy is one of the most prevalent neurologic conditions encountered by neurologists and nonneurologists. Geriatricians and primary care physicians often face the task of screening patients for early neuropathy when they have underlying conditions such as diabetes mellitus and evaluating patients who report new symptoms that suggest neuropathy. An understanding about different forms of neuropathies based on anatomic pattern and type of nerve fiber involvement and ability to perform basic neurologic examination reliably can help determine how to pursue further investigations and…
