Heterotopic Ossification of the Peroneus Longus Tendon in the Retromalleolar Portion with the Peroneus Quartus Muscle: A Case Report.

Heterotopic ossification (HO) is an ectopic formation of the lamellar bone in the soft tissues. Some authors have previously reported HO or calcific tendinitis of the peroneus longus tendon at the level of the cuboid bone, while the HO of the peroneus longus tendon in the retromalleolar portion has not been reported. The purpose of this report is to describe clinical, radiological, and histological features of this rare ossification and its treatment. To the best of our knowledge, this is the first report presenting a case of HO of the peroneus longus tendon, which developed in the retromalleo

Incidence of Heterotopic Ossification Following a Multimodal Pain Protocol in Total Hip Arthroplasty With the Posterior Approach.

Heterotopic ossification (HO) is prevalent after total hip arthroplasty (THA). Oral nonsteroidal anti-inflammatory drugs and cyclooxygenase-2 (COX-2) inhibitors have reduced the incidence of HO; however, to the authors' knowledge, no studies have reported the incidence and severity of HO with a pain protocol highlighted by celecoxib in the pre- and postoperative period with a posterolateral approach. Between October 2014 and October 2015, a retrospective study was conducted of 687 consecutive primary THAs with minimum 1-year follow-up performed between January 2009 and December 2013. All patie

Corrective and reconstructive surgery in patients with postburn heterotopic ossification and bony ankylosis: an evidence-based approach.

Articular heterotopic ossification (HO) and subsequent bony ankylosis (BA) are infrequent yet devastating complications of severe burn injuries. These conditions are diagnosed clinically and confirmed by imaging. Patients then begin active physical therapy until cleared for surgery, which remains the standard of care. This study critically reviews the existing literature on the diagnosis, treatment, and surgical outcomes of HO/BA in severely burned patients and provides an evidence-based treatment algorithm. A comprehensive search for all the studies addressing HO and BA after burn injury was

Heterotopic ossification after surgery for fractures and fracture-dislocations involving the proximal aspect of the radius or ulna.

BACKGROUND: The objectives of this study were to (1) determine the prevalence of heterotopic ossification after surgery for fractures and fracture-dislocations involving the proximal aspect of the radius or ulna, (2) identify risk factors associated with the development of heterotopic ossification in these injuries, and (3) characterize the severity and location of the heterotopic ossification and the associated range of elbow motion. METHODS: From 2004 to 2008, 142 elbow fractures and fracture-dislocations involving the proximal aspect of the radius or ulna were treated surgically at our ins

Clinically applicable antianginal agents suppress osteoblastic transformation of myogenic cells and heterotopic ossifications in mice.

Fibrodysplasia ossificans progressiva (FOP) is a rare autosomal dominant disorder characterized by progressive heterotopic ossification. FOP is caused by a gain-of-function mutation in ACVR1 encoding the bone morphogenetic protein type II receptor, ACVR1/ALK2. The mutant receptor causes upregulation of a transcriptional factor, Id1. No therapy is available to prevent the progressive heterotopic ossification in FOP. In an effort to search for clinically applicable drugs for FOP, we screened 1,040 FDA-approved drugs for suppression of the Id1 promoter activated by the mutant ACVR1/ALK2 in C2C12

Sonographic diagnosis of neurogenic heterotopic ossification in patients with severe acquired brain injury in a neurorehabilitation unit.

PURPOSE: To illustrate the ultrasonographic (US) and power Doppler US (PDUS) features of neurogenic heterotopic ossification (NHO) in consecutive patients with severe acquired brain injury and to evaluate the role of bedside US in diagnosis of NHO. METHODS: Ninety-two consecutive patients with severe acquired brain injury underwent clinical and laboratory screening for NHO. In 6 of 92 patients, bedside US confirmed the clinical suspicion of NHO. US diagnosis of NHO was then confirmed by other imaging methods. RESULTS: The incidence of clinical NHO was 7% (9/92) with one patient having multif

Clinical observations on surgical details of resection of heterotopic ossification at the hip in traumatic brain-injured adult.

Heterotopic ossification (HO) complicated with neurovascular bundle can be a very challenging operation. Preoperative planning before any HO resection is imperative. Plans to reconstruct nerve or artery should be in place. A case study is presented that involved a large bone mass of HO in hip joint which enclosed the sciatic nerve. Preoperative planning, microsurgical techniques, and equipment necessary for this complicated surgical procedure are reviewed.

A sciatic nerve lesion secondary to compression by a heterotopic ossification in the hip and thigh region--an electrodiagnostic approach.

A sciatic nerve lesion secondary to compression by a heterotopic ossification is rare. Operative release of the encased sciatic nerve in some cases may restore the function of the nerve partially or completely. However, in some cases the injury may be permanent. An electrophysiologic study is very useful to determine the location and severity of nerve damage, including axonal loss, demyelination, or both. An electrophysiologic study can emphasize the portion of the sciatic nerve that has been involved the most (lateral versus medial or peroneal versus tibial). In some cases an electrophysiolog

Osteonecrosis after treatment for heterotopic ossification in spinal cord injury with the combination of surgery, irradiation, and an NSAID.

STUDY DESIGN: Case report. OBJECTIVE: Heterotopic ossification (HO) is a frequent complication in spinal cord injury (SCI) that is often difficult to treat. Although surgery may become necessary, operative resection has been associated with complications and poor outcome due to a high recurrence rate. Additional methods of treatment to reduce the recurrence rate have been developed, including post operative irradiation and NSAIDs. This article presents three patients, who developed an osteonecrosis of the femoral head after the combined treatment for HO of surgery, irradiation, and an NSAID.

C-reactive protein as an early indicator of the formation of heterotopic ossifications after total hip replacement.

The formation of heterotopic ossifications after total hip endoprosthesis implantation is a well-known complication. During the postoperative course laboratory parameters are subject to partial change due to the development of heterotopic ossifications. However, these changes occur relatively late at a time when the application of prophylactic precautions is usually already decided. Any meaningful prophylactic treatment, however, has to be initiated immediately after surgery. In a prospective study we assessed the postoperative C-reactive protein (CRP) levels in 95 patients twice after total h

[A case of heterotopic ossification in the parapharyngeal region connected with the C2 vertebra in a 9-year-old girl].

A rare case of extraskeletal ossification in the parapharyngeal region related to C2 cervical vertebra was observed in a child, without any history of trauma, inflammation or endocrinological disease. One-step surgical intervention was carried out from the side of the neck and pharynx making possible complete removal of the lesion. The girl was admitted to the hospital with dysphagia and breathing difficulties increasing in the last 6 months. During that time the child was observed in laryngological hospital departments. After the operation Horner syndrome and hypoglossal paresis developed, bu

Heterotopic ossification as a complication of the staged total thigh muscles flap in spinal cord injury patients.

Between 1980 and 1990, 24 total thigh flap procedures were performed at Rancho Los Amigos Medical Center (Downey, CA) by the Pressure Ulcer Management Service. An unexpected occurrence was identified, that is, the rapid development of heterotopic ossification (HO) occurring in the exposed muscle flap between the first and second stages. There were 15 two-stage total thigh flap procedures on 14 patients performed between 1980 and 1990. Of these 15 flap procedures, 11 in 10 patients were found to have HO evident at the second-stage debridement/closure. In comparing our findings with those in oth

The role of post-operative radiation in the prevention of heterotopic ossification in patients with post-traumatic acetabular fracture.

Heterotopic ossification (HO) with subsequent pain and limitation of motion of the lower extremity is a common and significant problem for patients who suffer traumatic acetabular fracture (TAF). The incidence of heterotopic ossification is markedly increased for patients requiring surgical repair depending on the degree of trauma and the type of surgical repair necessary. Radiation therapy (RT) has proven to be the most effective surgical adjunct for the prevention of heterotopic ossification in patients undergoing total hip replacement (THR), but has not been reported in patients with trauma

The role of muscle fascia in heterotopic ossification and maintenance of skeletal muscle integrity in fibrodysplasia ossificans progressiva.

The rare genetic disorder fibrodysplasia ossificans progressiva (FOP) is characterized by progressive heterotopic ossification (HO) of skeletal muscles and associated soft tissues. FOP is caused by a gain-of-function mutation in the type l BMP receptor ACVR1 (also known as ALK2) that renders the receptor inappropriately responsive to activin ligands. HO is associated with muscle destruction and compromised muscle regeneration, although little is known of the mechanistic relationship between these pathophysiological disease manifestations. In mouse FOP models, HO is experimentally induced by di

An inducible knock-in mouse model of fibrodysplasia ossificans progressiva shows spontaneous formation of heterotopic ossification.

Fibrodysplasia ossificans progressiva (FOP) is an ultra-rare genetic disorder caused by mutations in ACVR1, most commonly the R206H variant. These mutations lead to heterotopic ossification (HO) in soft tissues, such as muscles, tendons, and ligaments. While people with FOP appear healthy at birth, they progressively develop HO starting in childhood, resulting in severe disabilities. Heterotopic ossification can be triggered by injuries, flare-ups, or occur spontaneously, and currently, there are limited medical or surgical treatment options available. To address these challenges, we generated

Effectiveness of indomethacin in preventing Heterotopic Ossification: a systematic review and meta-analysis of randomized controlled trials.

BACKGROUND: Heterotopic ossification (HO) is a frequent complication of joint trauma or surgery, commonly occurring after hip replacements, acetabular or other joint injuries, or surgeries. Indomethacin has long been used to prevent HO and is considered the first-line therapy. However, its effectiveness and necessity for HO prevention are still debated due to mixed evidence about its efficacy and potential side effects. METHODS: Following PRISMA guidelines, this systematic review and meta-analysis evaluated randomized controlled trials using the PICO framework. Searches were conducted across

Fulminant Heterotopic Ossification of the Lower Extremity After Gunshot Injury and Blunt Trauma: A Case Report.

Traumatic heterotopic ossification (HO) of the lower extremity is relatively rare but is of major importance in clinical practice. They are defined as posttraumatic abnormal formations of bone within soft tissue outside of the skeletal system. This article describes the clinical case of a 31-year-old male patient who suffered 2 traumatic events within 12 months-a gunshot wound in the lumbar spine/gluteal region followed by a severe traumatic brain injury with intracranial hemorrhage in a traffic accident as a pedestrian. Clinically, the patient was bedridden because of complete stiffening of t
