We present a surgical procedure for liver metastases with hepatic hilar invasion, which was originally developed for hilar bile duct cancer. A 64-year-old man with 8 metastatic nodules from colon cancer and hepatic hilar invasion received this technique, and lived for more than 5 years without any restriction in his life. The authors recommend this procedure for those patients in order to preserve their quality of life.

The ontogenesis of the endocrine pancreas has been a subject of controversy. The discussion essentially was about organogenesis and histogenesis of islets of Langerhans. Now, the endodermic origin seems well established by several experimental approaches. The variation of the aspects of the islets and of the number of endocrine cells are re-called as well as the functional activity during fetal life. Numerous neuropeptides have been found in endocrine pancreas, so the content of the different endocrine cell types is reviewed with respect to the classic hormones.

Infantile seborrheic dermatitis (ISD), a disease occurring in the first months of life, is an erythematosquamous skin disease of unknown origin. This article presents results of microbial studies in 20 patients with ISD. Isolation of candida in a high percentage may indicate a primary role of this micro-organism in the etiology of this disease. It is striking that this disease often starts after disturbing the microbial colonization of the intestinal tract. Often ISD develops during the transition of breastfeeding to humanized cow milk.

The need for a human-derived immune globulin to replace the equine antitoxins currently used in the treatment of botulism is well recognized. A small group of individuals who had received multiple immunizations with pentavalent botulinal toxoid were plasmapheresed for the purpose of collecting a botulism-immune plasma of human origin to be fractionated for the production of immune globulin. Human-derived immune globulin will offer the advantage over equine antitoxins of not inducing reactions to foreign protein and of having a prolonged effective half-life.

Subclavian artery aneurysm is an extremely rare condition with potentially life-threatening complications, including rupture and embolization. This condition is generally the result of atherosclerosis, medial degeneration, trauma, or infection. We report the case of an 83-year-old man who developed hemoptysis due to the rupture of a saccular aneurysm at the origin of the left subclavian artery. This case highlights the importance of early recognition and emergency management of subclavian artery aneurysms, which, despite their rarity, carry significant morbidity and mortality risks.

The relationship of pain to personality and the significance have been discussed. Pain may first be evidence of tissue damage, secondly it may be used as a means of communicating emotional distress to others and thirdly, it may be a means of manipulating others, expressing hostility or relieving guilt. The evidence is clear that an understanding of mental life and its relation to pain is important if this symptoms is to be dealt with effectively, irrespective of whether it is physical or psychogenic in origin.

Necrotizing fasciitis is a rare but life-threatening infection of the subcutaneous tissue. In addition to Group A streptococci, polymicrobial infection with various aerobic and anaerobic gram positive or gram negative bacteria is frequently observed. The present case illustrates that a high level of clinical suspicion is necessary for early diagnosis of this disease and confirms the value of computed tomography in patients with sepsis of unknown origin. Because of rapid progression of the disease early debridement of necrotic tissue is mandatory for survival.

In approximately 5% of the hypertensive population, hypertension is of a renovascular origin. Some of these patients may be cured of their disease by the modern treatment of transluminal dilatation of the stenosed renal artery. They no longer require hazardous surgery or life-long antihypertensive medication with its associated problems of side-effects, expense, and patient compliance. Decisions need to be made about which hypertensive patients should be investigated, how extensively, and the tests which should be performed to identify patients with this remediable form of hypertension.

Paraneoplastic pemphigus is a life-threatening autoimmune bullous disease associated with neoplasia, generally of lymphoid origin. Immunosuppressive therapy is often disappointing and there are only a few reports of patients surviving more than 2 years. These cases were generally associated with benign neoplasms. We report here the case of a patient with paraneoplastic pemphigus associated with non-Hodgkin B-cell lymphoma who had a surprisingly good response to systemic corticosteroids and remains free of lesions more than 3 years later despite progression of her neoplasm.

Spontaneous retropharyngeal hemorrhage from a cervical parathyroid adenoma is a rare complication of primary hyperparathyroidism. The necrotising fasciitis (NF) is a rare syndrome in hand surgery which is characterized by a severe course of the illness. Different kinds of bacterial species are responsible for the pathogenetic origin of the NF. As NF can be life-threatening for the patient, immediate surgical and systemic treatment must begin as soon as there is the slightest clinical suspicion. We report about two patients with NF of the hand, whereby the patients' lives were saved by radical, serial debridements, intensive medical therapy and selective antibiotic treatment.

Accessory soleus muscle is a rare condition which presents as a soft-tissue mass medial to the calcaneum and distal Achilles tendon. Though congenital in origin, it manifests in the second and third decades of life as a soft-tissue mass due to muscle hypertrophy. Patients may be asymptomatic or present with a painful ankle mass. It is important to be aware of this condition when interpreting CT or MRI of the ankle, which show characteristic findings of a normal muscle in an abnormal location.

The authors propose a new technique of bilateral ligation of the uterine arteries at their origin, in order to save the patient's life, the uterus and the adnexae in cases of refractory post-partum haemorrhage, especially associated with placenta praevia. To date, they have only performed this operation on cadavers at the first stage in their study. This technique could be performed surgically in most cases and presents certain advantages over the bilateral ligation of the uterine arteries at the level of the arch.

We report a HTLV-I positive infant, whose infection was confirmed by polymerase chain reaction. The infant presented with an acute, severe, generalized eczema, exfoliation and severe erythroderma that yielded to an acute proteic malnutrition and frequent staphylococcal infections, unresponsive to treatment, since the second month of life. Immunodeficiencies from other origin and other causes of erythroderma were ruled out. The histopathology studies and clinical course yielded to the diagnosis of infective dermatitis associated to HTLV-I. A review of the literature is performed.
