Paroxysmal tonic upgaze: A heterogeneous clinical condition responsive to carbonic anhydrase inhibition.

BACKGROUND: Paroxysmal tonic upgaze (PTU), defined as an involuntary upward movement of the eyes, has been considered as a benign phenomenon but may also be associated with ataxia and developmental delay. METHODS: We report eight children with PTU; six of them also exhibiting symptoms of ataxia and/or developmental delay. Treatment with carbonic anhydrase inhibition was offered to children with persisting and/or severe forms. RESULTS: Whole-exome sequencing and genome-wide array analysis (n = 7) did not reveal mutations in the three known genes associated with PTU (CACNA1A, GRID2, SEPSECS),

Slow reading in children with anisometropic amblyopia is associated with fixation instability and increased saccades.

BACKGROUND: Previous studies show slow reading in strabismic amblyopia. We recently identified amblyopia, not strabismus, as the key factor in slow reading in children. No studies have focused on reading in amblyopic children without strabismus. We examined reading in anisometropic children and evaluated whether slow reading was associated with ocular motor dysfunction in children with amblyopia. METHODS: Anisometropic children (7-12 years) with or without amblyopia were compared to age-similar normal controls. Children silently read a grade-appropriate paragraph during binocular viewing. The effectiveness of home-based pencil push-up therapy versus office-based therapy for the treatment of symptomatic convergence insufficiency in young adults.

PURPOSE: To compare the effectiveness of pencil push-up therapy (PPT) versus office-based vision therapy in patients with convergence insufficiency. MATERIALS AND METHODS: In this study, 60 students from Zahedan University of Medical Sciences with convergence insufficiency were selected. After determining their refractive error (with a retinoscope), near point of convergence (by millimeter ruler), near heterophoria (by alternate prism cover test), and positive fusional vergence at near (by prism bar), subjects were divided into two groups to receive PPT (at least three times a day for 5 minut

[Results of surgical treatment of Stilling-Türk-Duane retraction syndrome].

BACKGROUND: The typical signs of the retraction syndrome (Stilling-Türk-Duane) are abnormal head posture, marked limitation of abduction, slight limitation of adduction and convergence, which are always accompanied by retraction of the globe, narrowing of the palpebral fissure, vertical deviation and increased intraocular pressure upon adduction. All signs are explicable by the co-contraction of the horizontal muscles or the failing relaxation of the lateral rectus muscle. Abnormal synergistic innervation occurs when in aplasia of the abducens nerve the lateral rectus muscle receives abnormal

Oculogyric crisis in two patients treated with aripiprazole for chronic tics.

BACKGROUND: Oculogyric crisis and other acute dystonic reactions have been reported as a rare adverse effect of aripiprazole, a third-generation antidopaminergic medication used for the treatment of multiple neuropsychiatric conditions, including psychotic disorders, affective disorders, obsessive-compulsive disorder, and neurodevelopmental disorders. CASE DESCRIPTION: We document two cases (two females aged 16 and 22 years) diagnosed with a neurodevelopmental tic disorder (Tourette syndrome), who developed oculogyric crisis while taking medium/high-dose aripiprazole 20-30 mg daily as a first-

[Disorders of Simple Binocular Vision in Heterophoria and their Spectacle Correction].

This study deals with evaluation of binocular vision in group of young subjects without eye pathology. We examined at whole 68 subjects with average age 26 years, median 24 years. The sample was divided into two main groups. Group A contains subjects with far and near ortophoria (at whole 26 subjects = 38 %). Group B contains subjects with binocular vision disorder (at whole 42 subjects). One subject had strabismus. The most frequent non-strabismus disorder of binocular vision was convergence insufficiency (13 subjects = 19 %), simple esophoria (12 subjects = 17 %) and simple exophoria (8 subj

Convergence insufficiency prevalence in attention deficit and hyperactivity disorder children depends on the diagnosis criteria.

PURPOSE: Attention deficit and hyperactivity disorder (ADHD) is a neurodevelopmental condition commonly seen in children and adolescents, characterised by an increase in distractibility or inattention. Several studies have noted a higher rate of visual problems in this group, especially convergence insufficiency (CI), but when using different diagnostic criteria. The aim of this study was to evaluate visual function in ADHD children and non-ADHD controls to compare the different signs for diagnosis of CI. METHOD: In this prospective case-control study, a group of children aged 7-17 years, dia

An ocular motility conundrum.

Two siblings, an 11-year-old boy and a 7-year-old girl presented with bilateral symmetrical ptosis and limited eye movements. Having already been reviewed on a number of occasions by a variety of specialists in multiple hospital settings a diagnosis of their ocular motility disorder had remained elusive. We describe their cases, outline the differential diagnosis and review the investigations performed which were influential in finally making a diagnosis.

Congenital ocular motor apraxia.

PURPOSE: Congenital ocular motor apraxia is a rare disease characterized by defective or absent voluntary and optically induced horizontal saccadic movements. Jerky head movements or thrusts on attempted lateral gaze are a compensatory sign. Most affected children have delayed motor and speech development. Cases associated with systemic diseases, neurologic maldevelopment, metabolic deficits, and chromosomal abnormalities have been described. METHODS: Case report and review of the scientific literature. RESULTS: The authors describe the ophthalmologic, pediatric, and neurologic evaluations a

Neuro-Ophthalmologic Response to Repetitive Subconcussive Head Impacts: A Randomized Clinical Trial.

IMPORTANCE: Subconcussive head impacts have emerged as a complex public health concern. The oculomotor system is sensitive to brain trauma; however, neuro-ophthalmologic response to subconcussive head impacts remains unclear. OBJECTIVE: To examine whether subconcussive head impacts cause impairments in neuro-ophthalmologic function as measured by the King-Devick test (KDT) and oculomotor function as measured by the near point of convergence. DESIGN, SETTING, AND PARTICIPANTS: In this randomized clinical trial, adult soccer players were randomized into either a heading group or kicking (contr

Traumatic Brain Injury in Children: Do the Eyes Have It? The Orthoptic Evaluation of Traumatic Brain Injury.

Because the neuropathological changes caused by mild traumatic brain injury (mTBI) more often manifest as functional impairments than structural abnormalities, the clinical diagnosis of mTBI may rely too heavily on a combination of history and self-reported symptoms. The mechanism of injury in mild traumatic brain injury (mTBI) predicts that supranuclear pathways controlling eye movement systems would be vulnerable to damage, and diagnostic tests of these systems would be high-yield. In fact, tests of oculomotor function have proven to be highly sensitive in detecting neurological soft signs,

Case Report: Ocular Myasthenia Gravis Associated with In Vitro Fertilization Procedures.

SIGNIFICANCE: Ocular myasthenia gravis is a localized form of myasthenia gravis, which is a postsynaptic disorder of the neuromuscular junction that causes fluctuating weakness of extraocular muscles resulting from autoimmune mechanisms. In women with myasthenia, changes in sex hormone levels and administration of corticosteroids can trigger or worsen symptoms of myasthenia gravis. PURPOSE: To describe a case of seronegative ocular myasthenia gravis whose first symptom appeared a day after in vitro fertilization procedure. CASE REPORT: A 37-year-old woman suddenly developed mild ptosis and f

Impact of late effects on health-related quality of life in survivors of pediatric brain tumors: motility disturbance of limb(s), seizure, ocular/visual impairment, endocrine abnormality, and higher brain dysfunction.

BACKGROUND: Survivors of pediatric brain tumors are often affected by late effects, such as motility disturbance of limb(s), seizure, ocular/visual impairment, endocrine abnormality, and higher brain dysfunction, resulting from the disease and its treatment. Appropriate provision of supportive care will require understanding the effects of these experiences on survivors' health-related quality of life (HRQOL). OBJECTIVE: The aim of this study was to identify the relationships between late effects and specific aspects of the HRQOL of pediatric brain tumor survivors. METHODS: We distributed qu

Lymphocytic Hypophysitis Presenting as Bilateral Abducens Nerve Palsy: A Case Report.

Lymphocytic hypophysitis (LYH) is a neuroendocrine disorder characterised by autoimmune inflammation of the pituitary gland with varying degrees of pituitary dysfunction, visual field defects and ocular motility disturbance. The authors report an interesting case of a 50-year-old woman presenting with intermittent bilateral abduction deficits. Neuroimaging and histopathological findings are presented. To the authors' knowledge, this is the first report of recurrent horizontal binocular diplopia and complete bilateral internal carotid artery occlusion in association with LYH.
