Achieving ≥90% of the Age-Predicted Treadmill Time for Commercial Vehicle Operation Does Not Predict Future Cardiovascular Events.

BACKGROUND: Functional capacity (FC) has been shown to be a strong independent predictor of all-cause and cardiovascular (CV) mortality. Nomograms based on age and sex have been developed to standardise exercise times and FC during treadmill testing (TT). Many patients with prior CV disease perform TT as a requirement for commercial vehicle operation. The current recommendations state a level ≥90% of FC for age should be achieved during testing. Whether this level predicts future CV outcomes is not known. METHOD: Maximal exercise stress echocardiography was performed on 196 patients with prio

Lyon endoscopic submucosal dissection score: a preprocedure prediction model for operating time in colorectal endoscopic submucosal dissection.

BACKGROUND AND AIMS: Colorectal endoscopic submucosal dissection (ESD) is an effective curative technique for superficial colorectal lesions but remains technically demanding, with variable procedure times. This study aimed to develop and validate a preprocedure predictive model for colorectal ESD duration in the era of traction-assisted strategies. METHODS: We retrospectively analyzed prospectively collected data from patients who underwent colorectal ESD at Edouard Herriot Hospital (Lyon, France). The derivation cohort included 423 cases (January-December 2024), and an independent temporal

Institutional learning curve and factors of prolonged operation time of robotic distal pancreatectomy: An analysis of an initial 117 cases.

BACKGROUND: The granular methods by which centers can safely implement and effectively expand robotic distal pancreatectomy (RDP), including those related to appropriate patient selection during the learning curve period, remain unclear. This study aimed to verify that our strategic robotic surgical oncology program effectively implemented RDP as standard practice and to identify factors associated with prolonged operation time. MATERIALS AND METHODS: We performed a detailed analysis of the intraoperative and short-term outcomes of consecutive patients (October 2018-September 2023) undergoing

Within-laboratory SARS-CoV-2 real time PCR testing operations in Nepal: a simulation-based analysis.

BACKGROUND: COVID-19 has challenged entire health systems, including laboratories. To address the increasing demand for tests to inform the epidemiology of the disease and for case management purposes, many countries made significant investments to rapidly expand laboratory capacity for detecting SARS-CoV-2. In this study, we used a simulated laboratory environment, based on a model of operating laboratories in Nepal, to identify opportunities for improvement. METHODS: We developed a discrete event simulation (DES) model, based on data from and in collaboration with Nepali health authorities,

Patient acceptance, time efficiency, and inter-operator agreement in intra-oral scanning with fluorescence technology for caries detection.

BACKGROUND: Intra-oral scanners (IOS) have been widely used to obtain digital impressions for treatment planning and to enable a full digital workflow on virtual patients. Much less is known about the use of IOS for caries detection. This study focuses on the use of the scanners for caries detection and investigates the number of images, time and patient experiences as well as differences between operators. METHODS: Participants were recruited if they had caries present, their teeth were cleaned using a prophy brush and water and then scanned in a standardised manner. Some participants were sc

Outcome of Transfer Time Difference From Diagnosis to Operation Room in Acute Type A Aortic Dissection Complicated by Malperfusion.

BACKGROUND: There are few reports of time effect for postoperative outcomes in patients with acute type A aortic dissection (ATAAD) complicated by malperfusion syndrome (MPS), especially transfer time difference from diagnosis to operation room (OR). To elucidate whether time between diagnosis and OR might be a significant factor, this study evaluated surgical outcomes of ATAAD complicated by MPS. METHODS: Between October 2013 and June 2024, 159 patients with ATAAD underwent emergent aortic repair; 54.7% (87/159) presented with MPS, 45.3% (72/159) without MPS. Of 87 patients with MPS, 69.0% (

Reproducibility and concordance of Pedal Acceleration Time between students, residents and experts operators: Single-center prospective study.

Critical limb threatening ischemia (CLTI) is defined as peripheral artery disease (PAD) with rest pain, nonhealing wounds, or tissue loss. At this stage, this is a severe pathology with a high rate of ischemic events in the lower limbs and cardiovascular events. The ankle brachial pressure index (ABI) is the first line diagnostic method, but can be falsely high because of stiffened ankle arteries related to medial artery calcifications. The Pedal Acceleration Time (PAT) has been described as valid even in cases of diabetes or chronic renal failure. In order to evaluate this measurement in untr

Analyzing heterogeneity in biomarker discriminative performance through partial time-dependent receiver operating characteristic curve modeling.

This study investigates the heterogeneity of a biomarker's discriminative performance for predicting subsequent time-to-event outcomes across different patient subgroups. While the area under the curve (AUC) for the time-dependent receiver operating characteristic curve is commonly used to assess biomarker performance, the partial time-dependent AUC (PAUC) provides insights that are often more pertinent for population screening and diagnostic testing. To achieve this objective, we propose a regression model tailored for PAUC and develop two distinct estimation procedures for discrete and conti

Weight-bearing timing after the operative management of ankle fractures: Single center audit and critical appraisal of literature.

The postoperative weight-bearing policy following internal fixation of unstable ankle fractures varies greatly and is largely surgeon-dependent. While early weight-bearing is favoured for its superior functional and economic outcomes, late weight-bearing may be safer initially. However, consensus on the preferred treatment protocol is lacking. We audited practices at University Hospitals Leuven, Belgium, comparing them to existing literature to establish an updated protocol. In a retrospective study of 236 patients with unstable ankle fractures, we compared outcomes between early (EWB) and lat

The effects of operating height and the passage of time on the end-point performance of fine manipulative tasks that require high accuracy.

Sustained shoulder abduction, which results from an inappropriate worktable height or tool shape and long task hours, leads to an accumulation of muscle fatigue and subsequent work-related injuries in workers. It can be alleviated by controlling the table height or ergonomic tool design, but workers who are doing some types of work that require a discomfortable posture, such as minimally invasive surgery, cannot avoid these situations. Loads to the shoulder joint or muscles result in several problems, such as muscle fatigue, deterioration of proprioception or changing movement strategies of th

Can 3-Dimensional Printing for Calcaneal Fracture Surgery Decrease Operation Time and Improve Quality of Fracture Reduction?

We investigated whether 3-dimensional (3D) printed models can decrease operation time and improve the quality of reduction for calcaneal fractures. The study involved 48 patients with unilateral intra-articular calcaneal fractures, who were retrospectively case-matched according to Sander's classification, age, and sex. Group A (24 patients) was operated using 3D printed models as a preoperative and intraoperative tool, and group B (24 patients) was operated using standard techniques without 3D printed model. Operation time was significantly shorter for group A, compared to group B (82.3 ± 13.

Transposition of Great Arteries with Complex Coronary Artery Variants: Time-Related Events Following Arterial Switch Operation.

Coronary artery anatomy represents a challenging and, often, determining predictor of outcome in an arterial switch operation (ASO). Impact of specific coronary artery variants, such as single, intramural and inverted, on time-related events following ASO, is, yet, to be determined. We sought to compare early and late outcomes within the group of nonstandard coronary artery variants. Patients who underwent ASO from January 1995 to October 2010 were reviewed. Patients with coronary artery variants other than L1Cx1R2 ("standard" by Leiden classification) were included. Patients with single, intr
