Bladder neck involvement in non-muscle-invasive bladder cancer: risk implications and outcomes of BCG vs gemcitabine/docetaxel.

OBJECTIVES: To evaluate the impact of bladder neck involvement (BNI) on oncological outcomes in non-muscle-invasive bladder cancer (NMIBC) and compare outcomes between Bacillus Calmette-Guérin (BCG) and gemcitabine/docetaxel (Gem/Doce) intravesical therapies in this population. PATIENTS AND METHODS: We analysed consecutive patients with NMIBC who underwent transurethral resection of bladder tumour (TURBT) and received BCG or Gem/Doce from 2013 to 2023. BNI was defined as tumour presence at the bladder neck during TURBT. Recurrence-free survival (RFS) and progression-free survival (PFS) were a

Lymphoepithelioma-like carcinoma of the urinary bladder: a clinicopathologic study of 13 cases.

Lymphoepithelioma-like carcinoma (LELCA) of the urinary bladder is a rare variant of bladder cancer characterized by a malignant epithelial component densely infiltrated by lymphoid cells. It is characterized by indistinct cytoplasmic borders and a syncytial growth pattern. These neoplasms deserve recognition and attention, chiefly because they may be responsive to chemotherapy. We report on the clinicopathologic features of 13 cases of LELCA recorded since 1981. The chief complaint in all 13 patients was hematuria. Their ages ranged from 58 years to 82 years. All tumors were muscle invasive.

PD1-IL2v expands and induces effector CD8+ TILs, but not Tregs, in the BCG treated orthotopic non-muscle invasive bladder cancer model.

BACKGROUND: The combination of systemic immune checkpoint inhibitors with standard of care intravesical Bacillus Calmette-Guerin (BCG) therapy for non-muscle invasive bladder cancer (NMIBC) has shown potential in enhancing BCG therapeutic effects. However, challenges such as disease recurrence, progression, and immune-related toxicities remain a significant hurdle. Furthermore, the mechanism of action of BCG involves broad, non-specific immune activation. While this recruits bystander CD8+ and NK cells, it also triggers the undesired expansion of Tregs, which may ultimately hamper therapeutic

In-Situ Self-Assembling Multifunctional Peptide-Hydrogel Enables Efficient Bladder Cancer Therapy via Sustained Release of Bacillus Calmette-Guérin and Enhanced Immune Response.

Intravesical instillation of bacillus Calmette-Guérin (BCG) remains the standard-of-care immunotherapy for high-risk nonmuscle-invasive bladder cancer (NMIBC). However, its clinical efficacy is frequently compromised by rapid drug washout resulting from intermittent bladder voiding, whereas intensified dosing regimens are associated with severe adverse events, including tuberculous cystitis and hematuria. Herein, we report an enzyme-responsive peptide, D-Nap-GFFYp, that undergoes alkaline phosphatase (ALP)-triggered self-assembly into a hydrogel selectively at the NMIBC tumor site. Using tumor

[Outcome of Eight-Year-Experiences of BCG Intravesical Instillation Therapy in Patients with Non-Muscle Invasive Bladder Cancer in Asahikawa Medical University Hospital].

We retrospectively investigated the outcome of BCG intravesical instillation therapy for patients with non-muscle invasive bladder cancer. From 2011 to 2019, a total of 79 patients had received BCG intravesical instillation therapy for non-muscle invasive bladder cancer. Of these 79 patients, 62 were included in this study. The protocol of BCG intravesical instillation therapy was as follows: instillation once per week for 6 consecutive weeks as induction, followed by instillation twice (once per week for 2 consecutive weeks) at each of 4, 6 and 12 months after TUR-BT as maintenance. BCG compl

Liquid biopsy miRNA risk score in bladder cancer: Translational insights from patient-derived xenografts to prognostic stratification.

BACKGROUND: Bladder cancer (BlCa) surveillance requires lifelong invasive monitoring, imposing substantial patient burden and healthcare costs. This underscores the need for minimally-invasive precision medicine approaches. Herein, we investigated the circulating miRNome of BlCa patients to assess its clinical and translational utility. METHODS: miRNA-seq was performed on tumors and matched plasma samples from 15 patient-derived xenograft (PDX) mouse models. The screening cohort of the study consisted of 216 patients, and circulating miRNAs were quantified by RT-qPCR following 3'-end poly(A)-

Oncological Outcomes of Intravesical Bacillus Calmette-Guérin Versus Early Radical Cystectomy in Very High-Risk Non-Muscle-Invasive Bladder Cancer: A Retrospective Cohort From a Latin American Referral Center.

BACKGROUND: Very high-risk (VHR) bladder tumors represent a subcategory of non-muscle-invasive bladder cancer (NMIBC) with a high potential for progression to muscle-invasive bladder cancer (MIBC). Early radical cystectomy (ERC) is the standard treatment. However, for patients unfit for or refusing surgery, intravesical Bacillus Calmette-Guérin (BCG) immunotherapy remains a commonly used bladder-preserving approach. This study aimed to assess oncological outcomes in patients with VHR NMIBC treated with intravesical BCG or ERC. METHODS: We conducted a retrospective cohort study, including pati

Durvalumab in combination with BCG for BCG-naive, high-risk, non-muscle-invasive bladder cancer (POTOMAC): final analysis of a randomised, open-label, phase 3 trial.

BACKGROUND: Patients with high-risk non-muscle-invasive bladder cancer (NMIBC) often have recurrence or progression after transurethral resection of bladder tumour (TURBT) and subsequent BCG therapy. We aimed to evaluate whether 1 year of durvalumab with BCG could improve outcomes versus BCG alone for these patients. METHODS: This randomised, open-label, phase 3 trial enrolled patients aged 18 years or older with BCG-naive, high-risk NMIBC who underwent TURBT. Patients were randomly allocated (1:1:1) to receive intravenous durvalumab (every 4 weeks for 13 cycles) plus intravesical BCG inducti

Intravesical oncolytic virotherapy and immunotherapy for non-muscle-invasive bladder cancer mouse model.

OBJECTIVES: To test if intravesical instillation of both an anti-programmed cell death protein 1 (PD-1) inhibitor and an oncolytic reovirus would demonstrate a greater effect than either treatment alone, as non-muscle-invasive bladder cancer that is refractory to intravesical bacillus Calmette-Guérin can be treated by systemic anti-PD-1 immunotherapy and we previously demonstrated improved overall survival (OS) with six once-weekly instillations of intravesical anti-PD-1 in a murine model. Discordant fibroblast growth factor receptor 3 (FGFR3) expression between photodynamic diagnosis-positive and -negative bladder cancer lesions: An intra-patient paired analysis.

BACKGROUND: 5-Aminolevulinic acid (ALA)-based photodynamic diagnosis (PDD) improves the detection of non-muscle-invasive bladder cancer (NMIBC) during transurethral resection of bladder tumor (TURBT). However, some urothelial carcinomas exhibit no fluorescence (PDD false-negative; FN). The molecular background of FN lesions in human specimens remains insufficiently understood. Therefore, we performed an intra-patient paired comparison between PDD true-positive (TP) and FN lesions coexisting within the same bladder, focusing on immunohistochemical expression profiles, particularly FGFR3. The Antipsychotic Aripiprazole Induces Cytotoxicity in Bladder Cancer Cells While Preserving Urothelial and Bladder Function.

Aripiprazole (ARI), an atypical antipsychotic, has demonstrated anticancer activity in several malignancies and may be a candidate for drug repurposing as an intravesical therapy for bladder cancer, particularly non-muscle-invasive bladder cancer (NMIBC). This study evaluated whether brief, intravesical-like ARI exposure could induce cytotoxic effects in bladder cancer cells while preserving normal bladder structure and function. RT4 and T24 bladder cancer cells, together with non-malignant UROtsa urothelial cells, were exposed to ARI (1-300 μM) for 30 min or 2 h, and viability was assessed at

Comparative Evaluation of HMG Family Proteins and miR-106a-5p in Low-Grade Non-Invasive and High-Grade Muscle-Invasive Papillary Urothelial Carcinoma.

Urothelial carcinoma (UC) of the bladder exhibits low- and high-grade papillary forms with distinct prognoses. High mobility group proteins (HMGA1, HMGA2, HMGB1) and miR-106a-5p are involved in tumor progression, but their interplay in UC remains incompletely understood. The aim of this study was to compare the expression of these parameters in low- and high-grade papillary UC. Tissue samples from 80 patients (40 low-grade and 40 high-grade) undergoing transurethral resection or cystectomy were analyzed, with control samples consisting of tumor-adjacent tissues without histopathological altera

A stratified urine-based molecular diagnostic and prognostic model for non-muscle-invasive bladder cancer management.

BACKGROUND: Non-muscle-invasive bladder cancer (NMIBC) is characterized by a high recurrence rate requiring lifelong cystoscopic surveillance. Existing urine-based molecular assays mainly rely on mutations or methylation, which fail to capture large-scale genomic instability. Copy number variation (CNV) profiling offers complementary information on tumor evolution and aggressiveness, but its application in urinary diagnosis remains limited. We aimed to integrate CNV and DNA methylation signals from urinary DNA to establish a noninvasive and biologically informed stratified diagnostic model for

Factors influencing adherence to intravesical instillation therapies for non-muscle-invasive bladder cancer: a scoping review protocol.

INTRODUCTION: Non-muscle-invasive bladder cancer is often treated with repeated weekly intravesical instillations of chemotherapy or immunotherapy. The number of instillations received influences the risk of recurrence and progression; thus, treatment completion is crucial. However, previous research indicates that nearly half of patients do not adhere to treatment for various reasons. This scoping review aims to define the concept of treatment adherence to intravesical instillation therapies and identify and map the factors that influence treatment adherence in intravesical instillation thera
