The disc and the field in glaucoma.

The previous descriptions of the optic nerve head were reviewed. A study was done using discriminant analysis, in which an 85% separation was obtained by means of the discriminant function in a prospective study. The false positives showed a higher incidence of subsequent visual field defects from the true negatives. The pitfalls of the use of the optic nerve head appearance in the recognition of glaucomatous visual field defects were discussed.

ALK-positive anaplastic large cell lymphoma initially diagnosed as neurosarcoidosis in a 12-year-old girl.

We present a rare case of optic neuropathy due to anaplastic lymphoma kinase-positive (ALK+) anaplastic large cell lymphoma (ALCL) with optic nerve infiltration in a 12-year-old girl who presented with acute unilateral vision loss, diplopia, and headache after two prior hospitalizations at an outside facility for disk edema. She had a presumptive diagnosis of neurosarcoidosis and empiric treatment had been initiated with high-dose corticosteroids. Ongoing worsening of vision prompted presentation at our facility. Histopathological examination from a cervical lymph node biopsy revealed ALK+ ALC

Bilateral Optic Nerve Head Drusen Complicated by Choroidal Neovascularization Misdiagnosed as Papilledema and Neuroretinitis.

We report a case of bilateral optic nerve head drusen complicated by choroidal neovascularization (CNV) in the left eye at presentation. The presence of optic disc and macular edema in addition to exudation led to the misdiagnosis of neuroretinitis at an outside medical center. Swept-source optical coherence tomography (SS-OCT) and SSOCT angiography were critical in establishing the diagnosis and follow-up in a noninvasive manner. Secondary CNV associated with optic nerve head drusen responded well to intravitreal injections of anti-vascular endothelial growth factor in the left eye. Asymptoma

Primary open-angle glaucoma in blacks: a review.

Glaucoma is one of the leading causes of blindness worldwide. Primary open-angle glaucoma (POAG) is the most prevalent form of glaucoma and has a particularly devastating impact in blacks. In the black American population, POAG prevalence is estimated to be six times as high in certain age groups compared to whites. POAG is more likely to result in irreversible blindness, appears approximately 10 years earlier and progresses more rapidly in blacks than in whites. Racial differences in optic disk parameters have been reported and show that blacks have larger optic disks than whites. This findin…

Early detection of glaucomatous damage. I. Psychophysical disturbances.

In summary, automated perimetry and other psychophysical tests currently provide the most sensitive, reliable and reproducible means of clinically detecting early glaucomatous changes. Compared to optic disk and nerve fiber layer evaluation, such procedures provide a consistent, quantitative method of distinguishing between early abnormalities and normal variations.

[Misconceptions in diagnosis and treatment of papilledema caused by idiopathic intracranial hypertension].

Papilledema refers to the swelling of the optic disk due to increased intracranial pressure. In clinical practice, papilledema is often confused with optic disc edema. Idiopathic intracranial hypertension (IIH) is a major cause of papilledema and there are still misconceptions about the etiology of IIH, the classification and examination of papilledema, and the treatment of IIH. This article elaborates on the misunderstandings that may exist in the diagnosis and treatment of papilledema, in order to lay a foundation for the standardized diagnosis and treatment of papilledema in China.
