[Bone-anchored hearing aids and active middle ear implants].

This review is concerned with hearing implants, which are used in patients with hearing loss, who cannot be treated successfully with a conventional hearing aid. Among these implants, bone-anchored hearing systems (BAHS) can be used for conductive or mixed hearing loss, while active middle ear implants primarily are reserved for sensorineural hearing loss. For BAHS, active transcutaneous implants may replace percutaneous implants as a future first choice.

Piezoelectric Actuator with Frequency Characteristics for a Middle-Ear Implant.

The design and implementation of a novel piezoelectric-based actuator for an implantable middle-ear hearing aid is described in this paper. The proposed actuator has excellent low-frequency output characteristics, and can generate high output in a specific frequency band by adjusting the mechanical resonance. The actuator consists of a piezoelectric element, a miniature bellows, a cantilever membrane, a metal ring support, a ceramic tip, and titanium housing. The optimal structure of the cantilever-membrane design, which determines the frequency characteristics of the piezoelectric actuator, w

Prevention and Management of Complications in Otosclerosis Surgery.

Surgical management of otosclerosis is a relatively safe and effective procedure, with a high rate of hearing improvement and a low rate of undesirable outcomes and complications. Many of these potential complications are common to any middle ear surgery, but many are unique to surgery involving the stapes footplate. Preventing complications with stapes surgery and minimizing their impact may be achieved by preparation as well as surgical experience.

Magnetically driven middle ear ossicles with laser vibrometry as a new diagnostic tool to quantify ossicular fixation.

CONCLUSION: Information on the degree of incus fixation can be gathered by measuring the ratio of incus to umbo long process velocity through the ear canal. OBJECTIVES: To test a new method of quantifying partial ossicular fixation in an ear with an elevated tympanic membrane. METHODS: Measurements were made on four fresh-frozen human temporal bones. After elevating the tympanic membrane a small magnet was attached to the manubrium and an electromagnetic excitation coil was used to vibrate the ossicles. The vibration response of the tip of the incus long process and the umbo were measured be

[Preliminary functional results of tympanoplasty with palisade cartilage].

Total or subtotal reconstruction of the tympanic membrane with cartilage palisades counteracts the tendency to retraction observed in ears with functional problems of the tube or altered gas exchange of the mucosa, in contrast with "soft" autologous materials such as temporal fascia or perichondrium. Because of its low turnover, cartilage is more resistant to the prolonged absence of neovascularization from the periphery in (sub) total perforation making it more resistant to infection. However, the use of palisade cartilage has been questioned for the functional recuperation of the middle ear

Laser applications in ossicular surgery.

In the past 8 years, the author has used the laser in more than 2800 cases of tympanomastoid surgery. Of these, more than 600 involved middle ear ossicular surgery including otosclerosis, congenital deformities, and fixations of the ossicular mass for various reasons. During this time, there have been no complications, either short-term or long-term, associated with the use of the laser. There are many tasks the laser can perform with less trauma, more precision, and greater speed than conventional hand-held tools. The laser does not replace these tools in otologic surgery, but it does add an

[Stapes surgery for otosclerosis using a new titanium prosthesis with a superelastic nitinol clip: First experiences].

BACKGROUND: Coupling of the prosthesis to the long process of the incus is a critical step in stapes surgery. We report first experiences with the NiTiFLEX® stapes prosthesis, a further development of the Soft CliP® stapes prosthesis (Kurz, Dusslingen, Germany). Instead of pure titanium, the CliP® now consists of nitinol, a superelastic nickel-titanium alloy. This further reduces the pressure exerted on the long process of the incus, aiming to improve coupling and minimize the risk of incus luxation. METHODS: In a monocentric, retrospective cohort study, we evaluated air-bone gap reduction (4

Surgical and Audiometric Outcomes for Repair of Congenital Aural Atresia and Hypoplasia.

IMPORTANCE: Surgical repair of congenital aural atresia and hypoplasia (CAAH) is technically challenging. Long-term surgical and audiologic outcomes of atresiaplasty are incompletely understood. OBJECTIVES: To review the surgical outcomes for CAAH and analyze the hearing results. DESIGN, SETTING, AND PARTICIPANTS: A retrospective medical record review of CAAH outcomes was performed during an 11-year period from January 1, 2004, through December 31, 2014. The data analysis was undertaken from December 1, 2014, through January 31, 2015. The mean clinic follow-up time was 3.9 years, and the mea
