Methylguanidine (MG) which is known as a uremic toxin, is synthesized from creatinine (Cre). We have clarified that active oxygen plays an important role on MG synthesis in vitro and in rat hepatocytes. On the other hand, hyperoxia is very injurious in various tissues, and it has been reported that active oxygen produced in hyperoxia plays an important role on the tissue injury. This study was performed to investigate the effect of hyperoxia on MG synthesis in vivo. The subjects in this study were patients who were treated by hyperbaric oxygen therapy (HBO). Serum Cre, MG, and urinary Cre, MG

Critical decreases in oxygen delivery (DO2) and mixed venous oxygen saturation (SvO2) are associated with anaerobic metabolism and, therefore, lactic acidosis. We studied 50 consecutive patients with sepsis and 50 consecutive patients with acute myocardial infarction (AMI) in whom the arterial blood lactate was greater than 1 mmol/L in order to determine critical thresholds of DO2 and SvO2. In both groups, critical values of DO2 or SvO2 associated with lactic acidosis could not be identified. The DO2 ranged from 136 to 811 ml/min.m2 and SvO2 ranged from 28% to 73% in the patients with sepsis.

The primary goal of extracorporeal membrane oxygenation (ECMO) is to provide time for the effective treatment of the underlying disease; it does not directly treat the disease itself but serves as a bridge to recovery or to definitive therapies such as heart or lung transplantation. This review article aims to examine the fundamental components and various cannulation strategies of ECMO in pediatric patients, as well as its indications, anticoagulation strategies, and potential complications.

Supplemental oxygen is a cornerstone intervention in modern clinical practice, widely used to correct hypoxemia in emergency, perioperative, and critical care settings. While oxygen therapy is lifesaving, accumulating evidence indicates that excessive oxygen exposure can induce significant pathophysiological disturbances, particularly within the cardiovascular and pulmonary systems. Hyperoxia (PaO2 > 100 mm Hg) promotes the generation of reactive oxygen species (ROS), leading to oxidative stress, mitochondrial dysfunction, and the activation of pro-fibrotic pathways. When combined with mechani

Like all medical treatments, hyperbaric oxygen (HBO₂) has known potential side effects. HBO₂ is provided in different clinical settings by providers and staff with differing levels of expertise. It is important to understand the potential side effects of HBO₂ in order to allow for consideration of risk and benefit in discussion with the patient who is considering HBO₂. In this chapter, we will describe and quantify these side effects. Hyperbaric oxygen therapy is the treatment of patients with 100% oxygen at higher than atmospheric pressure provided in either a monoplace chamber (single person

Retinopathy of prematurity (ROP) is a vision-threatening disease among very preterm newborns in high-income countries and also in gestationally older newborns in low resource settings. The main classic risk factor and purported causal culprit is exogenous oxygen, which regulates angiogenic growth factors in the two phases of the disease. Along these lines, the vast majority of animal studies use oxygen-induced retinopathy models, with very limited data coming from non-oxygen models. We make the case in favor of alternative etiological paradigms and call for more models that employ infection an

Despite the prevalence of furans in organic chemistry, their direct conversion into diverse heterocycles via oxygen transmutation remains a formidable challenge. This limitation is primarily attributed to the robust endocyclic C-O bonds, its aromaticity, and the lack of strategies to activate inert oxygen atoms. In this study, we report a lithium-mediated strategy that enables the divergent transmutation of furan oxygen atoms. By employing a mechanochemical ball-milling protocol, we achieve this transformation under mild and operationally simple conditions, significantly enhancing the efficien

PURPOSE OF REVIEW: Adequate patient oxygenation during head and neck surgery is challenging as patients may have complex underlying pathologies and patient comorbidities, coupled with the need for shared airway access. Recent advances such as high-flow nasal oxygen, double jet ventilation (low frequency superimposed on high frequency) and ultra-narrow tracheal tube ventilation, have expanded the oxygenation options available. However, case selection and multidisciplinary team work coupled with a comprehensive understanding and high-quality training remain key to the safe, effective delivery of

Extracorporeal membrane oxygenation (ECMO) technology has relative contraindications in haemorrhagic disease but remains an effective therapeutic option for the treatment of pulmonary haemorrhage associated with respiratory failure. We report a critically ill case of a child with pulmonary haemorrhage and respiratory failure due to unintentional injury that was successfully treated with ECMO running at a low dose of heparin. An anticoagulation strategy with appropriate doses of heparin may improve the success of ECMO techniques in the treatment of severe pulmonary haemorrhage.

PURPOSE OF REVIEW: Despite improvements in reperfusion and adjunctive therapies cardiogenic shock associated with acute myocardial infarction is still associated with a mortality of 40-50%. Therefore, mechanical circulatory support devices are increasingly used. One option is veno-arterial extracorporal membrane oxygenation (VA-ECMO). VA-ECMO can be implanted percutaneously and blood is actively pumped into a tubing system outside the body which also incorporates an artificial lung for oxygenation and removal of carbon dioxide and then sent back retrograde in the aorta. This review summarizes

INTRODUCTION: Temperature and oxygen are two factors that profoundly affect survival limits of animals; too much or too little of either is lethal. However, humans and other animals can exhibit exceptional survival when oxygen and temperature are simultaneously low. This research investigates the role of oxygen in the cold shock death of Caenorhabditis elegans.
METHODS: The survival of C. elegans populations in combinations of oxygen concentrations and was assayed. Additionally, the effect of cold acclimatization, mutations in the cold acclimatization pathway, compounds, and antioxidant prote

One of the most critical factors impacting healthspan in the elderly is the loss of muscle mass and function, clinically referred to as sarcopenia. Muscle atrophy and weakness lead to loss of mobility, increased risk of injury, metabolic changes and loss of independence. Thus, defining the underlying mechanisms of sarcopenia is imperative to enable the development of effective interventions to preserve muscle function and quality in the elderly and improve healthspan. Over the past few decades, understanding the roles of mitochondrial dysfunction and oxidative stress has been a major focus of

PURPOSE OF REVIEW: Extracorporeal membrane oxygenation (ECMO) is increasingly used to temporarily support patients in severe circulatory and/or respiratory failure. Echocardiography is a core component of successful ECMO deployment. Herein, we review the role of echocardiography at different phases on extracorporeal support including candidate identification, cannulation, maintenance, complication vigilance, and decannulation.
RECENT FINDINGS: During cannulation, ultrasound is used to confirm intended vascular access and appropriate inflow cannula positioning. While on ECMO, echocardiographic

Background: Despite the lack of evidence to support the use of palliative oxygen to relieve dyspnea at the end of life, its prescription is widespread and often supported by local and national practice guidelines. Objectives: The objectives of this study were (1) to determine to what extent oxygen prescriptions meet the proposed prescription criteria in our institution, (2) to examine the indication of individual prescriptions in relation to the severity of dyspnea and (3) to review the utilization of opioids in patients receiving palliative oxygen. Methods: Retrospective chart review of cance

PURPOSE OF REVIEW: The principle of optimizing oxygen delivery to the injured brain rests on the premise that both hypoxia and hyperoxia are important mediators of secondary brain injury and should be avoided. This rationale has prompted a move towards incorporating oxygenation endpoints into the management of neurocritical care patients, particularly those with traumatic brain injury. The present review will seek to describe clinical strategies to optimize oxygenation in the acutely brain-injured patient, drawing upon relevant physiologic principles and clinical data, where it exists.
