The importance of masticatory function during development, the aim of early therapies. Hypoplasia of the upper maxilla is a widespread condition which orthodontists often need to face during early treatment. Mechanical maxillary expansion (both rapid and slow) produce undiagnosed effects on the palatal suture and unwanted side- effects on the periodontium and alveolar process, as well as anchorage-teeth displacement, root resorption, bone fenestration. It is clear that early therapies need a non-mechanical and non-traumatic approach in the respect of the physiology and biology of one of the mo…

Currently, the use of oral and systemic forms of bisphosphonates is increasing dramatically in a large group of patients either in the form of anti-osteoporosis medications or as a part of a chemotherapeutic regimen for several malignant diseases. As adult orthodontic treatment has become more widely accepted in most orthodontic practices, orthodontists must be aware of the risks, benefits, and effects of bisphosphonates use on the patient's general health status, as well as on their orthodontic treatment outcomes. This review aims to discuss the use of bisphosphonates, the complications assoc…

Soft-tissue lasers have numerous applications in orthodontics, including gingivectomy, frenectomy, operculectomy, papilla flattening, uncovering temporary anchorage devices, ablation of aphthous ulcerations, exposure of impacted teeth, and even tooth whitening. As an adjunctive procedure, laser surgery has helped many orthodontists to enhance the design of a patient's smile and improve treatment efficacy. Before incorporating soft-tissue lasers into clinical practice, the clinician must fully understand the basic science, safety protocol, and risks associated with them. The purpose of this art…

AIM: Presentation of results following therapy of patients with different forms of mandibulofacial dysostosis.
MATERIAL AND METHODS: Case r … m congenital facial malformations such as mandibulofacial dysostosis demands an experienced team of orthodontists and maxillofacial surgeons as well as persistence on the part of the patients and their families if good esthetic and functional results are to be achieved.

Cleft orthognathic surgery drastically affects the facial profile, masticatory function, and sets the foundation for future lip revision and rhinoseptoplasty. A multi-disciplinary cleft team comprised of dentists, plastic surgeons, otolaryngologists, orthodontists, oral and maxillofacial surgeons, and speech pathologists can ensure optimal holistic outcomes. These patients often require a maxillary advancement with mandibular setback, correction of any existing cants, and possibly a genioplasty with genioglossus advancement to achieve an orthognathic profile without worsening obstructive sleep…

When maxillary canine emerges in an abnormal position, the so-called ectopic eruption, it can lead to issues that require a combination of surgical, orthodontic, pediatric, and restorative treatments. If the ectopic eruption causes damage to other teeth, they may need to be removed, and the remaining teeth repositioned and reshaped to achieve a functional and esthetic result. This process requires a team effort from pediatric dentists, orthodontists, radiologists, and surgeons and a knowledge of tooth anatomy, root positioning, and restorative techniques. This case report details the replaceme…

Orthodontists need to understand the orthodontic risks associated with systemic disorders. Axenfeld-Rieger syndrome (ARS) is a rare autosomal dominant disorder with genetic and morphological variability. The risks of orthodontic treatment in ARS patients have been unclear. Here we describe the correction of an anterior open bite in a 15-year-old Japanese female ARS patient by molar intrusion using sectional archwires with miniscrew implants. An undesirable development of external apical root resorption (EARR) was observed in all intrusive force-applied posterior teeth during the patient's orth…

When a patient presents with TMJ pain, it could be all too easy for the orthodontists to turn the other way claiming that their occlusion has no connection with their symptoms and that they are suffering from a biopsychosocial disorder. This abundantly illustrated article attempts to demonstrate that patients can appear to display a stable occlusion even though their TMJ situation is unstable. This can give rise to dental abrasion, articular adaptations and pain. Orthodontics allows us to treat patients in pain provided that the treatment plan takes into account the need to restore a stable TM…

The biennial symposium of the Education, Research and Development Group (ERDG) of the New Zealand Association of Orthodontists (NZAO) was held in Tauranga on August 5 and 6, 2005. Following a well-tested format the symposium considered the benefits and risks of orthodontic treatment, a timely topic in an age when the role of health-related professions is under increasing public scrutiny in New Zealand. The registration fee included a copy of the book Risk Management in Orthodontics: Experts' Guide to Malpractice, edited by Graber et al (2004). The findings reported here represent the consensus…

The inability of orthodontists to change the cant of the maxillary occlusal plane without surgical intervention is a limitation of orthodontic treatment. LeFort I osteotomy with asymmetric maxillary impaction is often used to correct this problem. However, canting caused by extruded teeth can be corrected easily with normal orthodontic appliances and skeletal anchorage. The correction occurs through intrusion of the extruded teeth on 1 side of the maxilla, thereby avoiding a more aggressive surgical approach. A 22-year-old Korean man with facial asymmetry and mandibular prognathism treated in …

A questionnaire was sent to orthodontists in order to analyse transpositions of teeth and their treatment. The results of this study confirm that most transpositions are maxillary canine/premolar transpositions. These transpositions were found to be often associated with other dental abnormalities (such as agenesis and peg-shaped laterals). Considerable evidences supports they are genetic in origin, but, in accordance with the demands of evidence-based science, further study is needed to prove it. The results suggest that most practitioners try to correct transposition despite the many difficu…

Why does the eruption of some incisors cease when overbite has reached 3 mm and continue in other cases? Eruption stops only when an opposing force confronts it; normally, this is what occurs when incisors erupt into end-to-end occlusion. Tongue thrusting between molar teeth plays an etiological role only when it interferes with vertical dimension. Incisal overbite diminishes when incisal inclination increases: orthodontists can eliminate incisal overbite by inclining the anterior teeth labially. But a low interincisive angle is no assurance against relapse, because retention, to be reliable, …

Obstructive sleep apnea and hypoapnea syndrome (OSAHS) is important to orthodontists for several reasons. The authors, after defining OSAHS and describing it briefly, review the literature dealing with the cranio-facial morphology of apnea patients. They report the results of their cephalometric study, which is based on an architectural analysis of 274 patients. Their facial characteristics, especially vertical dimension, are consistent with those displayed by mouth breathing children. Changes in respiratory functioning modify the mechanical behavior and the structures of the upper airways. Os…

A 56-year-old man was referred by his dentist to an orthodontist as part of an interdisciplinary treatment plan for a complete dental rehabilitation. The patient presented with a mutilated dentition with a restricted envelope of function and generalized severe tooth wear with loss of vertical dimension. The patient showed a history of multiple endodontically treated and subsequently extracted elements associated with unexplained symptoms until a neurologist diagnosed trigeminal neuralgia. The orthodontic treatment in which the restricted envelope of function was eliminated and the vertical dim

An 11-year-old girl presented at an orthodontist asking for the correction of the crooked position of her upper and lower front teeth. In addition, the patient was concerned about the abnormal shape and size of tooth 21. There was a disto-occlusion, crowding and tooth gemination of tooth 21 with the shape of a dens invaginatus. The tooth, moreover, was infected. Teeth 21 and 14 were extracted as part of the treatment. By means of a guided transposition, tooth 23 was brought to the extraction site of tooth 21. Treatment was performed with hybrid fixed appliance techniques.

Gingival recession of lower incisors in the young population has become more prevalent in association with the increased uptake of orthodontic treatment. The two main predisposing factors are thin gingival biotype and bone dehiscence caused by labially displaced roots. Management is sometimes challenging, and the achievement of a successful result requires close cooperation between the orthodontist and periodontist. This article proposes a decision tree, which includes both orthodontic and periodontal considerations, to help the clinician in making the correct diagnosis and choosing the most s

Orthodontics is the planned movement of teeth that can be carried thanks to the provoked resorption and apposition of osseous tissue. From the standpoint of the periodontium orthodontists should always be cognizant of the risk of iatrogenically weakening soft tissue status but at the same time they should consider that an improved character of the periodontium is one of the benefits that can be derived from mechano-therapy. These periodontal improvements, in the broadest sense of the term, can result from traditional overall therapy as well as treatment aimed solely at a local condition requir…

In a special article entitled, "Impacted Maxillary Canines: A Review," Samir E. Bishara states: "The ectopic eruption and impaction of maxillary permanent canines is a frequently encountered clinical problem. The diagnosis and treatment of this problem usually requires the expertise and cooperation of the general practitioner, the paediatric dentist, the oral surgeon, and the periodontist, as well as the orthodontist." We will review the etiology, diagnosis and treatment of impacted maxillary canines. A technique will be outlined that allows a competent orthodontic clinician to singly, efficie

Used alone, Lefort I procedures and surgical mandibular reduction and de-rotation do not always restore facial symmetry; but by first correcting mesiodistal disharmonies, the orthodontist makes it possible for the surgeon to achieve the best results. Sometimes, long term stability of these improvements is jeopardized when the occlusal plane gradually begins to tilt again and asymmetry reappears in the mandible. By using electromyographic analysis in a series of patients with severe transverse facial disharmony she has treated, the author shows the important role played by the muscles of mastic
