Obstetric fistulae in southern Mozambique: incidence, obstetric characteristics and treatment.

BACKGROUND: Obstetric fistula is one of the most devastating consequences of unmet needs in obstetric services. Systematic reviews suggest that the pooled incidence of fistulae in community-based studies is 0.09 per 1000 recently pregnant women; however, as facility delivery is increasing, for the most part, in Africa, incidence of fistula should decrease. Few population-based studies on fistulae have been undertaken in Sub-Saharan Africa, including Mozambique. This study aimed to estimate the incidence of obstetric fistulae in recently delivered mothers, and to describe the clinical character

Predicting obstetric anal sphincter injuries in a modern obstetric population.

BACKGROUND: Perineal lacerations are common at the time of vaginal delivery and may predispose patients to long-term pelvic floor disorders, such as urinary incontinence and pelvic organ prolapse. Obstetric anal sphincter injuries, which are the most severe form of perineal lacerations, result in disruption of the anal sphincter and, in some cases, the rectal mucosa during vaginal delivery. Long-term morbidity, including pain, pelvic floor disorders, fecal incontinence, and predisposition to recurrent injury at subsequent delivery may result. Despite several studies that have reported risk fac

Systems factors in obstetric care: the role of daily obstetric volume.

OBJECTIVE: To evaluate whether relatively high-volume days are associated with measures of obstetric care in California hospitals. METHODS: This is a population-based retrospective cohort study of linked data from birth certificates and antepartum and postpartum hospital discharge records for California births in 2006. Birth asphyxia and nulliparous, term, singleton, vertex cesarean delivery rates were analyzed as markers of quality of obstetric care. Rates were compared between hospital-specific relatively high-volume days (days when the number of births exceeded the 75th percentile of daily

Obstetric antecedents of surgically treated obstetric brachial plexus injuries.

OBJECTIVE: To determine whether the anatomy of an obstetric brachial plexus lesion (OBPL) is causally related to the preceding obstetric history. DESIGN: Anatomical classification of the OBPL during reconstructive neurosurgical treatment in consecutive infants who had surgery for OBPL between 1986 and 1994 and relating these findings with the characteristics of the preceding birth. SETTING: De Wever Hospital, Heerlen, The Netherlands. SUBJECTS: All infants who had surgical treatment for OBPL between 1 April 1986 and 1 January 1994 (n = 130). RESULTS: An Erb's C5-C6 injury was preceded more

The inverted pyramid of care in modern-day obstetrics.

Traditional antenatal care consists of an accelerating frequency of visits with advancing gestational age, as the bulk of pregnancy complications occur in late pregnancy. The 'inverted pyramid of care' concept describes a focus on structured risk assessment in early pregnancy, to facilitate prevention and enhanced surveillance of those at increased risk of maternal, intrinsic fetal, and uteroplacental problems.Since publication of this concept in 2011, it has become clear that early pregnancy screening may be contingent, with a sub-population identified for re-screening in mid- or late pregnanc

[Gynecology, Obstetrics and Fertility - Reborn in the Digital Revolution].

In recent years, artificial intelligence (AI) has brought about a dramatic transformation in obstetrics, gynecology, and fertility medicine. AI, including methods such as machine learning, deep learning, and large language models (LLMs), is reshaping the way clinical data are collected, analyzed, and applied. Image processing, particularly through deep learning applications, significantly enhances the detection of fetal anomalies in prenatal ultrasound, the classification of embryo quality in fertility treatments, and the early identification of gynecological pathologies. The integration of ar…

Current State of Artificial Intelligence Model Development in Obstetrics.

Publications on artificial intelligence (AI) applications have dramatically increased for most medical specialties, including obstetrics. Here, we review the most recent pertinent publications on AI programs in obstetrics, describe trends in AI applications for specific obstetric problems, and assess AI's possible effects on obstetric care. Searches were performed in PubMed (MeSH), MEDLINE, Ovid, ClinicalTrials.gov , Google Scholar, and Web of Science using a combination of keywords and text words related to "obstetrics," "pregnancy," "artificial intelligence," "machine learning," "deep learni…

Clinical Prospects for Artificial Intelligence in Obstetrics and Gynecology.

In recent years, artificial intelligence (AI) research in the medical field has been actively conducted owing to the evolution of algorithms, such as deep learning, and advances in hardware, such as graphics processing units, and some such medical devices have been used in clinics. AI research in obstetrics and gynecology has also increased. This review discusses the latest studies in each field. In the perinatal field, there are reports on cardiotocography, studies on the diagnosis of fetal abnormalities using ultrasound scans, and studies on placenta previa using magnetic resonance imaging (…

Outcome reporting in cardio-obstetrics studies: A systematic review.

BACKGROUND: Although considerable variation in the reporting and definition of outcomes in cardio-obstetrics studies is acknowledged, the extent of this variation has not been documented. The primary objective of this systematic review was to highlight this variation and inform the development of a Core Outcome Set for studies on Cardiac disease in Pregnancy (COSCarP). METHODS: Medline, Embase, Web of Science and Cochrane Central databases were searched from 1980 to 2018 to identify all English-language publications on pregnancy and heart disease. Title/abstract screening and data extraction …

Acute kidney injury in obstetrics: Hospital-based cohort study.

Obstetric acute kidney injury (AKI) is a serious unsolved global health-care problem and is a significant contributor to the overall burden of AKI resulting in devastating maternal and foetal outcomes. We studied the characteristics of obstetric AKI and the factors related to its unfavourable outcome. A total of 110 patients developed AKI among 10,138 admission giving a frequency of 1.08%. The commonest risk factor was pre-eclampsia followed by haemorrhage and sepsis. Complete recovery of renal function occurred in 40.9%. However, 9.1% were left with end-stage renal disease. AKI due to sepsis,
