Paget's disease of bone (PDB) is a skeletal remodeling disorder diagnosed primarily via radiographs. In long bones, the early lytic stage of the disease is characterized by flame-shaped or blade of grass radiolucent bone resorption beginning in subchondral bone with variable length of extension into the metadiaphysis, and the later stages show bone expansion, cortical thickening, and coarsening of the trabeculae [1, 2]. Despite the usually diagnostic features, other considerations with overlapping appearance include aggressive benign and malignant bone tumors [3]. Malignancy such as lymphoma c

Objective: To characterize the phenotypic spectrum of patients with frontotemporal dementia (FTD) carrying the P392L SQSTM1 mutation. Methods: We describe the clinical phenotype of three well-characterized probands carrying the P392L mutation, and review ten previously published FTD cases with the mutation. Results: All three cases were male with a presenile age of onset (52 or 64 years). Case 1 developed an amnestic-anomic syndrome followed by a behavioral variant of FTD (bvFTD). Case 2, with an autosomal dominant family history of dementia, developed a bvFTD associated with parkinsonism. Bot

Paget's disease of bone (PDB) is characterised by increased and disorganised bone remodelling leading to various complications, such as bone deformity, deafness, secondary osteoarthritis, and pathological fracture. Pain is the most common presenting symptom of PDB, but it is unclear to what extent this is due to increased metabolic activity of the disease, complications, or unrelated causes. We conducted a cross-sectional study of 168 people with PDB attending secondary care referral centres in the UK. We documented the presence of musculoskeletal pain and sought to determine its underlying ca

Imaging in patients with Paget's disease of bone is very important clinically to show the presence of Pagetic abnormalities, assess disease progression, and identify adversely affected structures throughout disease course. Abnormalities and progression may be seen on radiographs, computed tomography, magnetic resonance imaging, and nuclear imaging. Herein, we report a case Paget's disease of bone showing diffuse characteristic pathology using technetium-99m-labelled diphosphonate tracer in bone scintigraphy (nuclear imaging). This case emphasizes the ability of nuclear imaging to rapidly visua

Measurements of bone markers (BMs) in peripheral blood or urine are a pivotal part of bone research within modern clinical medicine. In recent years the use of BMs increased substantially as they can be useful either to diagnose bone (related) disease and to follow its natural history, but also to monitor the effects of interventions. However, the use of BMs is still complicated mainly due to (pre)analytical variability of these substances, limited accessibility of assays, variable cut-off values in different countries and laboratories and heterogeneous results with regard to clinical implicat

Paget's disease of bone (PDB) is a focal disorder of osteoclasts, leading to chaotic bone remodelling, and it is characterized by the presence of focal areas of excessive bone formation alongside with areas of focal bone resorption. The typical radiographic feature is the cotton wool appearance. To date, bisphosphonates are the mainstay of the treatment. We hereby report the case of a young woman presenting with mandible PDB, with a relevant diagnostic delay and mistakenly treated for five years with chronic oral corticosteroids. After our evaluation, the patient received treatment with intrav

Skull fractures occur when forces striking the head exceed the mechanical integrity of the calvarium. A depressed skull fracture is a break in a cranial bone with depression of the bone into the brain. A depressed fracture may be open (compound), with a skin laceration over the fracture, or closed (simple), when the overlying tissue is not disrupted. The association between simple depressed fracture of the skull and elderly is rare. Paget disease of the bone is a chronic disease characterized by the deposition of abnormal bone tissue, more fragile than normal bone. We report a case of a 92-yea

Neoplastic degeneration represents a rare but serious complication of Paget's disease of bone (PDB). Although osteosarcomas have been described in up to 1% of PDB cases, giant cell tumors are less frequent and mainly occur in patients with polyostotic disease. We recently characterized a large pedigree with 14 affected members of whom four developed giant cell tumors at pagetic sites. The high number of affected subjects across multiple generations allowed us to better characterize the clinical phenotype and look for possible susceptibility loci. Of interest, all the affected members had polyo

Paget's disease of the sternum is rare. In patients with sternal Paget's disease, sternotomy, and harvesting of internal mammary arteries may reveal problems because of a thickened sternum and substernal adhesions. Saphenous vein graft or other arterial grafts may be used. Especially in younger patients who received saphenous vein graft, a repeat revascularization operation, because of earlier saphenous vein graft degeneration, may be needed in subsequent years. During the repeat operation, opening and reclosing the chest will still be a major problem. In appropriate patients, an operation thr

We reviewed 13 cases of tibial Paget's sarcoma constituting 14% of all registered Paget's sarcoma cases of the Scottish Bone Tumour Registry between January 1947 and June 2004. Eleven patients were male, and in ten patients the tumour involved the upper half of the tibia. In all cases, the main presenting feature was progressively worsening pain followed by a mass in six, a pathological fracture in five and tibial bowing in four. In nine patients, a lytic lesion was seen radiologically. Histologically, there were three osteosarcomas and ten malignant fibrous histiocytomas. Limb ablation was ca

This study investigated the perceived risks and complications associated with total hip arthroplasty for Paget's disease. A total of 98 Charnley low-friction arthroplasties were performed on 76 patients, 27 men (37 hips) and 49 women (61 hips), whose average age was 67.4 years (range, 51-79 years). Intraoperative blood loss averaged 388 mL (range, 110-1,730 mL), and minor heterotopic ossification occurred in 24 hips (25%), with significant changes in 4 hips (4%). After average follow-up of 10.4 years (range, 5.5-20 years), 10 acetabular (10%) and 8 femoral (8%) implants had loosened asepticall

The most important features of bone scintigraphy in metabolic bone disease are its high sensitivity and its capacity to easily image the whole body. Although in the early stages of evolution bone scintigraphy may present difficulties in disease detection because of its usual discrete generalized increased uptake, as the disease progresses, bone scintigraphy has well-recognized appearances, and with the knowledge of the different patterns of abnormality its highest diagnostic value can be obtained. Currently, the main clinical value of bone scan in metabolic bone disease is the detection of foc

Distinctive clinical, radiologic and pathologic features are seen in Paget's disease of bone. These distinct features can be divided into three phases; initial phase, midphase, and late phase. The clinical features may vary from patients being asymptomatic (involving a single bone) to patients having severe, multiple bone involvement with systemic disease. Radiologically, there are unique features which can differentiate Paget's disease from other bone diseases. The radiologic features follows the histologic findings on light microscopy. The correlation of the different phases with the clinica

New data provided by dual-energy X-ray absorptiometry, together with discordances between Howarth's and Coutris' tables for estimating the percentage of bone tissue affected with Paget's disease in a given individual, prompted us to reevaluate the percentage of bone tissue contained in each segment of the skeleton. We weighed each bone in the skeleton of a 30-year-old male, calculated weight ratios from a collection of 48 bones, performed dual-energy X-ray absorptiometry measurements in 22 patients using a Hologic QDR 2000 apparatus, and obtained information from a company that supplies medica

Osteosarcomas rarely affect the skull, preferring the long bones. As at other sites, osteosarcomas of the skull may be classified chiefly as de novo, post-radiation and post-Paget cases. Plain films of the skull and, even more, CT and MRI are the key diagnostic procedures for this disease. The treatment is surgery plus chemotherapy, in some cases radiotherapy. We report an odd case of post-radiation and post-Paget osteosarcoma in an elderly woman whose sister had been similarly affected.
