Aspiration cytology depends on skillful preparation of smears, which normally requires a trained cytotechnologist. Collection of aspirates into Saccomanno fixative permits preparation of high-quality smears by trained technologists no matter who does the aspiration. In children with an impalpable testis, laparoscopy is a valuable procedure that can be used in conjunction with inguinal and abdominal exploration to localize the testis. The instrumentation and technique of laparoscopy are described. If the testis is intra-abdominal, a Fowler-Stephens orchiopexy often is successful. Staging the Fowler-Stephens orchiopexy, with preliminary ligation of the testicular vessels in situ, followed 6 months later with transection of the vessels and orchiopexy, may improve the success of this technique by allowing collateral arterial flow to develop with minimal risk

Laparoscopy is a simple, safe and sure diagnostic method in order to visualize an abdominal testis. When there is no abdominal testis at laparoscopy, the vas deferens and the testicular vessels have to be looked for very carefully. When both these structures pass through the internal inguinal ring, operation can be limited to the inguinal region. The results of laparoscopy are reported in nine patients with impalpable testis.

Fracture of the carpal scaphoid usually results from a fall on an outstretched dorsiflexed wrist. Snuffbox tenderness is an important clue. Because the fracture line may not be visible on the initial roentgenograms, follow-up films are often required. Direct palpation of the distal pole of the scaphoid can enhance the evaluation of wrist pain.
