We report on a 40-yr-old man with both primary enteropeptidase deficiency and celiac disease. He suffered from severe intestinal malabsorption and growth failure as a child. Enteropeptidase deficiency was found and pancreatic enzyme replacement therapy resulted in a growth spurt. Enteropeptidase levels in his intestinal mucosa and intraluminal fluid remained very low throughout childhood and early adult life. Celiac disease was confirmed by characteristic abnormalities in tests of intestinal function and in mucosal biopsies, which recovered when he instituted a gluten-free diet. He remains cli

BACKGROUND: Gallstones represent a significant burden for health care systems worldwide and are one of the most common disorders presenting to emergency room. Ultrasonography, complete blood picture test and liver function tests are procedures of choice in suspected gallstones or biliary diseases. They are the most sensitive, specific, non-invasive and inexpensive tests for the detection of gallstones. Our main objective was to evaluate the relationship of ultrasonographic findings, hemolytic indices and liver function tests with gallstones.
METHODOLOGY: It was a prospective study carried out

INTRODUCTION AND IMPORTANCE: A hydatid cyst is a parasitic disease caused by the development of the larval form of Echinococcus granulosus (1). The liver and lungs are the most frequent locations, while pancreatic involvement is unusual (<1 %), even in countries where hydatid disease is endemic (2). We report a rare case of a hydatid cyst of the pancreas, revealed by acute pancreatitis. Through this observation and a review of the literature, we discuss the diagnostic and therapeutic challenges of this rare localization of a hydatid cyst.
CASE PRESENTATION: A 60-year-old man from rural southw

Background and Objectives: Functional dyspepsia (FD) often overlaps with Pancreatic Exocrine Insufficiency (PEI), leading to diagnostic delays. We aimed to evaluate the prevalence of PEI in patients presenting dyspeptic symptoms using the survey-based PEI test and to assess the clinical response to Pancreatic Enzyme Replacement Therapy (PERT). Methods: This study included 91 patients with PEI and 58 control subjects. PEI was evaluated using the PEI Patient-Reported Outcome (PRO) instrument and classified as mild, moderate, or severe according to the 18-item PEI test. Objective fat malabsorptio

The aim of this study was to investigate the role of the parasympathetic (cholinergic and peptidergic) nervous system in the regulation of exocrine pancreas function in piglets during their early postnatal development. The cholinergic and peptidergic regulatory pathways of exocrine pancreatic function were tested by the specific muscarinic receptor blocker 4-diphenylacetoxy-N-methylpiperidine-methiodide (4-DAMP) and bombesin, respectively. At the age of 2 weeks, piglets were surgically fitted with a chronic pancreatic duct catheter, a duodenal re-entrant cannula and a jugular vein catheter. Th

BACKGROUND: Alterations in cerebral cortex structure among patients with pancreatic diseases have been reported, yet the underlying causes remain unclear. This study employs Mendelian randomization (MR) to elucidate the causal effects of four pancreatic diseases (benign and malignant pancreatic cancer, acute and chronic pancreatitis) on cerebral cortex structure.
METHODS: This study utilized genome-wide association study (GWAS) summary data from 437,418 participants in the Finnish consortium to identify the predictive role of genetic factors in acute pancreatitis. Similarly, data from these 4

AIMS: Exocrine pancreatic abnormalities have been reported in type 2 diabetes mellitus (T2DM), but their clinical relevance remains uncertain. We assessed whether T2DM is associated with a clinically meaningful exocrine pancreatic phenotype.
METHODS: Adults undergoing endoscopic ultrasound (EUS) for non-pancreatic indications were prospectively enrolled. Patients with known or suspected pancreatic disease were excluded. Non-diabetic controls were frequency-matched to patients with T2DM by sex and age. EUS abnormalities, pancreatic body diameter, elastography, faecal elastase-1 (FE-1), symptom

The pancreatic endocrine tumors are uncommon neoplasms and are classified into non-functional and functional tumors. According to whether the secreted hormones are originated from pancreatic islet cells or not, the tumors are also classified into normotopic and ectopic tumors. Except for insulinoma, more than 60% of them reveal malignant behavior. The presence of endocrine tumor is diagnosed when patients develop a hormone-specific symptom, and the location of tumors are usually diagnosed by a combination of ultrasonography, computed tomography, magnetic resonance imaging and selective angiogr

OBJECTIVE: The diagnosis of autonomic neuropathy in diabetic patients is based on cardiovascular reflex tests. Since cardiac function may be affected by arteriosclerosis and cardiomyopathy in type 1 diabetes mellitus, alternative tests reflecting vagal nerve function, in other organ systems, are needed. In this study the pancreatic polypeptide (PP) response to a mixed meal was evaluated in healthy subjects and in recently diagnosed type 1 diabetic patients.
MATERIAL AND METHODS: The PP response was studied at different levels of the vagally mediated reflex arch by application of different sti

BACKGROUND AND AIM: Intestinal barrier dysfunction has been implicated in the development of infectious complications of acute pancreatitis. Nucleotide-Binding Oligomerization DomainContaining Protein 2 (NOD2) plays an important role in the proper functioning of intestinal defense mechanisms. Here, we investigated the frequency of NOD2 variants in patients with mild and severe acute pancreatitis.
MATERIALS AND METHODS: Groups 1, 2 and 3 comprised healthy participants and patients with mild and severe pancreatitis, respectively. Four NOD2 variants and serum interleukin-6 (IL-6), Tumor Necrosis

OBJECTIVE: Recurrent acute pancreatitis is an uncommon diagnosis in teenagers. Excluded alcohol and biliary stones, more prevalent aetiologies in these group of patients are genetic pancreatitis, pancreatic duct system abnormalities, neoplasia, traumas and congenital abnormalities of the duodenum such as duodenal duplication or diverticulum. Two reported cases of recurrent pancreatitis associated to midgut malrotation were described in English literature. Bowel malrotation is a difficult diagnosis in a teenager or a young adult and a common delay is documented. Ladd's procedure is largely reco

Insulinomas are rare functional pancreatic neuroendocrine tumors that typically manifest with classic hypoglycemic symptoms, such as diaphoresis, palpitations, and tremors. Although infrequent, neuroglycopenic symptoms associated with insulinomas have been reported, often leading to delayed diagnoses. Here, we present the case of a 31-year-old male with pancreatic insulinoma who experienced recurrent episodes of seizures and confusion preceded by diaphoresis, tremors, and palpitations. During these episodes, he was found to be hypoglycemic. Comprehensive evaluations, including brain and abdomi

The progression of diabetic retinopathy after combined pancreatic and kidney transplantation was studied in eight patients for 12 to 49 months. Four patients who had rapid pancreatic graft failure constituted a control group for comparison with four patients who retained functioning grafts. Using Fisher's exact probability test, the authors found no posttransplantation difference between the two groups in visual acuity lost, severity of diabetic macular edema, extent of capillary closure, progression of preretinal gliosis, development of disc or preretinal neovascularization, or worsening of t

Patients with normal or borderline sweat tests present a diagnostic challenge. In spite of the availability of genetic analysis and measurement of nasal potential difference, there is still uncertainty in diagnosing cystic fibrosis in some patients. CA 19-9 is a tumor-associated antigen whose levels were previously found to be elevated in some cystic fibrosis patients. We investigated whether serum CA 19-9 levels can contribute to establishing the diagnosis of cystic fibrosis in patients with a borderline sweat test, and evaluated the influence of different clinical variables on CA 19-9 levels

Hereditary pancreatitis is a rare genetic disorder, usually associated with gain-of-function mutations in the PRSS1 gene. While in the Indian pediatric population, the SPINK1 mutations are more frequently reported, making PRSS1 mutations, particularly the de novo variants, rare. An 11-year-old school-going Indian girl presented with recurrent acute pancreatitis beginning at the age of 5 years. This progressed to chronic pancreatitis by the age of 10 years. Metabolic, autoimmune, structural, and infectious causes were excluded. The genetic test done revealed a heterozygous pathogenic PRSS1 c.86

Objective: Type 2 diabetes mellitus (T2DM) is a chronic condition resulting from insulin resistance and insufficient β-cell secretion, leading to improper glycaemic regulation. Previous studies have found that excessive fat deposits in organs such as the liver and muscle can cause insulin resistance through lipotoxicity that affects β-cell function. The relationships between fat deposits in pancreatic tissue, the function of β-cells, the method of visceral fat evaluation and T2DM have been sought by researchers. This study aims to elucidate the role of pancreatic fat deposits in the developmen

OBJECTIVE: To examine the relation between hepatic reticuloendothelial system (RES) dysfunction and the development of acute biliary pancreatitis. In an opossum model, the authors tested the hypothesis that RES blockade can turn the mild pancreatitis seen after pancreatic duct obstruction (PDO) into the severe form.
SUMMARY BACKGROUND DATA: Biliary obstruction is considered the decisive event in gallstone pancreatitis. Suppression of the RES occurs during biliary obstruction.
METHODS: Eighteen opossums were placed into three groups of six animals each: group A, RES blockade with lambda-carra

Bioengineering approaches to regulate stem cell fates aim to recapitulate the in vivo microenvironment. In recent years, manipulating the micro- and nano-scale topography of the stem cell niche has gained considerable interest for the purposes of controlling extrinsic mechanical cues to regulate stem cell fate and behavior in vitro. Here, we established an optimal nanotopographical system to improve 3-dimensional (3D) differentiation of pancreatic cells from human pluripotent stem cells (hPSCs) by testing gradient-pattern chips of nano-scale polystyrene surface structures with varying sizes an

Fine structural studies of human pancreatic adenocarcinomas have demonstrated their composition of one major secretory cell type, a mucin secreting cell. It was also demonstrated that pancreatic tumor cells have a high endocytotic activity via the regular endocytotic pathway with uptake via coated pits and vesicles. These findings indicate a striking similarity between pancreatic tumor cells and human mononuclear phagocytes. Therefore monocyte-related functions like endocytosis, lysosomal enzyme secretion and superoxide anion production were measured in various cell lines of human pancreatic a
